Sheran Daniele, Founder & Artistic Director enters her 45th year in dance and music education, joined by her highly qualified professional staff, whose resumes range from Broadway to American Ballet Theater, Julliard to Joffrey, Ballet Nacional de Cuba and Korean National Institute of Arts. Ballet Arts offers an extensive range of education in private voice and instrument lessons plus Ballet, Jazz, Tap, Irish Step, Hip-Hop, Boy’s Classes, Modern, Creative Movement, Story Time Dance, Character Dance, Latin/Salsa, and Adult Classes. Classes are available for students age 2 to Adult in every skill level, beginner to advanced. Placements are at discretion of the director.

ENROLLMENT IN OUR PROGRAM IS FOR THE FULL SCHOOL YEAR; SEPTEMBER TO THE RECITAL(TYPICALLY THE FIRST OR SECOND WEEKEND IN JUNE)

Tuition Payment Schedule: Tuition is due in three parts; one third at registration, one third by November 15th and one third by January 15th. Tuition is Non-Refundable.

Full Tuition Payment at Registration receives 5% off your total. Two(2) or more styles of dance taken per week per student earn 5% tuition discount (applied to the 3rd payment) or paying in full receive another 5% off immediately. Excludes private, duo and trio lessons, 10 consecutive week classes, extra pointe classes, and Junior and Senior companies. The installment plan is offered only as a convenience and does not represent a portioned, partial or temporary enrollment in our program.

Due to the very limited number of available instructor hours and high demand for private

instruction, 24 hour notice of cancellation is required. Otherwise you will

be charged your lesson tuition fee.

Included in the tuition is the registration fee, administrative fee and performance fee valued at

$150 per student for school year tuition. $50 for the 10 week sessions.

$25 for the 10 week sessions with no performance. This portion of the tuition is not pro-rated.

CHILDREN’S BALLETWe are specialists in youth ballet. Our teaching methods emphasize the basics of movement and the language of dance following the Russian Vaganova training system, emphasizing precision and control. He is a member of the BMI-Lehman Engel Theatre Workshop and the Dramatists Guild. He is certified to teach the International Baccalaureate Theatre program.Mr. Story excels in a wide variety of vocal techniques; classical, show-tune, pop and jazz and specializes in audition and performance preparation. Piano, his first instrument is joined by guitar and electric bass. His teaching style is trauma-informed and based in the ideas of student-led and project based learning.

Young Storytellers - Ages 5-8For the youngest performers. Students write and design a simple story such as a Fairytale, your child’s first structured play. Imagination Factory - Ages 9-12All students conceive, develop and create a One Act play for the stage. A powerful reinforcement for positive choices and development, this work increases social-emotional skills creative collaboration. Acting Boot Camp - Ages 13-19For the experienced student of the stage; Learn healthy, holistic acting techniques that keep the pros working. This acting and stage movement Boot Camp will give you experience with a range of techniques including Stellar Adler, Uta Hagen, Commedia dell’arte, Alexander Technique, Viewpoints Technique, and Improvisation. This class is designed for the actor to take their acting and stage movement to the next level. Culminates in an end of the year performance. Centre Stage will have the dress code on site. All dancers must be dressed in the dress code for in-studio or Zoom. Remember you receive 10% off throughout the school year all items at Centre Stage!

GENERAL INFORMATION

1. See Our Safety Procedures page 2.

2. Enrollment in our program is for the full school year; September to the Recital (typically the first or second weekend in June). Tuition is due in three parts; one third at registration, one third by November 15th and one third by January 15th. All tuition fees include registration, administrative and recital fee. All tuitions are not refundable.1

3. Please make checks payable to Centre Stage at Ballet Arts LLC. A 3% processing fee for credit card payments will be applied. Any failed payment transactions are subject to a $30 fee.2

4. To register by mail; please send registration with a check made out to “Centre Stage at Ballet Arts, LLC”, and mail to Centre Stage at Ballet Arts LLC, 504 Fifth Avenue, Pelham, NY 10803. Include post-dated checks for Nov. 15 and Jan. 15 if not paying in full.

5. Centre Stage at Ballet Arts LLC accepts students through January 31, 2022 unless a class is closed due to maximum enrollment.

6. The Ballet Arts Program follows the Pelham Public School System for most closings, including holidays and related closings.3 For complete schedule of no classes and weather related closings, go to our website www.ballet-arts.net.

7. All dance, gymnastics, fitness, cheerleading, and yoga essentials are available through Centre Stage.

8. Class Make-ups: to be eligible for a make-up notice must be given. Only 2 make-ups are permitted; no make-ups after January 31st, 2022.

9. Private, Duo and Trio Lessons: Any cancellation requires 24 hour notice or you will be charged in full, including Zoom.

10. Our dress code is strictly enforced, In the Studio or on Zoom.

11. Mrs. Daniele reserves the right to dismiss a student for any reason, including but not limited to non-payment, tardiness, excessive absences or inappropriate behavior.

12. Pro-rating tuition begins October 17, 2021. New students joining after Oct. 17, no later than Jan. 31, will be pro-rated per class which they have missed. Included in the tuition is the registration fee, administrative fee and performance fee valued at $150 per student for school year tuition. $50 for the 10 week sessions. $25 for the 10 week sessions with no performance. This portion of the tuition is not pro-rated.

13. All rehearsals are mandatory. Chronic absences during recital preparation (after January 31st) require private lesson make-up at an additional fee or will result in the student being dismissed from recital.

14. Schedule and staff subject to change.

15. We are committed to the absolute security of your personal information. We will not share your information for any purpose.

TUITIONTuition fees are non-refundable. Pro-rating tuition begins October 17, 2021. New students joining after Oct. 17, no later than Jan. 31, will be pro-rated per class which they have missed. Included in the tuition is the registration fee, administrative fee and performance fee valued at $150 per student for school year tuition. $50 for the 10 week sessions. $25 for the 10 week sessions with no performance. This portion of the tuition is not pro-rated.

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT1. In consideration for participating in PERFORMING ARTS CLASSES, PERFORMANCE, CAMP, RECREATION, ATHLETIC ACTIVITIES, FITNESS ACTIVITIES OR

ANY OTHER ACTIVITIES DIRECTED BY CENTRE STAGE AT BALLET ARTS, LLC. aka BALLET ARTS – THE PERFORMING ARTS CENTER OF SOUTHERN WESTCHESTER (HERETOFORE CSBA-LLC) and other valuable consideration, I hereby RELEASE, WAIVE, DISCHARGE AND CONVENANT NOT TO SUE CSBA-LLC, their officers, servants, agents, and employees (hereinafter referred to as RELEASEES) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES, or otherwise, while participating in such activity, or while in, on or upon the premises where the activity is being conducted or in transportation to and from said premises.

2. To the best of my knowledge, I can fully participate in this activity. I am fully aware of risks and hazards connected with the activity, including but not limited to the risks as noted herein, and I hereby elect to voluntarily participate in said activity, and to enter the above-named premises and engage in such activity knowing that the activity may be hazardous to me and my property. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTY DAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or any loss or damage to property owned by me, as a result of being engaged in such an activity, WHETHER CAUSED BY THE NEGLIGENCE OF RELEASEES or otherwise.

3. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS THE RELEASEES from any loss, liability, damage or costs, including court costs and attorney’s fees, that may incur due to my participation in said activity, WHETHER CAUSED BY NEGLIGENCE OF RELEASEES or otherwise.

4. It is my express intent that this Release and Hold Harmless Agreement shall bind the members of my family and spouse (if any), if I am alive, and my heirs, assigns and personal representative, if I am not alive, shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO SUE the above named RELEASEES. I hereby further agree that this Waiver of Liability and Hold Harmless Agreement shall be construed in accordance with the laws of the State of New York.

5. I UNDERSTAND THAT THE RELEASEES WILL NOT BE RESPONSIBLE FOR ANY MEDICAL COSTS ASSOCIATED WITH AN INJURY I MAY SUSTAIN.

6. I further agree not to violate any directive or instruction made by the person or persons in charge of said activity and that I will further assume the complete risk of any activity done in violation of any rule or directive or instruction.

7. I also understand that I should and am urged to obtain adequate health and accident insurance to cover any personal injury to myself which may be sustained during the activity or the transportation to and from said activity.

COVID RIDER

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that RELEASEES have put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that RELEASEES cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to RELEASEES staff and other clients and their families. I voluntarily seek services provided RELEASEES and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending RELEASEES scheduled activities.

I attest that:* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache,

sore throat, or new loss of taste or smell.* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold RELEASEES harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of, RELEASEES or that may otherwise arise in any way in connection with any services received from RELEASEES. I understand that this release discharges RELEASEES from any liability or claim that I, my heirs, or any personal representatives may have against RELEASEES with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from RELEASEES. This liability waiver and release extends to RELEASEES together with all owners, partners, and employees.

IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign it voluntarily as my own free act and deed; no oral representations, statements or inducements, apart from the foregoing written agreement, have been made; I am at least eighteen (18) years of age and fully competent; and I execute this Release for full, adequate and complete consideration fully intending to be bound by same.
