Defining Overweight and ObesityOverweight and obesity are both labels for ranges of

weight that are greater than what is generally considered

healthy for a given height. The terms also identify ranges

of weight that have been shown to increase the likelihood

of certain diseases and other health problems.

Definitions for AdultsFor adults, overweight and obesity ranges are determined

by using weight and height to calculate a number called

the “body mass index” (BMi). BMi is used because, for

most people, it correlates with their amount of body fat.

an adult who has a BMi between 25 and 29.9 is

considered overweight. an adult who has a BMi of 30 or

higher is considered obese.

it is important to remember that although it correlates with

the amount of body fat, BMi does not directly measure

body fat. as a result, some people, such as athletes, may

have a BMi that identifies them as overweight even though

they do not have excess body fat.

Other methods of estimating body fat and body fat

distribution include measurements of skinfold thickness

and waist circumference, calculation of waist-to-hip

circumference ratios, and techniques such as ultra-

sound, computed tomography, and magnetic resonance

The U.S. Occupational Safety and Health administration

(OSHa) recommends that “ nursing home employers

help reduce the number and severity of work-related

musculoskeletal disorders (MSDs) in their facilities.”

a MSD can include conditions, such as low back pain,

rotator cuff injuries, carpal tunnel syndrome and sciatica

(pain in the lower back and leg caused by compression

of the sciatic nerve).

To protect both healthcare workers and residents from

unnecessary injury, OSHa also recommends that manual

lifting of residents be minimized in all cases and eliminated

when feasible, and that employers implement effective

ergonomic processes that will help identify problems

and provide training to solve those problems.

Human dignity is an essential part of health care for all residents, including those who are obese.1 Similar to residents who are extremely underweight, bariatric patients are at risk for a host of difficult and costly-to-resolve skin conditions, making prevention absolutely crucial.2

Try using the information provided here to arm your staff, as well as bariatric patients and their families, with the knowledge, strategies and products to achieve optimal outcomes.2

Bariatric skin challengesBariatric patients face unique challenges that make them more susceptible to skin issues: • Reachingcertainareasofthebody–especially underneath the large belly fold (known as the pannus) – can be difficult for bariatric residents and their caregivers.2 Poor personal hygiene can result, especially in relation to toileting. Urine or feces left on the skin, even for a relatively short period of time, can contribute to skin breakdown.3

• Circulationtendstobecompromisedamongbariatric residents, primarily because blood does not travel as efficiently through fatty tissue.2 in addition, obese residents are at high risk for diabetes, which also causes poor circulation.2 Compromised circulation can impair the skin’s ability to heal. • Skinrubbingtogether–suchasbetweenthethighs– is common. This situation often leads to tissue breakdown.2

• Large,deepskinfoldsmakeitmoredifficultforobese residents and their caregivers to assess and monitor changes in the skin.2

• Physicalimmobilityisattheheartofobesity-related skin injury, including pressure ulcers.1 This is usually due to the inability to adequately turn or reposition the resident.1 Pressure ulcers often develop as the result of pressure, friction and shear.

Common skin problemsThree common skin problems for bariatric patients are friction and shear injuries, conditions related to poor toileting hygiene and incontinence, and pressure ulcers.

in all these situations, staffing ratios may have to be increased in order to deliver adequate care.2 Use the appropriate number of staff to prevent injury.1

in addition, be sure to invite equipment specialists or materials managers onto patient care units to help increase their understanding of patient care challenges and to show them how equipment can meet the clinical needs of patients and caregivers.1

Friction and shear injuries. Due to their increased weight, obese patients are at higher risk for friction and shear in-juries. When skin rubs against and across skin, the friction generated can injure the epidermis and deeper layers of the skin. When obese patients are bedbound or chair-bound and slide along hard surfaces (e.g., sliding down in bed when the head of the bed is elevated), shear injuries can occur.2

Ways to help minimize friction and shear injuries:• Selectclothingthatisroomyenoughtominimizeskin-

to-skin contact and allow for adequate positioning.2

• Usecornstarch-basedpowders,cottonfabric,gauze or other padding material to separate skin surfaces from one another.2

• Placesofttowels,papertowelsorlinenpillowcases between surface contact areas. This helps keep skin off skin and absorbs excess moisture. if the pieces of fabric are large enough, they may even be useful during repositioning.2

• Repositionobesepatientseverytwohours.1 - a mechanical full body lateral rotation device

can be helpful. although it can be an effective tool, mechanical repositioning is not a substitute for human touch. Further manual repositioning of arms, legs, breast and pannus is often required.1

- Be watchful about repositioning tubes and catheters as well. They should be placed so that the patient does not rest on them. Tube and catheter holders may be helpful.1

• Providebariatricequipmentthatiswideandsturdy enough to prevent rubbing and accommodates proper positioning (beds, chairs, geri-chairs, and wheelchairs).2 Pressure from side rails and arm rests not designed to accommodate a larger person can cause pressure ulcers on the patient’s hips.1

• Usepressurereductionandpressurereliefdevicesspecially designed for bariatric patients (mattress overlays, mattress replacements, specialty beds, and chair and wheelchair cushions).2

• Minimize high Fowler’s and semi-Fowler’s positions.2

Toileting hygiene issues and incontinence dermatitis or incontinence-related skin breakdown. Difficulties manag-ing personal hygiene, as well as incontinence caused by increased abdominal pressures, predispose obese patients to incontinence dermatitis and incontinence-related skin breakdown.2

People who experience leakage of urine or stool due to increased intra-abdominal pressures on the bladder or

bowel may benefit from scheduled toileting programs, fluid management schedules, dietary interventions (fiber and fluid), or biofeedback.2

Ways to help to minimize incontinence dermatitis and incontinence-related skin breakdown:2

• Usealong-handledmirrorregularlytoinspect the perineal and buttock area for skin integrity.• Usewipesorsprayno-rinsewashesroutinely following urination and defecation.• Useahairdryeronthelowsettingtokeepthe

perineal and buttock areas dry.• Useassistivedevices,suchashandheldshower heads, for personal hygiene whenever possible.• Provideaccesstoappropriatelysizedtoileting

devices (bariatric commodes and bariatric bedside commodes).

• Atthefirstsignofskinbreakdown,applyaskin protectant product.

Pressure ulcers. Skin often breaks down at the occiput due to the bull-head configuration of the head and neck. Skin folds and skin-to-skin areas can create so much force that a pressure ulcer occurs in an area that is normally not at high risk.4

Ways to help prevent and/or care for pressure ulcers:4

• Orderspecialbariatricpressure-reducingequipment for all immobilized or high-risk bariatric patients, including beds, commodes, and wheelchairs.• Vigilantpressureulcerprophylaxisiscritical,including

frequent inspections and turning and repositioning.• Carefullymonitorhighriskareasforbreakdown,

including heels, sacrum, occiput between skin folds, and skin-to-skin areas (especially the penis, buttocks and thigh folds).

• Carefullyinspectalllinesandtubestomakesurethey are not creating pressure points or caught in skin folds (i.e., under the pannus and skin-to-skin areas).

Educating obese patients and caregivers about these potential problems and empowering them with knowl-edge, strategies, and products goes a long way in helping to minimize bariatric skin problems.2 Providing patients and caregivers with written materials can help motivate them to incorporate preventive skin care strat-egies into their daily routines.2
