NEW BED TYPE CATEGORIES FOR REPORTING PURPOSES FROM MARCH 2008

1. Introduction: This Policy Directive should be read in conjunction with the Bed Type - NSW Health State-wide Data Standards Version 5 of the Admitted Patient Data Collection Dictionary. The purpose of this policy is to direct those involved with the maintenance of inpatient Patient Administration Services (PAS) to correctly allocate Bed Type categories for reporting purposes. This Policy Directive should be distributed to all staff involved in the maintenance of inpatient Patient Administration Systems (PAS). This includes those who manage and develop PAS statewide systems and those who manage the PAS at the hospital level (eg. PAS managers and Medical Record Staff). The Area Health Service and Hospital executive, and the intensive care clinical management staff, are to be advised and consulted on implementation of this Policy Directive to ensure there is a correct Bed Type allocation. 2. Purpose: This change will assist in identifying bed types in health facilities that until March 2008 have not been stored in the HIE and only reported via the Sustainable Access Program (SAP) reporting. The additional bed types have now been implemented in the HIE as part of the Release 4.3.5. In the instance of Bed Type 87 there is a requirement from Health Systems Performance Improvement Branch to identify the patients of the newly established Medical Assessment Units (MAU). The additional bed types are as follows:

Code Domain Definition 01 Mixed Purpose:

A bed staffed 24 hours a day and designated by the facility for the accommodation of medical, surgical and/or obstetric patients of a non-specific age. Beds in this category are typically located in small and rural hospitals (such as Multi Purpose Services) where medical, surgical, and obstetric patients are accommodated in the same ward as standard practice. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may be used for ‘overflow patients’ (that is, patients who would usually be accommodated in another Bed Type if those beds were available).

02 Rehabilitation: A bed staffed 24 hours a day and designated by the facility for the accommodation of patients primarily requiring rehabilitation care. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may be used for ‘overflow patients’ (that is, patients who would usually be accommodated in another Bed Type if those beds were available).

03 Palliative: A bed staffed 24 hours a day and designated by the facility for the accommodation of patients primarily requiring palliative care. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may be used for ‘overflow patients’ (that is patients who would usually be accommodated in another Bed Type if those beds were available).

04 Psychiatric Acute: A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate patients who present with acute episodes of mental illness. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are counted as “Days in a Designated Psychiatric Unit”.

Code Domain Definition 05 Psychiatric Rehabilitation:

A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate patients requiring care which has a primary focus on intervention to reduce functional impairments resulting from psychiatric disorders that limit the independence of patients. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are counted as “Days in a Designated Psychiatric Unit”.

06 Psychiatric Secure: A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate of forensic patients (or other patients) requiring a higher level of care and containment than other mental health patients. Forensic patients are people with a mental disorder that has led them to commit criminal offences, or makes it likely that they will offend in the future if not adequately treated or contained. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are counted as “Days in a Designated Psychiatric Unit”.

07 Drug and Alcohol: A bed staffed 24 hours a day that is officially recognised by NSW Health for the accommodation of patients requiring treatment for substance use/abuse on an admitted patient basis; excluding detoxification. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are NOT counted as “Days in a Designated Psychiatric Unit”, despite the fact they can be co-located with designated psychiatric beds.

08 Brain Injury Rehabilitation Program Rehabilitation: A bed staffed 24 hours a day and officially designated by NSW Health Department circular for the accommodation of admitted patients receiving care in a rehabilitation unit for a brain injury under the Brain Injury Rehabilitation Program. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’.

12 Psychiatric Intensive Care: A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate patients requiring a higher intensity of management than can be readily provided in a general acute psychiatric bed. These beds are staffed and designed for the accommodation of patients who:

• are severely disturbed and represent a high risk of being a danger to themselves, others or the environment

• present with threatening or violent behaviours and cannot be safely managed in a less restrictive environment

• require higher levels of nursing observation. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are counted as “Days in a Designated Psychiatric Unit”, but NOT as “Hours in Intensive Care”.

13 Post Natal Depression: A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate patients with post natal depression. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are counted as “Days in a Designated Psychiatric Unit”.

Code Domain Definition 14 Residential Aged Care – High (Nursing Home):

A residential aged care place operated in a residential aged care facility to frail older people with high levels of functional dependence, 24 hours per day, 7 days per week for the long term, either by registered nurses or under the supervision of registered nurses. Residential aged care places are operated under the terms of the Aged Care Act 1997 and funded and regulated under the Act by the Australian Government. In order to access high-level residential aged care, people must have been assessed by an Aged Care Assessment Team as requiring care within levels 1-4 of the Resident Classification Scale, 1 level 1 being the highest care level.

15 General Intensive Care: A bed staffed 24 hours a day that meets the Intensive Care role delineation of level 4 or higher, and used to accommodated patients requiring intensive care. Beds classified as Intensive Care beds must be specially staffed and equipped to provide observation, care and treatment to patients with actual or potential life-threatening illnesses, injuries or complications, from which recovery is possible. The bed is serviced by special expertise and facilities for the support of vital functions and utilises the skills of medical, nursing and other staff trained and experienced in the management of these problems. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Time in this Bed Type count towards “Hours in Intensive Care”. This category excludes “Neonatal Intensive Care Units” and “Psychiatric Intensive Care Units”.

16 Neonate Special Care Nursery: A bed staffed 24 hours a day that is approved by the federal Government for the purpose of the provision of neonatal special care. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Time in this type of unit does not qualify for “Hours in Intensive Care”. Newborns admitted to this Bed Type are considered to be “Qualified Newborns”. Special Care Nurseries that do not have federal Government approval are excluded from this Bed Type.

A bed, stretcher, trolley or chair in an area within the hospital staffed 24 hours a day and designated as an Emergency Department of Level 3 or above. Note: Beds in this category may not be used for ‘overflow patients’. Beds classified as ‘Emergency Medical Units’ are excluded from this category.

18 Mother Craft: A bed staffed 24 hours a day that is designated by the Department for accommodating patients who require mothercraft services only. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’.

20 Psychiatric Extended Care: A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate patients who require care over an indefinite period because they have a stable but severe level of functional impairment and an inability to function independently and thus require extensive care and support. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are counted as “Days in a Designated Psychiatric Unit”.

21 Neuro-Psychiatry: A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate patients who require neuro-psychiatric care. These beds are attended by specialists in Neurology or Neuropsychiatry. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Days spent in this type of bed are counted as “Days in a Designated Psychiatric Unit”.

22 Dual Diagnosis: A bed staffed 24 hours a day that is designated by the NSW Health Centre for Mental Health to accommodate patients who require specialist care for co-existing mental health and substance use disorders. A residential aged care place operated in a low-care residential aged care facility to people with low levels of functional dependence, 24 hours per day, 7 days per week for the long term. Residential aged care places are operated under the terms of the Aged Care Act 1997 and funded and regulated under the Act by the Australian Government. 32 Collaborative Care Service Provider – Mental Health: A “virtual” bed established on the patient administration system to record mental health admitted patient activity provided by another public or private hospital under a collaborative (or contract) care arrangement. Note: The days the patient is accommodated in these beds counts towards the patient days for both the purchasing facility and the facility providing the collaborative care service.

33 Coronary Care: A bed staffed 24 hours a day that is designated by the facility to accommodate patients who require cardiac monitoring (both bedside and central), but which otherwise does not meet the criteria of ’15 - Intensive Care’. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Time in beds with this Bed Type do NOT count towards “Hours in Intensive Care”.

34 High Dependency Care: A bed staffed 24 hours a day that is designated by the facility to accommodate patients who require critical care expertise that is less resources intensive than would be allocated if accommodated in an Intensive Care Bed but more resource intensive than the care provided in a general bed. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Time in beds with this Bed Type do NOT count towards “Hours in Intensive Care”.

Code Domain Definition 37 Neonatal Intensive Care:

A bed staffed 24 hours a day that meets the Intensive Care role delineation of level 3 or higher, and used to accommodated neonates requiring intensive care. These beds must be capable of providing complex, multi-system life support for neonates for an indefinite period. They must be located in an area capable of providing mechanical ventilation and invasive cardiovascular monitoring; or care of a similar nature. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Time in this Bed Type count towards “Hours in Intensive Care”. This category excludes “General Intensive Care Units” and “Psychiatric Intensive Care Units”. Newborns admitted to this Bed Type are considered to be “Qualified Newborns”.

38 Transplant: A bed staffed 24 hours a day that is designated as a special unit dedicated to care of transplant recipients. This category includes beds in units dedicated for bone marrow (haemopoietic stem cell), renal, heart, heart-lung, liver, and pancreas transplants. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’.

39 Same Day Renal Dialysis: A chair/bed in a designated specialised day stay unit that is dedicated to the provision of renal dialysis services. Note: Renal Dialysis units generally have regular business hours and open and close each day at set times, however in some facilities these units may be open at midnight. Beds in this category may not be used for ‘overflow patients’.

40 Specialist Spinal Injury: A bed staffed 24 hours a day that is designated as a special State unit dedicated to the initial treatment and subsequent ongoing management and rehabilitation of patients with acute spinal cord injury. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Time in this Bed Type do NOT count towards “Hours in Intensive Care”.

Code Domain Definition 41 Severe Burns:

A bed staffed 24 hours a day that is designated as a special State unit dedicated to the initial treatment and subsequent rehabilitation of severely injured burns patients. Patients accommodated in these beds usually have greater than 10 per cent of body surface affected for adults, and greater than 5 per cent of body surface affected for children. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may not be used for ‘overflow patients’. Time in this Bed Type do NOT count towards “Hours in Intensive Care”.

42 Dental Service: A bed in a specialised unit designated for the treatment of the patients receiving dental services. Note: Dental Service units generally have regular business hours and open and close each day at set times, however in some facilities these units may be open at midnight. Beds in this category may not be used for ‘overflow patients’.

43 Paediatric: A bed staffed 24 hours a day that is designated by the facility for accommodation and treatment of patient’s aged older than 29 days and less than 16 years. Note: Beds in this category may be used for a mix of both day stay and overnight patients, but must be staffed for overnight patient care. Beds in this category may be used for ‘overflow patients’ (that is, patients who would usually be accommodated in another Bed Type if those beds were available). This category may only be used by general hospitals and is NOT to be used by the specialist children’s hospitals – “The Children’s Hospital at Westmead”, or “Sydney Children’s Hospital”.

46 Medical: A bed staffed 24 hours a day and designated by the facility to primarily accommodate medical patients of a non-specific age. Funding: A flexible care place located within a facility under the terms of the Australian Government Transition Care Program Guidelines and the Aged Care Act 1997. The facility must be modified and designated for the provision of transitional aged care. Transitional Aged Care places must be used to provide a mix of services for older people at the conclusion of a hospital admission. Services will include low intensity therapy such as physiotherapy, occupational therapy and social work and nursing support and/or personal care for clients to complete their restorative process and optimise their functional capacity. Services are provided for a limited time period of up to 12 weeks with a possible extension of another 6 weeks. The clients accomodated in these places must:

Patients can be admitted, prepared for the surgical procedure, then monitored and provided with appropriate pain relief post-surgery before protocol based discharge occurs within 24 hours. Extended Day Only (EDO) can be for emergency or elective procedures (surgery and occasionally non surgery e.g. diagnostic procedures, medical procedures). 87 Medical Assessment Unit (MAU): A bed in a dedicated Medical Assessment Unit (MAU) which is a physical ward that is close to the Emergency Department although not co-located. The MAU is an alternative pathway for adult non-critical medical patients; in many cases these will be elderly complex medical patients with co-morbidities. Patients within the MAU can expect to stay for a period of up to 48 hours, after this time most will be discharged home (with community services where appropriate), while some who require specialised care will be transferred to an inpatient bed.

99 Lodger / Boarder Beds: Beds used to accommodate boarders and lodgers who are not in receipt of health care. People accommodated in these beds are commonly relatives (e.g. parents or children) of patients.
