Vitality Check Biometric Screening

Measures your: • Body Mass Index (BMI)

(height, weight, waist circumference)• Blood glucose• Blood pressure• Total cholesterol• Cotinine (screening for nicotine)You can earn as much as 4,025 Vitality Points for this activity if all your results fall within a healthy range.

If you’re unable to meet in-range results, Vitality automatically provides you with an alternative goal, or Reasonable Alternative Standard, to earn the same amount of points as an in-range result. Learn more at Vitality > My Profile > My Goals.

If you think you are unable to meet an activity-only Vitality standard (or a reasonable alternative to such standard), you might qualify for an opportunity to earn the same reward by different means with your doctor’s approval. Please visit the Forms page on the Vitality website to download the applicable waiver form. If necessary, complete your portion of it, have your doctor complete his/her portion of it and then submit it to Vitality. Vitality will use your doctor’s recommendations to apply a medical accommodation for you in light of your health status.
