Affidavit of Loss

 I, Bezaleel T. Baengan, of legal age, Filipino, married and a resident of Brgy. 48 Cabungaan, Laoag City, after having been duly sworn to in accordance with law, depose and state THAT:

1. I own a Smart sim card with cellphone no. 09193657836 which was issued by Smart Laoag;

2. Sometime on February 29. 2016, while traversing the streets of Laoag City, I lost my cellphone containing the said sim card;

3. Efforts were exerted to locate my cellphone, but in spite of diligent search, it could not be found and the same is now beyond recovery;

4. As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my application for the issuance of a new Smart sim card with the same cellphone number, in lieu of the one that was lost;
