It is mutually agreed by the people above to confirm the fact as follows:

At 13h30, April 22nd 2015, there is a shift performing the coronary intervention which need using drug coated stents with 4.0/24 size, we have used one product among the consigned goods for performing intervention from Golden Gate Trade Services Joint Stock Company. When we draw out the Stent Biomime 4.0/24, the external cardboard still remains with integrity, the stamp containing theproduct parameters still remains available, sealing stamp also remains with integrity too. When opening the cardboard, inside is a nylon bag which contains an opened stent, however, the surface of such bag still has the stamp containing theproduct parameters available except the stent.

Outside the machine room and inside the control cabin of the intervention room at this moment, the representative of Golden Gate Trade Services JSC, Mr.Le Ba Dai has been available here. we has immediately notified him and Mr.Dai has already called to the company for notification. After the intervention shift, we has agreed to form a minutes of confirmation, and simultaneously returning to Mr.Dai all products and packagings as the basis for Golden Gate company to handle.(Detailed specification of the product: Stent Biomime 4.00x24; Lot: BE 95; REF: BIO 40024; SN: CL24BE95022; Expired date: 12/2015)
