F or general practitioners in Québec, November 28, 2014 will remain a date marked by anger and disbelief. On that day, contrary to all expectations, the Minister of Health and Social Services, Gaétan Barrette, presented Bill 20 in the National Assembly. Since the bill was introduced, we have seen a rare display of shields being raised among general practi- tioners. And this is entirely understand- able! Let me briefly remind you of what this bill imposes, among others, for all general practitioners. The bill calls for stiff penalties if general practitioners do not care for a minimum number of registered patients and work a minimum number of hours in institutions. A new AMPs plan has thus been established. For work in institutions, the minister himself will set the number of hours that can be authorized for each type of work and will let the agences know through directives. Depending on terms and conditions that will be determined by regulation, the bill establishes a new mechanism for reduction of remuneration in cases where the obligations are not met. This is the end of the professional approach and the joint review between the FMOQ and the Ministry. In fact, the FMOQ has been taken out of the picture. None of its representatives can be a Chief of a DRMG and the minister, the height of authoritarianism and arbitrariness, reserves the right to modify physicians’ remuneration himself to impose his vision in terms of accessibility. Quite a few of us have been right to speak of medical dictatorship. AMOM’s position Like its members, AMOM is outraged by the legislative practice that Gaétan Barrette has subscribed to. His bill constitutes an unprecedented attack against the professional independence of general practitioners, their integrity and the quality of their work. Presented with disdain and smugness, Bill 20 will not go without response from AMOM. The Executive, working in collaboration with those of the other associations affil- iated with the FMOQ, started working several weeks ago on its plans for responding. AMOM says NO to Gaétan Barrette and to the lies that he is circulating to sell his bill to the public. AMOM will not allow him to dictate the practice of family medicine in Québec! Bill 20:AMOM says “NO”!

F or general practitioners inQuébec, November 28, 2014 willremain a date marked by anger

and disbelief. On that day, contrary to allexpectations, the Minister of Health andSocial Services, Gaétan Barrette,presented Bill 20 in the NationalAssembly. Since the bill was introduced,we have seen a rare display of shieldsbeing raised among general practi-tioners. And this is entirely understand-able! Let me briefly remind you of whatthis bill imposes, among others, for allgeneral practitioners. The bill calls forstiff penalties if general practitioners donot care for a minimum number ofregistered patients and work a minimumnumber of hours in institutions.

A new AMPs plan has thus beenestablished. For work in institutions, theminister himself will set the number ofhours that can be authorized for eachtype of work and will let the agencesknow through directives.

Depending on terms and conditionsthat will be determined by regulation,the bill establishes a new mechanism forreduction of remuneration in caseswhere the obligations are not met. Thisis the end of the professional approachand the joint review between the FMOQand the Ministry. In fact, the FMOQ hasbeen taken out of the picture. None of

will be important and full. Our goal is to fight Bill20 to see to it that it is withdrawn, plain andsimple. The success of our action rests on the abilitythat we have to mobilize and to remain watchful.All physicians, regardless of their place of practice,young and less young, must remain united in soli-darity, standing shoulder to shoulder. You all know

the saying: “Divide and conquer”? Physicians willhave to face the adversity as one, all together andlinked by a single goal: abolish Bill 20.

O ur Association’s Annual General Meeting tookplace on November 21, 2014.

The day started off with a meeting of theparticipants with the President-Executive Directorof the Montréal Agence de santé, Mrs. PatriciaGauthier, who spoke to them about first line carein Montréal. She addressed the structure of thesystem as well as what has been achieved in recentyears to harmonize and facilitate how patientsprogress through the network.

This was followed by a morning of professionaldevelopment during which 130 physicians partici-pated in three hours of continuing medical educa-tion. Sessions dealt with diabetes, rheumatologyand the Ebola virus. At the same time, a smallgroup of physicians attended an interactive confer-ence on the DSQ and DME.

Lunch was served following the morningsessions and was followed by the business segmentof the day – the actual annual meeting. The usualreports were presented, including finance, contin-uing medical education, the secretary’s report andthen Dr. Michel Vachon presented the president’sreport. He spoke to the members about the variousadministrative and political dossiers, and then hegave the floor to Dr. Louis Godin who discussedthe main developments in the negotiations.

The president of the FMOQ mentioned thedifficulties with the current negotiations. Thearrival of a new minister led to major changes inrelations and in the manner of negotiating. Dr.Godin reported on Dr. Barrette’s bill aiming torestrict medical practice by creating obligations interms of patient load and tasks in institutions,weighted according to number of years in prac-tice. This would replace the current AMP plan. Thedata presented are incomplete and seem to be a

negotiating position forthe spring of 2015.(Editor’s note: We nowknow that this was theannouncement of Bill20 which was to betabled in a sneaky andhasty manner in theNational Assembly onNovember 28). Theyear 2015 is shaping upto be quite challengingand it will be veryimportant to remainunified.

After Dr. Godin’s speech, the results of the fallelection were announced by Me Pierre Belzile, whois the head of the FMOQ Legal Department andElections Chairman. Please see the list of delegateselected for 2014-15.

The afternoon, which was full of discussionsand exchanges, ended with several drawings. Dr. Vachon then invited the members to a cocktailreception.

For the past several years, AMOM has taken theopportunity of the annual meeting to pay tributeto its members who have been in active practicefor 50 years. In 2014, the Association thanked Dr. Jean Binda, Dr. Albert Nantel and Dr. PatriciaForbes for their tireless dedication.

The next AMOM annual general meeting willtake place on November 20, 2015 and we areplanning an afternoon and evening format.

Did you Know?

T he Agence de santé et de services sociaux deMontréal has worked on several projectsover the past few years to improve acces-

sibility to specialized and diagnostic services forpatients of family physicians. Some of theseservices are already established and are oper-ating. Do you know about them?

Pediatric Access Mechanism

Offered throughout the Montréal region, thismechanism allows access to consultations ingeneral pediatrics, endocrinology and respira-tory disease (respirology). Gastro-enterologyand allergy are to be added shortly.

Clinical Triage (Accueil clinique)

Already operating in Ahuntsic/Montréal-Nord,Lucille-Teasdale, Pointe-de-l’Île, Coeur-de-l’Îleand Saint-Léonard/Saint-Michel, as well as inSacré-Coeur. The service is being developed inother sectors.

This service allows for treatment of semi-urgent

conditions that would require an investigation by anurse who uses an algorithm (e.g. to rule out TPP).

CRDS (Centre for distribution of service requests)

In the east sector, a service to access ultra-sound is in place.

In the west sector, a service for access tospecialties enables service for the 5 CSSSs of theMcGill University Integrated Health Network(RUIS). The specialties available are ophthal-mology and orthopedics. Cardiology, gastro-enterology and respirology will be added shortly.

You can find the description of the servicesoffered and the required referral forms on thewebsite of your local CSSS, in the sectionreserved for professionals/physicians.

It is hoped that the changes in structure willnot interfere with the implementation of theseservices, which are very useful for patients.

of general physicians who would agree to treat other doctors. Please, do not hesitate to contact us!

T hanks to the effort made to recruit new members by both the professional staff and the physicians whoare members of the Executive, last November AMOM reached the magic number of 1,705 membersin good standing. This has enabled AMOM to obtain a 28th delegate to the FMOQ Council.

Remember that our association has by far the largest number of delegates at the Council, which is of primeimportance when the time comes to focus on specific issues related to Montréal. Of the new members, 32 arefrom the class of 2014. Bravo to all and let’s aim for a 29th delegate in 2015!
