Condition Monitoring Examination

1. Application forms

a) PSL57 CM – Application form, in future this is the only version of the application form we

will accept. If any others are received, they will be returned to the candidate with a

request to complete the current version.

b) Witness statements – in future applications will not be processed if the witness

statement has not been completed.

c) Original photos – may I remind you that the 2 x original passport photos (1 x signed by

the witness) must be sent with application forms. If application forms are sent by email,

a scanned photo can emailed at the same time but MUST be followed with the original

photo’s being sent with the examination papers. Original copy of application form can

be sent with examination papers as well.

d) Training Certificates – in future a copy of the training certificate MUST be sent with the

application form (if available). Or if possible with the exam papers, or if not possible

sent as soon as available. NO TRAINING CERTIFICATE, NO RESULT.

e) Training Certificates validation – please be reminded that training is only valid for 2

years, if the candidate wishes to apply for certification after 2 years they will need to re-

attend the relevant course.

f) Work experience – please be reminded of the minimum requirements:

Method Category 1 Category 2 Category 3

VA 6 months 18 months 36 months

Mature Candidate at Category 2 route (All Methodologies) – Candidates with a minimum

of 5 years (with no significant interruptions) work experience are able to apply for

Category 2 certification (by-passing Cat 1). The candidate must provide evidence of

attending a training course of a minimum of 38 hours (Cat 1) and then of course, passing

the Category 2 examination.

2. Examinations

a) Identification forms – all Invigilators must ensure they are completed and signed prior to

the start of the examination, including the reference number from the photo ID

provided by the candidate. NO ID, NO EXAM!

b) Candidates’ name and signature – please be reminded that candidates MUST complete

the front page of the exam paper and each subsequent page with their name, signature

and the date. It will be at the discretion of the Scheme Manager if a completed exam

paper is accepted without all pages having been duly signed.

3. Results Notices

Results Notices – please be reminded that all candidates will initially receive an email with

their exam result, which will be followed by the official Results Notice, whether they have

passed or failed.
