A. Personal StatementI have training in the field of public health, but especially the experience of community organizing,coordinating events, health fairs and clinics.

I have dealt with top executives, joining in purposes different personalities and organizations. All myprofessional life I have been successful in the field of wellness community organizing projects to achievehealth promotion. I have been conducting specifically different activities in public health. Also for theeducation of health professionals, for engaging populations on projects and to change primary care incommunities.

I have the motivation and the necessary expertise required to organize groups and entities to conduct a joint research program. Also my experience has focused on health facilities, quality control of servicesand data collection on this community projects.

I recently participated in the certification of electronic medical record of several doctors in Puerto Rico asa consultant and know very well the processes of the new amendment Hippa confidentiality. I amcertified on cybersecurity in the education system of State Georgia EU.

Wanting to start making a research career that has efficacy to needy populations in the Global Health.

C. Contribution to Science

Overall our contribution to science has always been oriented on public health, prevention, protection andpromotion of health.

In 1993 we are part of the core group of American College of Sports Medicine on the Osteoporosis group.This led us to conduct a clinic in Puerto Rico Wellness and Education in the area of osteoporosis, developnew therapies and a group of specialist doctors and physical therapists in physiotherapy with aninterdisciplinary group supported by laboratory and nuclear medicine Image.

In 1994 we conducted a study of quality control of the only existing Cardiac Rehabilitation Program inPuerto Rico also coordinating services with doctors and performing control of health status of patients withthe review of several tests including telemetry, densitometry, nuclear perfusion study of bones, Mugasnuclear studies, and other vascular doppler studies.

In 1997 we are part of the advisory body to the medical advisor to the President on special projects ofresearch and development for private industry, for the wellness program of Triple S, the main healthinsurance in Puerto Rico.

In 2000 part of the body of advisors to the Secretary of Health, Department of Health of Puerto Rico, toparticipate in the development of the laboratory of physical fitness of the Division of Prevention and Health

Promotion Department. This function lab had the evaluation the physical capacity and had the riskmanagement being of all employees of the Department and the study was published by the Departmentthat included epidemiological surveillance data (BRFSS) published by the CDC.

In 2012 we were invited by SPEVCO a company in North Carolina and advisers of the government of theUnited States to participate in a mock emergency disaster two states coordinating jointly in the history ofthe United States. The first of its kind in the United States, where I trained for put together the pieces ofthe mobile hospital in a mock of disaster sponsor by two state, North Carolina and Mississippi. The productMonarch, the mobile hospital facilities designed by advisers of John Hopkins and the company Spevco.This exercise was the first of the two states of emergency coordination after Hurricane Katrina.

In 2014 we participated in the coordination of the first cancer hospital clinic Dr. Isaac Gonzalez Martinez,

who since 1957 the first rural clinics, I had the good fortune to coordinate with the mayor and othercommunity leaders of the Rural Municipality of Utuado.

 All these events need assessment required, demographic studies and other studies of social research toachieve the objectives of prevention and promotion and only one was published in a cybernetic programcalled Authorstream which still remains for public satisfaction. (Digital)
