    Emergency trip wire was not installed in afavourable position.

    The wires on either side of the conveyors were lower

    than the conveyor frame.Trip wire was inaccessible if someone fell onto

    conveyor.

    The position of the emergency trip wireswere corrected.

    The wires on either side of the conveyors were raised to

    a more accessible positionOverhead trip wires with pull chains were also

    installed as further precautionary measures.

    Unauthorised Access to Conveyor

    The conveyor was opened at the rear andeasily accessible.

    The rear was closed off to mitigate the riskof unauthorised entry onto the conveyor.

    Audible Alarm

    Conveyor starts up on automatic modedepending on:

    Level of the pulper and the consistency.

    Audible alarm with a 15 second delay wasinstalled to alert the operator of theimminent start up of the conveyor.
