Steve Jobs1955-2011

“About a year ago(2003), I was diagnosed with cancer. I had a scan at 7:30 in the morning, and it clearly showed a tumour on my pancreas. I didn't even know what a pancreas was. All the doctors told me was that this was almost certainly a type of cancer that is incurable, and that I should expect to live no longer than three to six months."“Later that evening, I had a biopsy, where they inserted an endoscope down my throat, through my stomach and into my intestines, put a needle into my pancreas and removed a few cells from the tumour. I was sedated, but my wife, who was there, told me that when they viewed the cells under a microscope the doctors started crying because it turned out to be a very rare form of pancreatic cancer that is curable with surgery. I had the surgery and I'm fine now."If it wasn't for the wonders of health screening, diagnosis and treatment, Jobs would not be able to introduce iPhone 3G on June 9, 2008, new iPod media players on Sept 9, 2008, and unveil a lighter MacBook Air laptop on Oct 20, 2010

2012 marks a very special year for the BP Healthcare Group as the Group celebrates its 30th Anniversary.

This year, the founder's second generation is actively

taking charge of the Group’s business activities and

expansion programmes, assisted by the committed

and loyal Group's senior management and staff force.

How We StartedI did a Management Buy Out (MBO) on January 1, 1982, from my former boss after working for his company for 14 months at the age of 23. Confidently, I changed the company name to BP Clinical Laboratory with my girlfriend, who had subsequently become my wife. BP represents my surname, Beh, and my wife's surname, Poh. I am glad and fortunate to have taken the courage to venture into my very first business which has yielded promising and encouraging results. BP Clinical Laboratory Partnership Company became a Private Limited Company (PLC) and later seeded the Group’s expansion to over twenty companies and investments.

How We Have ProgressedFor the past three years, the Group has registered triple digits compounded annual growth rate (CAGR) in terms of sales and profit. The Group completed its first phase of PROJECT 50, which aims to set up 50 branches for each of its business entities as below:-

Regional expansion has already taken place with renowned global partners, namely, the Redbull family in Thailand and the Ciputra Group in Indonesia. These global partnerships help generate more opportunities to grow and expand our outreach in this region. As both countries have larger populations and business opportunities as compared to Malaysia, this augers well for BP Healthcare Group’s aspiration to be a billion-ringgit company. Our objective is to develop another 100 outlets in Indonesia and 50 in Thailand within the next 3 years. BP Pharmacy is expanding parallel to its in-house dispensaries to meet BP Diagnostic Centres’ demand. It has openly introduced online pharmacy to the public to serve more customers, in contrast to the previous practice of providing sales to a limited number of only a few thousand private clinics. BP Pharmacy recently re-introduced the wholesale pharmacy business in which we adopted a new business concept via banking facilities as third-party operators to do away with the collection burden with customers. BP Pharmacy currently has several successful proprietary products and with our trusted brand and global network reach, we hope to introduce more products to better serve our customers. In addition, the Group is also setting up a chain of retail pharmacies under Lovy Pharmacy which will focus on a whole new business approach, including drive-through pharmacies, home deliveries and will be offering open online purchases from the Lovy Pharmacy website. The Group intends to up-grade 10 BP Diagnostic Centres' dispensaries to become the Lovy Pharmacy chain within the first quarter of 2012 and to open another 50 Lovy Pharmacies by the end of 2012. Lovy Pharmacy will be operating either alongside a BP Diagnostic Centre, or as an independent retail pharmacy. The Group aspires to expand Lovy Pharmacies as among one of the few largest retail pharmacy chains in the country within the next three years! BP Hearing Solutions Sdn Bhd is now the sole distributor in Malaysia for Beltone, a renowned hearing aid product from Denmark, in which its parent company is listed with a market capitalization of a few billion Euros. The Group has a panel of audiologists to provide and supervise professional audiograms and hearing fittings. To further develop this hearing service, the Group has recruited and re-enforced a panel of ENT specialists to further increase and to provide comprehensive hearing solutions service, thus offering patients who require specialist service to benefit from a truly one stop centre.

BP Food Testing has been expanding very rapidly and is now offering a wide variety of tests for food materials. With that in place, we are now introducing a wide range of new tests for environment testing via our new company, BP Environment Testing Sdn Bhd. Concurrently, the Group is also planning to develop other testing service related to oil and gas this year.

This year the Group will be investing and introducing the following complementary services and facilities:-

Dental care

Eye care

Expanding in-house Corporate Medical Panelship service

Expanding in-house Corporate Nutritionist and Dietician Panelship service.

8.00 am: Rushing to work, without a proper breakfast. Perhaps a quick stop at the mamak stall for a roti canai

and teh tarik or a drive thru’ for a sausage muffin with egg and a hash brown, Yums!

9.30 am: Meetings and deadlines to rush!

7:00 pm: Back at home. Exercise? Definitely no time for that!

Sounds familiar? That my friend is the recipe for the growing obesity trend of our nation. 2 in 5 of the adult population in

Malaysia are now rated overweight (BMI>25) and obese (BMI>30). The topic of obesity is somewhat controversial,

especially when many of us are still in denial. Living in a developing nation, we are more prone to buying food that lacks

nutrition and as the skyscrapers increase, so has our sedentary lifestyle.

Every centimetre added to your frame extent an invitation to “unwanted guests” that not only cause health problems but

also affects our state of mind. Don’t be fooled by the absence of pain when you could already be at pre-diabetic or

pre-hypertensive levels.

Now the big question is: is it that hard to control our weight? Let’s begin with little steps to achieve our ultimate weight

management goal!

Step 1: Know your vital statisticsWhat is your ideal weight? Using the BMI (Body Mass Index) formula: BMI = Weight (kg) / [Height (m)]2, you can measure

your ideal body weight. Using BMI of 22.5 will provide you your ideal body weight.

You should also know how much of your current weight is contributed by fat. It is not excess weight that is unhealthy but

excess fat. The ideal body fat percentage is up to 20% for men, and 25% for women. Get the help of a health professional

to analyse and monitor your body composition.

The waist to hip ratio (WHR) which is your waist measurement divided by your hip measurement gives an indication of

where most of the body fat is being deposited. For good health, WHR for men should be 0.9 and 0.8 for women. A high

ratio indicates central (abdominal) obesity, and increases the risk of heart diseases and diabetes as there is more visceral

fat (fat deposited within the abdominal cavity).

Step 3: Let’s get physicalIt doesn’t stop at just having a healthy diet. Being physically active

keeps heart disease, diabetes and hypertension at bay and

increases your fitness level. Start simply by walking. Walking as

much as 10,000 steps daily is equivalent to burning off a plate of

chicken rice or a slice of cheese cake. How to achieve 10,000 steps?

Use the stairs instead of the elevator

Park your car further away from the entrance

Make effort to walk to your colleague’s desk instead of using the phone

Walk out to an eating place far from your office – especially if you have a desk job

Walk in the park on alternate days

Limit seated activities at home – start with sweeping and mopping regularly

Walk to nearby destinations, like the grocery store or convenience shop instead of driving

Step 2: You are what you eat!You may be familiar with this phrase by now. Start by practising healthy

cooking methods. This should include less fat, less salt and less sugar,

so you can still enjoy a variety of food but on a healthy basis. A little effort

goes a long way, adopting these practices helps you prepare healthy

meals:

Remove any skin or visible fat from meats before cooking

Use paper towels to absorb excess oil from fried foods

A non-stick pan helps decrease oil usage

Opt for healthier cooking methods like steaming, boiling, baking or using the oven

Cool down soup or curries and remove any visible surface fat

Opt for low-fat salad dressing

Avoid using too much sugar while cooking

Use natural spices like black pepper or ginger or lemon to flavor food

Reduce consumption of preserved and salted foods

What is Upper Gastrointestinal (GI) Endoscopy? The literal meaning of endoscopy is 'looking inside' (endo=inside; scopy=looking). Hence, an upper GI endoscopy, also known as gastroscopy is a procedure that uses a lighted, flexible endoscope to see the insides of the upper GI tract. The upper GI tract includes the esophagus (the pipe that connects the mouth to the stomach), stomach, and duodenum (the first part of the small intestine).

Who Will Benefit from a Gastroscopy?Usually performed as an outpatient procedure, gastroscopy is commonly used to help identify digestive related problems such as:

Dyspepsia (feeling of indigestion and abdomen fullness)

Gastric pain

Gastric reflux (heartburn)

Swallowing difficulties

Excessive vomiting

The doctor may also suggest a gastroscopy to be performed if

he or she has a strong reason, such as strong family history, to

believe that you are at risk of having an oesophageal or gastric

cancer.

Gastroscopy is also performed as part of a work-up for patients

with unexplained weight loss or unexplained anaemia and to

confirm Helicobacter pylori infection which is an important cause

of stomach/duodenal ulcers.

How to Prepare for a Gastroscopy? The upper GI tract must be empty before an upper GI endoscopy is performed. Generally, no eating or drinking is allowed for at least 4 hours before the procedure. Patients may also be asked to temporarily stop certain medications that they are taking prior to the procedure that could affect blood clotting or interact with sedatives, a type of medication often given during an upper GI endoscopy to calm the patients.

During the ProcedurePatients are requested to lie on their side on the examination table. Analgesics are often given in the form of a spray to “numb” the throat and sedation in the form of an injection into the vein is done. The endoscope is carefully fed down through the esophagus to the stomach and then duodenum, looking for any abnormalities. Biopsies of tissue can be taken through the endoscope for examination under the microscope to exclude cancerous cells or for identification of Helicobacter pylori infection.

Recovery from a GastroscopyAfter an upper GI endoscopy, patients are observed in a recovery room while the sedative wears off. During this time, patients may feel bloated or nauseated. Unless otherwise directed, patients may immediately resume their normal diet and medications.

What is Helicobacter pylori and am I at Risk?Helicobacter pylori (H. pylori) is a spiral-shaped bacteria which lives in the stomach lining. Most people who are infected with H. pylori

do not have any symptoms and will never develop illness as a result of the infection. Some people, however, develop ulcers in the stomach (gastric ulcers) or in the duodenum (duodenal ulcers) which can cause unresolved gastric pain. Infected persons have a 2- to 6-fold increased risk of developing gastric cancer compared with their uninfected counterparts.

We do not know how exactly H. pylori infection spreads, or why some people develop illness although most people do not. The bacteria may spread from person to person through fecal-oral or oral-oral routes. It has also been suggested that contaminated food or water may be sources of infection, but the evidence is not conclusive. In the United Kingdom, over half of people aged 50 years or over are infected with H. pylori. Gastroscopy is a reliable diagnostic tool for H.pylori infection besides methods such as blood serology, stool sampling or breath test. For most people, once the infection is acquired, it persists without treatment.

BP Healthcare Group is currently introducing this new diagnostic tool to its current comprehensive screening packages to further suit its customers' needs. Starting from 2012, upper gastroscopy service is available at the BP Cheras Diagnostic Centre. Done as a daycare procedure by a trained physician, it will be used to screen for any digestive-related problems especially among H.pylori-infected patients and first degree relatives of gastric cancer patients.

What is CT Scan?Computed Tomography is an imaging method that uses x-rays and computer to create cross-sectional pictures/images of the body. It has been used as medical imaging since 1972 and has evolved into the 4th generation presently. CT has developed from single slice CT to multi-slice CT (MSCT) and dual source CT over a duration of 39 years. There is great improvement in speed, patient comfort and resolution with present generation of CT. With this advanced technology, CT provides excellent image quality for diagnostic confidence at the lowest possible radiation dose. A whole body CT will take about 30 sec to complete with 64 MSCT.

CT provides greater clarity and reveals more details than regular x-ray/ultrasound exams of internal organs, bones, soft tissue and blood vessels. It helps doctors and radiologists to diagnose cancers, cardiovascular disease, infectious disease, appendicitis, trauma, musculoskeletal disorders etc.

What are the uses of CT?CT can be used to scan any part of the body. It is one of the best and fastest tools for studying the chest, abdomen and pelvis because it provides detailed, cross-sectional views of all types of tissue. Commonly done CTs are head, neck, chest, abdomen and pelvis. CT is mainly used to solve clinical problems. Physicians often use CT examination to:

Quickly identify injuries to the lungs, heart and vessels, liver, spleen, kidneys, bowel or other internal organs in cases of trauma.

Guide biopsies and other procedures such as abscess drainages and minimally invasive tumor treatments.

Plan for and assess the results of surgery, such as organ transplants or gastric bypass.

Stage, plan and properly administer radiation treatments for tumors as well as monitor response to chemotherapy.

With tremendous research and development in CT, health screening using CT is gaining momentum and is more acceptable. New generation of CT offers good diagnostic image quality with low radiation dose. Currently, low dose CT lungs, CT colonography and cardiac CT for calcium scoring and coronary angiogram can be offered as health screening tool with guidelines. Many studies had proven that these screening CT help to reduce morbidity and mortality as well as healthcare cost.

Lung cancer is the 3rd commonest cause of cancer death in Malaysia. In the United States, the lifetime risk of developing invasive lung cancer is 1 in 17 for men and 1 in 18 for women. It is estimated that over 80% of lung cancers could be cured if detected at an early stage. CT is more sensitive than chest X-ray in detecting lung cancer. Studies show that there are 20% fewer lung cancer deaths screened with low-dose MSCT compared to chest X-ray.

The indications for screening CT lungs are:

History of lung cancer

10 packs/year or more smoking history (1 pack of cigarettes per day for 10 years, 2 packs of cigarettes per day for 5 years, etc.)

Past history of smoking (less than 10 years ago)

Repeated exposure to secondhand smoke

Exposure to other cancer-causing agents (e.g. asbestos and radon)

Age more than 40 years

Colorectal cancer is the 2nd commonest cause of death due to cancer in Malaysia. Colorectal cancer is the third most common cancer diagnosis in the United States and the second leading cause of cancer-related deaths. Each year, about 150 000 people diagnosed of colon cancer, and about 50 000 will die of it. However, if detected early, the 5-year survival rate for colon cancer exceeds 90%. Recent data studies showed CT colonography is comparable to optical colonoscopy for the detection of cancer and polyps of significant size (more than 1cm).

The indications for screening CT lungs are:

Older than 50 years

Have had polyps in the past

Family history of colon cancer

Test positive for blood in the stool

Elderly patients who are frail or infirm - difficult for colonoscopy.

When colonoscopy is attempted but incomplete because the full length of the colon is not visualized, CT colonography may be done on the same day.

If a tumor is found on colonoscopy and it blocks passage of the scope, CT colonography will permit a thorough check of the entire colon.

Ischaemic heart disease or coronary artery heart disease (CAD) is the number one cause of death in Malaysia and leading cause of death worldwide. In America, it is the commonest cause of death above 20 years old. Cardiac CTs are used for calcium scoring and coronary angiography. Calcium is a marker of CAD, the amount of calcium detected is a helpful prognostic tool. The higher calcium score, the higher the risk of CAD. Studies have shown that the results of CT coronary angiography are as good as conventional invasive coronary angiography.

The major risk factors for CAD are:

High blood cholesterol levels

Family history of heart attacks

Diabetes mellitus

High blood pressure

Cigarette smoking

Overweight or obese

Low physical activity/sedentary lifestyle

These risk factors can be used as indications for cardiac CT.

Why CT scan is better than X-ray?CT scans produce detailed cross sectional images of the body that appear to open the body up, allowing doctor to look at it from inside, where else X-ray or radiograph only allows doctor to look ‘through’ the body. CT is better than X-ray because:

Enables to see and differentiate soft tissue e.g. muscles, liver, kidneys, brain, eyes etc.

Detects smaller lesion; 3mm compared to X-ray (10mm).

Detects subtle fracture which cannot be seen in X-ray.

Detects subtle changes in the organ e.g. interstitial lung disease, early stroke etc.

Images can be processed into 3D and multi-planes.

Able to view narrowing of blood vessels and their course.

Able to see gastrointestinal tract clearer and process the images into virtual colonoscopy/endoscopy.

Able to do perfusion scan of brain.

Able to visualise the paranasal sinuses clearer.

Provides better visualisation of prolapsed intervertebral disc and spinal canal stenosis.

CT Scan in BP RadiologyBP Diagnostic Centre has been offering general X-ray and ultrasound examinations in the diagnostic centres (DC) since 1996. Today, the radiology services have expanded to include digital X-ray, mammography, teleradiology and DEXA for more accurate and better quality health screening.

Recently, BP Radiology has launched CT scan services for health examination in Specialist Centre Glenmarie and Cheras, using one of the advanced, faster and higher resolution multi-slice CT (MSCT). It is able to perform Head2Toe CT within few minutes. The flagship 128 MSCT scan will be placed in Specialist Centre Ipoh by the second quarter of 2012.

The CT examinations will also be offered to private clinics and other BPDCs. By teleradiology, the CT images will be interpreted and reported faster. The CT reports will be sent by internet or email to respective diagnostic centres or private clinics which referred the patients.

Computed Radiography (CR) uses very similar equipment to conventional radiography except that in place of a film to

create the image, an imaging plate (IP) made of photostimulable phosphor is used. The imaging plate is housed in a

special cassette and is placed under the body part before the x-ray exposure is made. Hence, instead of taking an

exposed film into a darkroom for developing in chemical tanks or an automatic film processor, the imaging plate is run

through a special laser scanner, or CR reader, that reads and digitizes the image. The digital image can then be viewed

and enhanced using software that has functions very similar to other conventional digital image-processing software,

such as contrast, brightness, filtration and zoom.

TechNews Computed Radiography

Computed Radiography systems are the most common medical applications used in radiology because they have been

proven for their reliability for over more than two decades and flexibility to address a variety of clinical applications. CR IPs

can be retrofitted to existing exam rooms and used in multiple x-ray sites since IPs are processed through a CR reader

(scanner) that can be shared between multiple exam rooms. The advantages of CR compared to conventional X-ray are:

No silver based film or chemicals are required to process film.

Reduced film storage costs because images can be stored digitally.

Computed radiography often requires fewer retakes due to under- or over-exposure which results in lower overall radiation dose to the patient.

Image acquisition is much faster - image previews can be available in less than 15 seconds.

By adjusting image brightness and/or contrast, a wide range of thicknesses may be examined in one exposure, unlike conventional film-based radiography, which may require a different exposure or multiple film speeds in one exposure to cover wide thickness range in a component.

Better images quality - can be enhanced digitally to aid in interpretation.

Images can be stored on disk or transmitted for off-site review and report by teleradiology.

Lower cost to operate because no chemicals, dark room storage and staff required to organize them.

CR in BP RadiologyBP Radiology started to use CR for general X-ray and mammography since year 2007 at Cheras and Ipoh Diagnostic

Centres (DC). At the same time the teleradiology services was also launched with reporting centre at Cheras DC. In

combination with application-specific plates and cassettes, CR is able to support a broad range of applications:

General Radiology

Orthopaedics

Extremities

Dental (will be available soon)

Peadiatrics

Mammography (only in Specialist Medical Centre Glenmarie, Cheras and Ipoh)

CR has been installed nationwide in all BPDCs. By using teleradiology, digital images of CR from other DC will be

transmitted to Glenmarie, Cheras or Ipoh Specialist Medical Centre for interpretation and reporting. All the digital X-ray will

be read and reported by radiologists in these centres. The X-ray reports will be accurate and fast, therefore minimising error

in reporting.

CR will also be installed in general practitioner clinics (GP) which subscribe for X-ray services from BP Radiology. For GPs

who subscribed for these services, the reports for all the X-rays will be done by a group of radiologists in BP Radiology,

and therefore, reducing the workload and reporting error of the GP.

BP Healthcare Group, apart from medical testing and diagnostic services, started its industrial testing division (heavy metals analysis) since year 1996. In year 2004, BP Food Testing Sdn Bhd was established to provide comprehensive food microbiology and chemical analysis.

The food testing laboratory has been equipped with various high-end instruments including Graphite Furnace Atomic Absorption Spectrometer, Flame Atomic Absorption Spectrometer, Flow Injection Atomic Spectrometer, Ultraviolet-Visible Spectrophotometer, High Performance Liquid Chromatography, Gas Chromatography and Gas Chromatography Mass Spectrometer.

With the largest network (more than 77 collection centres throughout Malaysia), BP Food Testing received numerous bird’s nest samples for analysis, including nitrite, microbiology and heavy metals testing.

In recent years, economic growth has led to high demand for quality of life and a better environment. Amidst rapid growth of the manufacturing sector, the awareness of good quality products and environmental impact has begun burgeoning among society. However, with the lack of internal laboratories within the manufacturing factories, outsourcing of test samples to commercial laboratories are common practice in Malaysia, in order to comply with Department of Environment (DOE) requirement. This trend is due to high investment on establishing and maintaining a well-equipped laboratory.

Our BP environmental testing laboratory for water, wastewater and oil analysis will be equipped with comprehensive high-end instrumentations to include Total Organic Carbon Analyzer, Ion Chromatography, Inductively Coupled Plasma Optical Emission Spectrometer and Inductively Coupled Plasma Mass Spectrometer.

Inductively coupled plasma optical emission spectroscopy (ICP-OES), also referred to as inductively coupled plasma atomic emission spectrometry (ICP-AES), is an analytical technique used for the detection of trace metals. It is a type of emission spectroscopy that uses the inductively coupled plasma to produce excited atoms and ions that emit electromagnetic radiation at wavelengths characteristic of a particular element. The intensity of this emission is indicative of the concentration of the element within the sample.

Examples of the application of ICP-OES include the determination of metals in wine, arsenic in food, and trace elements bound to proteins. ICP-OES is often used for analysis of trace elements in soil, and it is used for motor oil analysis.

Analyzing used motor oil reveals a great deal about how the engine is operating. Parts that wear in the engine will deposit traces in the oil which can be detected with ICP-OES. ICP-OES analysis can help to determine whether parts are failing. In addition, ICP-OES can determine what amount of certain oil additives remain and therefore indicate how much service life the oil has remaining. Oil analysis is often used by fleet manager or automotive enthusiasts who have an interest in finding out as much as about their engine's operation as possible. ICP-OES is also used during the production of motor oils (and other lubricating oils) for quality control and compliance with production and industry specifications.

Inductively coupled plasma mass spectrometry (ICP-MS) is a type of mass spectrometry that is highly sensitive and capable of the determination of a range of metals and several non-metals at concentrations below one part in 1012 (part per trillion). It is based on coupling together an inductively coupled plasma as a method of producing ions (ionization) with a mass spectrometer as a method of separating and detecting the ions.

The variety of applications exceeds that of ICP-OES and includes isotopic speciation. Due to possible applications in nuclear technologies, ICP-MS hardware is a subject for special exporting regulations.

A typical ICP-MS is able to detect in the region of nanograms per litre to 10 or 100 milligrams per litre or around 8 orders of magnitude of concentration units.

Unlike atomic absorption spectroscopy, which can only measure a single element at a time ICP-MS has the capability to scan for all elements simultaneously. This allows rapid sample processing.

One of the largest volume use for ICP-MS is in the medical and forensic field, specifically, toxicology. A physician may order a metal assay for a number of reasons, such as suspicion of heavy metal poisoning, metabolic concerns, and even hepatological issues. Depending on the specific parameters unique to each patient's diagnostic plan, samples collected for analysis can range from whole blood, urine, plasma, serum, to even packed red blood cells. Another primary use for this instrument lies in the environmental field. Such applications include water testing for municipalities or private individuals all the way to soil, water and other material analysis for industrial purposes.

In recent years, industrial and biological monitoring has presented another major need for metal analysis via ICP-MS. Individuals working in plants where exposure to metals is likely and unavoidable, such as a battery factory, are required by their employer, to have their blood or urine analyzed for metal toxicity on a regular basis. This monitoring has become a mandatory practice implemented by OSHA, in an effort to protect workers from their work environment and ensure proper rotation of work duties (i.e. rotating employees from a high exposure position to a low exposure position).

A Doctor’sPoint of View

The attack on disease must not be meddlesome: the desire to do something must be guided by sure argument that good will come of it. Gibson AG. The Physician’s Art. Oxford: The Clarendon Press, 1933

Health screening has been growing very popular nowadays, but does the public truly understand the concept of health screening and what screening can and cannot do? In my daily practice, I encounter a number of public misconceptions on health screening.

Differing Cases of PerceptionsI would like to share this real life story of Mr. A, who came to see me with his laboratory test results. He said, “Good morning, doctor. I am here to collect my report. I am very afraid to die and please, I want to hear the good news”. Many who come for health screening are mentally prepared to only accept a clean bill of health. This is a fallacy. Having regular health checks do not mean that you will not die or get sick in the future.

The true concept of screening in healthcare is to actively seek and identify a disease or pre-disease condition in asymptomatic individuals. This will help detect a medical condition early on so that intervention or treatment can be carried out. This will lead to improved prognosis and even cure for the detected issue. We must understand that health screening does not prevent death or prevent sickness.

UnderstandingHealth Screening’s

True ConceptBy Dr Leong Hui Shan, Physician

Page 34: BPalive2012 compile WebPdf - BP Healthcare...Holding Sdn Bhd and his partners SEDC Perak, Yayasan Perak and IGB Utara jointly owned Kinta Valley Resort Sdn Bhd investment which has

Another scenario that I encountered involved Miss B, who came for a health check and was told that her blood tests revealed low haemoglobin levels. She would need further tests to be carried out to ascertain the cause of anaemia so that the appropriate treatment could be given. She replied, “Doctor, I thought that the blood tests were complete and I would know everything about my health but why are you still asking me to go for some more tests?”

Health screening is not a one-off event. A screening examination is a preliminary test to detect abnormalities earlier. Subsequent detailed assessment and further investigation will be carried out to ascertain the cause and the degree of severity. In order for treatment to be given, a correct diagnosis must be made. Screening alone does not give complete information regarding a particular disease. Further referral for expert opinion may be needed after screening.

An abnormal result may need to be monitored to assess its trend of change. Further investigation may be needed to verify those who had been screened positive for a condition to really have the abnormality (true positive), and to eliminate those who were screened positive but do not actually have the abnormality (false positive). So I would say that health screening is just the beginning of a journey to achieve empowerment of a person’s own health.

Being Proactive About HealthFinally, health screening is not and can never be a universal remedy. If you ask me as a healthcare provider on whether I will encourage the public to go for health screening, my answer is certainly yes. As long as we perform screening on the basis of sound research evidence, then screening will remain a powerful tool in the fight against disease.

PhysicianThe term "physician" is used somewhat differently in different countries. In the United Kingdom and Commonwealth nations, a physician is a specialist in Internal Medicine while in the United States, it is a broader term used for a doctor who practises Family Medicine or General Practice. Instead, the term “internist” is used in the US to describe such a specialist.

Internal medicine is a medical specialty dedicated to the diagnosis and medical treatment of adults. We are trained on learning the prevention, diagnosis, and treatment of diseases of adults. While physicians typically do not perform surgeries, we find that with the evolution of medicine, we now perform some non-invasive or minimally invasive procedures like scopes, biopsies, and angioplasty.

Healthy from WithinOne of the most important parts of our role is bringing to our patients an understanding of wellness, which encapsulates disease prevention and the promotion of health. Diseases such as diabetes, hypertension, and dyslipidemia are some of the issues physicians may treat and help manage on a regular basis. Often, physicians may consult or refer the patient to see a more specialized physician if a more serious issue arises.

Physicians can choose to focus their practice on general internal medicine, or may take additional training to "sub-specialise". Cardiologists for example, are physicians who sub-specialise in diseases of the heart whilt other examples of sub-specialties are endocrinology, hematology, neurology, gastroenterology, nephrology, pulmonology, rheumatology and allergy and immunology.

Simply put, a physician is a specialist who practices medicine related to the organs and systems of the body. In BP, we often work closely with the rest of our in-house medical officers and other specialists. We are committed to focus on the art of diagnosis and treatment, preventive medicine and emphasis on patients’ education towards maintaining a healthy life.

Too often physicians are asked about who we are and what we do. While the term means different things in different countries, at the end of the day, physicians take care of your internal wellbeing.

Top Reasons For Medical Check-UpThe pre-employment medical check-up is done for three main reasons:

Safety: It is considered critical that a new applicant be free from medical conditions that could result in sudden incapacitation leading to accident, especially for health- and safety-sensitive jobs such as driving, piloting, construction-related work etc. Incorrect placement of new applicant to a certain job might result in incidents or accidents with high potential for personal injury, plant and environmental damage and harm to the company’s reputation and financial loss. For example, if a company wishes to hire a lorry driver, the medical officer should ensure that the new applicant is free from serious diseases such as uncontrolled hypertension, heart disease and poor vision among others, which can seriously affect the safety of the applicant, co-workers and other road users.

Health: Pre-employment medical check-ups are also meant to establish the baseline health data against which the future health status of the worker can be compared. It is also to identify existing medical conditions which could be adversely affected by occupational exposures.

Apart from the above-mentioned objectives, the employer has other additional objectives for conducting pre-employment examination which include:

Productivity and efficiency: Most employers would like their new recruits to be free from an medical illness which may affect productivity once they are employed.

Cost: The cost of medical treatment is escalating and employers who provide medical care coverage, especially those who provide unlimited coverage, will prefer to hire new recruits without any medical condition to save cost.

With those objectives in mind, it is our duty to advise the company whether the new applicant is suitable for the job or not.

When I joined the BP Diagnostic Centre (BPDC) in 2004, there were only 2 BP Diagnostic Centres nationwide, one in Ipoh and the other, in Subang Jaya. The majority of our customers were walk-in cases with minimal corporate cases. Over the years, as more Diagnostic Centres were set up nationwide, the concept of a one-stop health screening centre began to be well accepted by the general public and corporate clients, with more corporate companies linking-up with BPDC to provide their various healthcare needs. Among the healthcare services provided, the pre-employment medical check-up is top of the list.

Pre-employment medical check-up, as the name suggest, is a medical examination conducted to check the health status of the job applicant and to advise the company whether he/she is fit to be offered the position. It usually consists of history taking, physical examination, blood tests and urine tests.

A, a 30-year old lady came for pre-employment

medical check-up with no significant medical

history. On examination, a suprapubic mass was

palpated. Ultrasound scan confirmed the diagnosis

of a huge uterine fibroid. Fit or unfit for employment?

1 Although a uterine fibroid itself doesn't rule out

employment, since the mass is huge and surgery

removal will be inevitable, it will affect productivity and if

the company does provide medical expenses coverage

for the employee, it will incur extra cost to the company.

In view of this, she was certified unfit for employment, at

least temporary until surgical removal of uterine fibroid

is done.

B and C, young ladies in their early-20s, came

together for a pre-employment medical check-up

after applying to be the cabin crew for one of the

airlines in the country. B was diagnosed to have mild

thoracic scoliosis while C with flat foot. Fit or unfit?

2 B and C were both certified unfit for employment mainly

because the abnormalities detected, i.e thoracic

scoliosis and flat foot, are among the medical

conditions listed by that particular airlines company as

unfit for employment. Although the abnormalities are

not serious and do not affect employment in the

majority of other companies or some other airlines

company, they were certified unfit to work in this case.

D, a 40-year old lady, came for pre-employment

medical check-up to work in one of the banks. She

had no previous medical history. All examinations

were normal except that her chest X-ray revealed

cardiomegaly (enlarged heart) with a cardiothoracic

ratio on 0.56. She was told about the abnormality

detected and was not happy with the finding. The

next day, she came back with another chest X-ray

report given by another specialist centre. This X-ray

report was normal. Her chest X-ray was reviewed by

our in-house radiologist and the finding remained

the same - cardiomegaly. Fit or unfit?

3 D was a special case as there was another contradicting

X-ray report from a different specialist centre. It took me

some time before I could decide whether she was fit or

unfit for employment. To protect the interest of the

company, and despite having another normal chest

X-ray report from another specialist centre, she was

certified unfit for employment. However, if the company

insisted on hiring her, she would need to see a

cardiologist to further assess her condition and to

conduct an echocardiogram to rule out cardiomegaly.

E, a 25-year old lady, came for a pre-employment

medical check-up. All examinations were normal

except her urine was reactive for morphine. She was

not on any medication during the examination. Fit or

unfit?

4 E was certified unfit because her urine was reactive for

morphine. Although this result did not mean that she was

a drug addict, to protect the best interests of the

company, it was best to certify her unfit for employment.

A repeat urine test was not advisable because the second

urine sample could be easily be tampered with and would

affect the accuracy. However, a morphine confirmation

test could be done on the first sample again for

verification but it would incur extra cost to the company.

Hence, it would be up to the company itself to decide on

whether or not to proceed with the confirmation test.

F, a 46-year old gentleman, came for a

pre-employment medical check-up, with a history of

hypertension for the past 2 years and is currently on

medication. His blood pressure was well controlled.

Fit or unfit?

4 F, despite having hypertension, had all examination

results that were normal including his blood pressure.

There was no sign of end organ damage due to

hypertension. He was certified fit for employment.

Verdict: Fit or Unfit? To understand the fitness criteria for pre-employment medical check-up, I would like to share a few clinical scenarios and explain in detail why the applicant was certified fit or unfit for employment in each case.

Pre-employmentMedical Check Up

Tol Free: 1-800-88-7171 Email: [email protected]

For Corporate Services please contact

Although most of the pre-employment medical check-ups are straight forward, we sometimes do encounter difficulties in deciding whether or not to certify the job applicant as fit or unfit for employment. To ensure that we do not go wrong, we stick to a few rules of thumb such as:.

1) Our clients are corporate bodies who send their recruits to us for medical check-up. Our decisions are based on the interest of the corporate bodies, and not the recruits’.

2) Some corporate bodies have their own criteria of fitness for employment, and these actually help us to decide whether a recruit is fit or not. We use their criteria as our guide and if the recruit doesn't meet these criteria, we will certify he/she as unfit.

3) For those corporate bodies without a clear list of medical conditions which deem the recruit unfit for employment, it is the doctor's duty to ensure that the applicant’s fitness can cope with the job. Again, the interest of the corporate bodies shall prevail and the benefit of doubt will be given to the corporate bodies and not the recruits.

Although we are the one to certify whether the candidate is fit or unfit for employment, the final decision of employment is made by the company. Our job is to detect any medical illness, and to inform the company and give them proper advice. In my opinion, anyone can work if they wish to despite having serious medical illnesses. The questions are, does the job they applied for suit them and will offering them a job affect the company’s productivity and incur more cost to the company?

I believe that if given a choice, Apple Inc would keep Steve Jobs as their CEO for another 10 or 20 years despite knowing that he had cancer and it was not curable.

Hearing SolutionsBy William Chan, Director of BP Hearing Solutions

What is hearing loss?Hearing loss can be described at any degree of reduced sensitivity to sound, varying from mild-to-profound. According

to World Health Organization (WHO), common causes of hearing loss include exposure to excessive noise, head and

neck injury, ageing, and use of ototoxic drugs including some antibiotics. Hearing loss may also develop due to unknown

factor or otherwise could be inherited too. Hearing loss can either present at birth or acquired later in life.

There are two types of hearing impairment. Conductive hearing impairment is a problem in the outer or middle ear and

often it is medically or surgically treatable. A common example is chronic middle ear infection. Sensorineural hearing

impairment is a problem with the inner ear, or occasionally with the hearing nerve. It is usually permanent and requires

rehabilitation such as the use of a hearing aid.

What is the effect of hearing loss on you?You might be surprised to know that a typical person with hearing loss waits over five years to see a health care

professional for an evaluation. Although hearing loss is not life threatening, it significantly lowers the overall life quality for

those suffering from it. Most sufferers found themselves embarrassed with their hearing loss and thus gradually withdraw

themselves from social activities. Hearing loss can lead to communication breakdown, misunderstanding, low

self-esteem, depression and loneliness. Furthermore, inability to hear warning sounds such as car horn or fire alarm

might become fatal. Therefore, denying hearing loss might not be a good solution at all. Early detection and intervention

could make a difference.

What is our solution? We offer affordable solution for wide range of hearing loss within quality and premier product range from Beltone. Discreet

and light-weighted Receiver-In-Ear (RIE) hearing aids provide less visible solution for your hearing problem. We

exclusively present the world's smallest RIE hearing aid, True 17 from our premier product range for advanced listening
