    BR COMMENTS Compliance PMC EXPLANATION Remarks

    1 Approval of Concrete Mix Designs - Till date

    Approved M80, M70, M50, M45, M40, M30 & M5 Grade of Concrete, however the following criteria are

    missing

    Temperature of Concrete in any one of the trial not yet recorded YESAll the trial mixes have been performed/endorsed by the 3rd party independent testing agency (NABL Accredited), then all the mix designs

    were revalidated at project site after commissioning of RMC plant with 3 batches of trials which provides the details of

    workability/temperature. These have been formalized through CWIR process.

    As discussed/agreed, CWIR copies

    provided to BR for record on 12th

    Oct 2015.

    As per Specifications for Structural Concrete dated April 2015 Rev 01, Clause 4.5 A,

    B, C & D are not fully fulfilled in any one of the trial

    RCPT (Rapid chloride permeability test) conducted once as per the orignal specifications, and in the new project specification, the tests

    related to durability has been deleted.

    M5 & M40 Grade of Concrete fly ash is used, however no third party test reports for fly ash are available till

    dateN/A

    Primary mix design for M40 is approved with GGBS, and the alternative mix design with flyash have been approved for future purpose for the

    upper floors/in case of any emergency. M40 with flyash is approved on the basis of MTC provided by the p roduct manufacturer.

    As per Specifications for Structural Concrete dated April 2015 Rev 01 Clause 4.5 H Heat of hydration test

    cube(s) for M70 & M80 shall be prepared to mimic conditions on the site - not complying this requirementYES Please refer RFI : D09-C01-RFI-0070, Dated : 07th April 2015, where in this requirment is waived.

    Oct 2015. ITP (Inspection and Test Plan) for Site RMC Plant - inadequate information. For

    example, Impact Value and Crushing Value test are critical when using M70 & M80

    Grade of Concrete and ITP states only Third Party test at 6 months interval not

    acceptable

    YES Raw material testing requirements for the aggregates have been followed in accordance with IS 4926, Annex B.

    Specifications for Bonded Post Tensioned Concrete dated Feb 2015 does not specify

    the concrete requirements - no RFI (Request for Information) raised till date and

    Concrete Mix Design approved and concrete poured at site.

    As per the GFC Drawing related to PT Works - The Grade of concrete is clearly defined in Doc.No.JW-ST-DWG-NT-SB2-3 R1. So, no further

    details required. To take care of this, the Mix d esign is designed for 28D strength and n ot for 56 days, eventually the stressing operations will

    be carried out as per PT designer recommendations and the min.value attained on compressive strength of the concrete.

    Couplers - Awarded to JB Engineering which is not listed in D09-SPEC-GEN-002 List of Approved

    Materials, Suppliers & Vendors - the undersigned checked with HILL / L&T Quality Representative.

    They have no past experience in performance of JB Engineering and no slip test was conducted andfactory visit report not seen, which was also concern?

    As per D09-SPEC-ST-001 Rev.01, Sec.2.10 - All reinforcing bar couplers to beAncon/Barteck or similar approved and available in india

    ,

    subject to the meeting the requirements on elongation, Yeild stress and comliance to new IS code for couplers i.e. IS 16172. JB Engineering

    has provided necessary test reports and met the above conditions. Provided test reports are in compliance with IS 16172 i.e.

    Strength/Ductility test under static forces/Slip under static forces/Cyclic Tensile test/Low cycle fatigue/High cycle fatigue. Tests have be en

    performed in IIT Roorke / other accredited labs. JB Products have been used in various ICONIC projects in india vide, those are :

    NHPC,NHAI,Chennai Metro, Delhi Metro, IRCON, Gammon/HCC high-end profile projects. Apart from the above, the delivered couplers at site

    have been testing in approved 3rd party agency (GLOBAL LAB.) at regular in tervals. Test reports and the results are sasifactory and meets the

    project specification requirements.

    On 09th October, Coupler

    Manufacturing company (J.B.Engg)

    provided presentation and shown

    the test reports as per IS code

    requirements. ITP for Threadingprocess will be issued by 17th

    Oct.2015 with the QC checklist to

    monitor the cutting/forging &

    threadin rocess.3 Rebar Cutting and Bending Works:

    No ITP in place YES

    Cutting & Bending of reinforcement is addressed i n approved Method statement for Reinforcement works, Ref : DTF - 0155, L&T Cut/Bent of

    rebars are in accordance with BBS/GFC Drawings, PMC is carrying out the Reinforcement inspection checks through CWIR/Checklist process

    for the activity. Rebar profiles will be inspected during reinforcement installation inspection which is having a separate ITP, Ref : DTF-0274

    Not seen Automated Stirrup Bending Machine - manual bending is under way which

    increases the chance of human errorsN/A L&T/HILL will ensure that the bars are in accordance with the pr ofiles approved as per GFC Drawings/BBS submitted.

    Visited Tower 2 ICC column C 13 Non Tower Area, L length found to be 280 mm and short by 20 mm N/AThis particular area is not offered for the inspection. L&T QA/QC check s are balance, then CWIR will be submitted to PMC for final

    inspection/approval.

    PMC ensured that the bars are

    replaced whatever deficient.

    Form work System - as per Form work design calculations the thickness of plywood mentioned is

    12mm densified - However this item comes under Temporarily Works Design, Form work design

    verification and validation by competitive authorities report not seen.

    As per D09-SPEC-ST-001 Rev.01, Cla.4.9 - The design of formwork shall be the responsibility of the contractor for all in-site and pre-cast

    elements. L&T has a separate Formwork division and managed by the competent design team, PMC has a sked L&T to submit the CV of

    Formwork design head, then L&T submitted vide letter ref : D09-C01-HIL-0136, Dated : 27th March 2015. The same is reviewed and

    approved by the Structural consultants. Further at site level, L&T Formwork/QC team checks the support system as per the design

    calculations/drawings during CWIR process.
