branching out in support of its charitable mission. From our humble beginnings over 100 years ago to the recent design and construction of a new state-of-the-art hospice inpatient facility, VNHC continues to raise the bar for community-based healthcare.

OUR MISSION Nonprofit since 1908, VNHC provides high quality, comprehensive home health, hospice and related services necessary to promote the health and well-being of all community residents, including those unable to pay.

Each year the Annual Report provides a valuable opportunity to assess the many accomplishments and challenges experienced in the prior year. In reviewing the last year’s events, ‘branching out‘ emerged as a common topic and therefore, was selected as the theme for our 2010 Annual Report. In these pages, you will read about the numerous ways we have ‘branched out’. Here are a few highlights.

Perhaps the most visible example is our new Serenity House. If you’ve driven down Carrillo Street towards Santa Barbara lately, you may have noticed how far the construction on the new House has progressed. The opening is close at hand and we are honored to be able to offer this important option to our hospice patients and their families. Recently Don Schumacher, President of the National Hospice & Palliative Care Association, was given a tour and declared, “This will be the most beautiful hospice house in the entire country!” Thanks to the generosity of individuals and foundations, the new Serenity House will double the number of beds available by the end of 2011. We are also ‘branching out’ when it comes to providing home health, hospice and personal care services to more members of our community. This was especially evident in the Santa Ynez and Lompoc Valleys. We also re-located and enriched our Bereavement Program to better meet the specific needs of those who have suffered a loss. We enhanced our Integrated Therapies Program, adding healing touch to increase comfort and relaxation for our hospice patients. In another arena, we are hoping you have already “liked” us on Facebook. 2010 saw us ‘branching out’ into new forms of communication, including Twitter, LinkedIn and a blog – all of which can be accessed through our expanded website at www.vnhcsb.org. As we have since 1908, we continue to embrace new endeavors, to grow in the ways we serve our community. We are able to do this because of your generous and ongoing support. This report includes a list of all of the individuals, organizations and foundations that contributed to Visiting Nurse & Hospice Care throughout 2010. We are truly grateful to our donors as well as the patients and families it has been our privilege to serve.

thE SaNSUM clINIc, Carpinteria Branch, is a multi-specialty clinic with a staff of 25. This local, community-based clinic is the largest private health care provider in Carpinteria, serving residents of all ages. The Family Medicine team includes Drs. Thomas Beamer, Ali Javanbakht, Rachel Trautwein, and Carolyn Murphy. Dr. Rayna Talanian practices Internal Medicine and Dr. Eduardo Clark addresses patients’ Urgent Care needs. Our staff say this clinic is “compassionate,” “approachable,” and “easy to work with.”

thE cENtER fOR WOUNd MaNagEMENt provides a full range of services for effective wound treatment and management of associated medical conditions. The expert team of professionals, led by Drs. John Deacon, John Dexter and Salvatore Garofalo, are specially trained and licensed in wound care. The Center recently expanded services to Santa Ynez Valley Cottage Hospital. Among the ways our staff describe this clinic are “thorough,” “easy to work with,” and “the best in town.”

ur programs in Santa Ynez and Lompoc Valleys have grown steadily since we established a local office in 2008. Our

efforts to build relationships and increase community awareness are producing results. Home Health admissions grew

7%, serving 275 patients. Hospice served 77 patients, a 29% increase over 2009. In addition to providing patient care services

at home we work with many community facilities including the Center for Wound Management at Santa Ynez Valley Cottage

Hospital, Atterdag Village of Solvang, Solvang Friendship House and Lompoc Skilled Facility.

Throughout the year VNHC provided numerous presentations and clinical in-services to community groups, physician offices

and hospital staff. In March we offered our first Hospice Volunteer Training in Santa Ynez and hosted a Remembrance Service

in June at Solvang Friendship House. Additionally, we helped provide flu shots at SYVCH Health Fair and Lompoc Senior

Expo in the fall. Finally, in December we hosted Angels Among Us, our annual tree lighting event, at Santa Ynez Valley First

Presbyterian Church. 2010 marked the first year we have hosted that event in north Santa Barbara County.

Making a Difference

VNHC’s hospice services are augmented by a most valuable treasure – our volunteers. They provide that helping hand in every area of hospice. With the expert guidance of Arlene Stepputat, who joined our staff as Volunteer Coordinator in October 2010, and Associate Coordinator Michelle Cervantez, the volunteers provide special care to patients at home, assistance at Serenity House, transportation to appointments, beautiful relaxing music and integrated therapies such as therapeutic touch, massage and Reiki to patients, family members and caregivers. With over 95 volunteers ranging from adolescent to much older, many individualized patient needs are attended to.

Our volunteers have a wide range of experience and talents to share. They go through an extensive training that prepares them to be an integrated part of our team. Two new programs introduced in 2010 are our “Bouquets of Love” flower delivery program where volunteers, in collaboration with the Dream Foundation, deliver flowers to designated hospice patients every Saturday, and our new pet therapy program “Faithful Friends.”

petite woman in her late 40’s, Janet lay in her hospital room propped uncomfortably against

the stiff pillows on her bed. She was breathing with difficulty even though oxygen flowed quietly through the tubes in her nose. Janet’s appearance contradicted the fact that her lung cancer had progressed significantly and there was nothing more her doctors could offer in terms of treatment. As one of her doctor’s entered the room, Janet held back tears when asked how she was feeling. “I want to go home... see my garden. I hate being in the hospital,” she said. Her doctor agreed and following a brief check of her vitals, discussed her options for hospice care and invited a liaison from Visiting Nurse & Hospice Care to stop by for an orientation and evaluation.

The very idea of hospice and its association with dying went against Janet’s stubborn conviction and fear of accepting defeat. However, as she listened to the VNHC liaison explain the many benefits of hospice – the comfort,

symptom relief and quality of care she would receive –

Janet finally agreed she had nothing to lose. The thought

of being in her garden, not having to return to the hospital

and of the compassionate assistance she would be getting

from the hospice team provided great comfort and relief to

both her and her husband.

Janet’s hospital bed was placed before a large window looking out at a flowering pink hibiscus with birds singing on branches hovering delicately over the garden below. With the help of the VNHC hospice team, she received a

doctor-supervised medical regimen set up to manage symptoms while a social worker helped Janet and her husband find acceptance and express feelings they may not have been able to convey to each other otherwise. A chaplain read her poetry and one of the hospice volunteers started providing massages to help her relax.

As her health declined, Janet began to accept that she was dying, but she did not stop her courageous fight to live. One of Janet’s final wishes was to see the sunset one more time. On a balmy fall evening, her husband and

the hospice chaplain carried her to the car, oxygen tank in tow, and drove to Butterfly Beach in Santa Barbara.

As the glowing sun met the horizon, the three held hands and watched as Janet breathed in one last sunset. Just off shore, a family of dolphins surfaced as if to say their own goodbye. She couldn’t giggle, but she squeezed her husband’s hand and in that near perfect moment, Janet knew the time had come to say goodbye.

Shortly after her visit to Butterfly Beach, Janet passed away peacefully in the comfort of her own home. Her husband said she had become so tiny at the end that she had to use every last molecule of her energy just to take a breath. Janet’s determination to enjoy and appreciate her life was an inspiration to each person who cared for her. And with the support she received from her family and VNHC staff, Janet was able to enjoy her final days with dignity and comfort among people she loved.

amantha, a 41-year-old single mother from Goleta, was diagnosed with breast cancer two years ago following a routine physical exam. Although the cancer was detected early, her condition, coupled with a rare metastatic disease involving

her bones, was painful. The pain and fatigue severely limited Samantha’s ability to care for her two children and continue working. Her oncologists focused intensively on a curative course of treatment that would work into her busy schedule. However, the burden of fighting the disease was overwhelming and she agonized over the thought of not being there to watch her children grow up.

Eventually the pain got so bad, Samantha was unable to sleep or eat, spending much of her day curled up in bed and relying on outside help to get her kids to and from school. At this point, her longtime internist referred her to the Community Palliative Care program, a collaboration among VNHC, the Cancer Center of Santa Barbara and Hospice of Santa Barbara, which provides a comprehensive approach to patients and families facing a life threatening illness. Within days of beginning the program, her palliative team of doctors, nurses, social workers and spiritual counselors had her pain under control and she was up walking around, sleeping and eating – back to a relatively normal life with her family. Samantha recently completed her latest course of chemotherapy and her oncologist feels her scans are showing a good response to the treatment.

VNHC offers comprehensive skilled nursing and rehabilitation services to community residents throughout Santa Barbara including Santa Ynez and Lompoc Valleys. Our Home Health team provides compassionate, quality care to patients 24 hours a day, 365 days a year. Services a patient may receive include:

• monitoring vital signs, pain

levels and other symptoms

• administering medication, wound care and injections

• physical, occupational and speech therapy

• social, emotional and

• medical equipment loan

through our loan closet

2010 HOME HEALTH SERVICES BY CATEGORY

ne crisp evening last October, 82 year old Pat was walking to the kitchen when her right hip, replaced in 2008, gave out. Following a painful fall and a trip to the emergency room, Pat learned her new hip had been recalled several

months earlier by the implant manufacturer. Doctors at the hospital informed Pat she’d suffered a small concussion and a fractured pelvis for which she was sent home with crutches and a variety of pain killers to lessen the pain.

Having lived alone for 40 years and always extremely active, Pat was stunned and not sure how she would manage. She couldn’t drive, couldn’t shop for food, couldn’t do laundry or anything but the most simple jobs around the apartment. Her doctor called a home-health agency but their services cost more than she could afford. On crutches and in considerable pain, Pat was desperate. Finally, she called her doctor back and asked for a referral to Visiting Nurse & Hospice Care. He did and by the end of the day, one of our nurses was at Pat’s home evaluating her needs and scheduling a variety of services including physical and occupational therapy, personal care to help with laundry and light house cleaning, and even a walker and bath bench from our Loan Closet. Within weeks, Pat was back on her feet and doing the things she loved thanks to the compassionate care she received from the nurses at VNHC.

oanne’s husband Ron did not want a caregiver. In his 80’s and suffering from a number of age-related conditions, he no longer had the ability to help his wife out around the house, let alone himself. Joanne argued with Ron for months about

getting some help until his doctor finally convinced the stubborn husband that his wife needed a break.

Since becoming a full time caregiver, Joanne had lost over 15 lbs. and increasingly suffered from fatigue and back pain due to severe osteoporosis. Ron’s doctor explained that respite care, temporary relief to those caring for family members, would be of great benefit to both he and his wife who could no longer do everything on her own. He recommended they contact Visiting Nurse & Hospice Care and within a couple days, a qualified personal care aide was in their home to help prepare meals, take care of some light housekeeping and most importantly, offer companionship to Ron so Joanne could leave the house for a few hours to run errands and enjoy the company of friends.

“I call her my angel. My husband and Emma just click together and when I get home, he has a big smile on his face. Ron really trusts her and is always happy when I return,” Joanne said of her respite care.

VNHC offers patients a variety of personal care services to help with daily activities such as bathing, grooming, meal preparation, shopping and transportation. We also provide licensed geriatric care management to help coordinate a patient’s care options. Some of these services include:

What is Personal Care?

Personal Care

n 2010, we completed implementation of our electronic medical record system which began at the very end of last year. All of our programs – home health, hospice and personal care – now utilize one system for electronic records ensuring

that timely and accurate medical information is available around the clock. Additionally, all core staff are now equipped with mobile phones that allow them to access an electronic medical record wherever they are. Because our work is done in patient homes, this ease of access is important to ensure that medical information for our patients is always up to date. The electronic record also provides a seamless transition between VNHC staff and a physician’s office. We communicate more efficiently and more effectively, making sure that patients receive the highest quality care.

This investment in technology is an investment in better patient care. We know that by improving our ability to communicate internally and externally, we improve our ability to serve our patients and their families, making technology work for us.

isiting Nurse & Hospice Care’s 6-bed hospice inpatient facility, Serenity House, celebrated its 15th Anniversary in July 2010 and was honored to care for its 1000th patient. While the health care industry has experienced significant

changes over the last decade and a half, the mission of Serenity House has not. It is a special place where a person can live their remaining life with high quality care, comfort and dignity – offering round-the-clock medical care from a team of specially trained doctors, nurses, social workers, spiritual counselors, home health aides and volunteers who support emotional, spiritual and physical needs.

Tending to the physical, emotional and spiritual needs of our patients and their families calls on the talents of an exceptional group of people – our staff and volunteers. Several of the clinical staff attended workshops, conferences and trainings in 2010 to enhance their knowledge and skills while providing them with tools to ensure a home-like environment of peace and serenity.

Our phenomenal volunteers are an essential part of the hospice team. Numbering more than 30, one member of this extraordinary group can be found at Serenity House from 9 AM until 7 PM every day of the week.

In 2010, Serenity House saw a 10% increase in admissions, 122 compared to 111 in 2009. Patient days grew from 1,872 in 2009 to 1,970 in 2010, representing a 10.5% increase; and our average daily census rose from 5.1 to 5.4 days.

To address this growing need, construction continued in 2010 on a new Serenity House on Miramonte Drive in Santa Barbara to provide more end-of-life beds for patients from throughout the County. The new Serenity House, made possible through the generous support of the community, is scheduled to open in the spring of 2011.

The Viagenco project served over 1200 patients in 2010 including 221 home visits to palliative care patients in Mbita, kenya. In addition, over $32,600 was raised by VNHC staff to support education initiatives for about 150 orphans.

We also receive tremendous annual support from

community members and we are introducing our

new Leadership Giving Councils in this report. Our

Chairman’s, Centennial and Founders’ Council members

make annual gifts to support subsidized care and

community programs that make it possible for us to

generously care for our community today.

Finally, we thank those capital campaign donors who are

making the new Serenity House a reality. As construction

draws to a close and we begin to prepare the new house

for patients, we feel blessed to have received the support of

so many in our community for this important effort.

To fulfill our charitable mission to provide care to all those in need, Visiting Nurse & Hospice Care provided $1,330,827 in subsidized care in 2010. All of our services – home health, hospice and personal care – are available on a sliding fee scale or, when all personal financial resources have been exhausted, at no charge. No one in need of care is turned away because of an inability to pay. It is the generous support of the community that allows us to ensure that no one suffers needlessly and alone.

In 2010, we received just over $1million from 10 different estates where individuals had remembered VNHC in their will or estate plan. These gifts, in combination with those of our other Samaritan Society members, allow us to plan for the future and help us maintain our services in perpetuity with a continued goal that no one is turned away because of an inability to pay.

promote the health and well-being of all community residents, including those unable to pay.

OUR MISSION

Many of the branches that we have begun in

2010, we will continue to grow and extend in

2011. With the opening of the new Serenity

House, we will have increased capacity, doubling

the number of patients cared for in the house by

the end of 2011. We will continue to grow and

expand to serve more patients and families in the

Santa Ynez and Lompoc Valleys. We will continue

to enhance our electronic medical record and the

technology that supports it, knowing that this

investment is a direct investment in improved

patient care.

However, perhaps most importantly of all, we

will continue to strengthen our commitment to

our charitable mission, serving all those in need,

including those unable to pay knowing that

this bedrock commitment is the essence of all

that Visiting Nurse & Hospice Care does in our

community.
