ELIGIBILITYTo be eligible for health care coverage through BCCPT, you must be:

• Under the age of 65;• A resident of Pennsylvania;• A US citizen or eligible non-citizen;• Screened and diagnosed with breast or

cervical cancer, or a precancerous condition of the breast or cervix;

• Uninsured or have insurance coverage that does not include breast or cervical cancer treatment.

NOTE: There are no income or resource limits for BCCPT.

GETTING STARTEDTo be eligible for health care coverage through BCCPT, you must first be screened and diagnosed with breast or cervical cancer, or a precancerous condition of the breast or cervix, through the HealthyWoman Program or through another health care provider. If you are screened by another provider or need further tests, you can be referred to a HealthyWoman Program provider in your community for the next steps in screening and diagnostic testing.If you are determined eligible for BCCPT, your health care coverage begins the date you are screened and diagnosed with breast or cervical cancer, or a precancerous condition of the breast or cervix, and will last through the length of your treatment. Your continued eligibility will be reviewed prior to the expected end of your treatment, or at least once a year.

What is the HealthyWoman Program?The HealthyWoman Program is a breast and cervical cancer early detection program of the Pennsylvania Department of Health. It provides the following free services for eligible women:

• Clinical breast exams

• Mammograms

• Pelvic exams

• Pap smears

• Follow-up diagnostic tests for abnormal screening results

What is the Breast and Cervical Cancer Prevention and Treatment Program?

The Breast and Cervical Cancer Prevention and Treatment (BCCPT) Program is a Medical Assistance program offered by the Pennsylvania Department of Human Services. It provides full health care benefits to women needing treatment for breast or cervical cancer, or a precancerous condition of the breast or cervix. Health care benefits include, but are not limited to:

• Doctor or clinic visits

• Lab tests and X-rays

• Prescriptions

• Hospital visits, including surgery

• Home health agency services

• Medical supplies and equipment

• Emergency care

HOW TO APPLYHere are two quick and simple ways to apply for BCCPT:

1. Through the HealthyWoman program:

• Call the HealthyWoman Program at 1-800-215-7494 to find out if you qualify for free screening and find out the location of the closest screening clinic.

• Have a screening or diagnostic test for breast or cervical cancer provided by the HealthyWoman Program.

• The HealthyWoman Program will assist you with the BCCPT application (PA 600 B) and send the completed application to the local county assistance office (CAO).

2. Through your health care provider:

• If you were diagnosed with breast or cervical cancer, or a precancerous condition of the breast or cervix, by a health care provider not part of the HealthyWoman Program, your provider can download the PA 600 B at www.healthywoman.health.pa.gov.

• You and your provider will fill out the PA 600 B and the provider will fax it to the HealthyWoman Program. The HealthyWoman Program will then send the completed application to the local CAO.
