STEP 1 Choose the questionnaire. Just photocopy from your box of questionnaires, print from your CD-ROM, or enter the child’s information into ASQ Online to select the right questionnaire.

STEP 2 Ask the parent to complete the questionnaire. Include the introductory handout from the User’s Guide to answer their questions and get them on board with screening.

STEP 3 Parents complete the questionnaire and return to you. Parents record what the child can do. (Encourage parents to try the activities with their child if they aren’t sure of the answer.)
