It is always important for the nursing shift reports to have baseline information about the patient such as bedside documentation and patient charts. Most shift reports lack important information such as a patients response to nursing interventions as well as identified nursing care problems. The nursing staff experience a lot of stress as a result of inconsistencies in shift reports content. 9

Change of shift reports plays an important role in nursing hence those preparing them needs to be conversant with ways on how to come up with an objective, descriptive as well as future oriented reports. The reports enhances a smooth transition between shifts hence there is a need for those preparing them to incorporate factual data rather than giving personal responses which do not give adequate focus on the patient. Order to come up with best reports, nurses can make use of organizing frameworks such as Gordons (1994) framework which helps a nurse to know the needed information when preparing the report as well as helping those reading the reports to comprehend them. This ensures no information about the client is lost. 11
