This is to permit/allow my son/daughter ________________________________________________________to join the JOINT BSP/GSP CAMPORAL-ENCAMPMENT on August 28-30, 2014 to be held at

Masulog High School, Brgy. Masulog, Canlaon City, Negros Oriental, Philippines.

I have considered the benefits that my son/daughter will derive from his participation in

this activity with the understanding that every precaution is to be taken to ensure his/her safety in

camp.

I shall not held the Camp Staff/Troop Leaders, the Girl Scout of the Philippines and the Boy

Scouts of the Philippines responsible for any untoward accident that may happen beyond their

control.
