LETTER FROM THE CHAIR & EXECUTIVE DIRECTORAs we write this letter in the midst of a global pandemic, we are reminded of how critical the work of HVO truly is. Our mission is to improve the availability and quality of health care through the education, training, and professional development of the health workforce in resource-challenged countries. We are providing instruction and professional collegial support for health professionals, many of whom are now on the frontlines of delivering care during a global health crisis.

Reflecting back on 2019, it was a strong year for HVO. We had 380 volunteers who served in 425 assignments, and 89 e-volunteers provided 3,600 hours of long-distance training and mentoring. All these efforts contributed to 3,280 providers receiving training.

In 2019, HVO began a new program, the Rita Feinberg Fellowship. Named in memory of a dear board member who had devoted her life to international development, the fellowship provides support for volunteers to serve in specific longer-term assignments requested by project sites. At the beginning of 2020, the first two fellows started their assignments in Bhutan in the fields of forensic pathology and emergency medicine training.

HVO opened eight new projects in 2019. Sixteen of our host-country colleagues received scholarships to participate in training and conferences around the globe. In Laos, the nurse anesthesia program was recognized by the International Federation of Nurse Anesthetists, and the program has now had three graduates. Haiti’s physical therapy association, Société Haïtienne de Physiothérapie, was inducted as a full member in the World Confederation for Physical Therapy, a goal the association had been working toward for several years with HVO’s assistance. In Nepal, the first two fellows graduated from the gynecologic oncology fellowship program and passed the International Gynecologic Cancer Society exam. In Malawi, the planning process started for the first national speech and language therapy program. These are just a few of the significant accomplishments of a productive and rewarding 2019 for HVO.

In early 2020, our world changed with the coronavirus pandemic. HVO pivoted to increase efforts in e-volunteering so that learning could continue even without volunteers at project sites. We were fortunate to be able to build on the strong e-volunteering foundation laid in 2019. Although COVID-19 necessitated suspending our in-person work, we are moving forward with e-volunteer assignments and exploring innovative ways to provide training and curriculum development.

We continue to conduct HVO’s transforming work in a thoughtful, financially prudent manner. We recognize that our work depends on the commitment of dedicated volunteers and the continued generosity of our members, donors, and sponsors. We view our financial stewardship as a core underpinning of our mission. Your continued support for improving global health care is greatly appreciated.

Yvonne Vaucher, MD, MPH, a neonatologist at the University of California San Diego, has volunteered with HVO in Uganda every year since 1998. She first worked in the Mulago Hospital

Special Care Baby Unit (SCBU) in Kampala, where she helped train pediatric residents. The unit had limited equipment, but it did have a remarkable resource in Dr. Margaret Nakakeeto, the head of the SCBU. Dr. Nakakeeto strove tirelessly to improve the care for sick newborns and assembled a group of dedicated nurses to assist in giving newborns a better chance for survival.

Dr. Vaucher’s annual visits over 21 years have given her the perspective of time to see the many significant changes that have occurred. Obstetric and neonatal services have moved to a nearby referral hospital, and the larger, better-equipped, and better-staffed NICU is under the direction of Dr. Nakakeeto.

A neonatology fellowship program has been established at Mulago Hospital, and by 2023 Uganda will have five fellowship-trained neonatologists, in addition to five neonatal nurses. The fellowship was developed by two physicians who initially received training with Dr. Vaucher as pediatric residents. Dr. Ezekiel Mupere went on to receive his PhD in epidemiology at Case Western Reserve University and is now head of the Department of Paediatrics and Child Health at Makerere University, School of Medicine. Dr. Victoria Nakibuuka is currently Uganda’s sole fellowship-trained neonatologist, having pursued a neonatology fellowship in South Africa. She established and directs the NICU at a nonprofit hospital in Kampala.

Another physician who trained as a pediatric resident with Dr. Vaucher, Dr. Jane Aceng, requested assistance with a hospital in northern Uganda, and, together, the physicians designed a small NICU. Recognized for her health care work in that area of the country, which had suffered through years of conflict, Dr. Aceng is now Uganda’s Minister of Health.

Dr. Vaucher says it has been a joy to work with professionals who are so committed to improving newborn survival in Uganda. As an HVO volunteer, she has found that the greatest reward is watching former students become leaders in advancing health care, and she is confident that Uganda’s newborns are in good hands.

HVO’s 2019 Golden Apple Award was presented to the team of anesthesia providers at Angkor Hospital for Children in Siem Reap, Cambodia:

Bunrum Ly, Eng Hor, Kang PuthCheat, Im Peng An, Ho Sophal, and Dr. Nhoung Pheakdey. HVO’s nurse anesthesia project in Cambodia began in 2004 with a goal of improving care delivery. It evolved to implement a curriculum that trained nurses to become nurse anesthetists, and later the project began focusing on developing the nurse anesthetists’ teaching abilities. Today, this dedicated team of anesthesia providers not only provides expert care but also teaches other anesthesia providers in Cambodia and trains nurse anesthetists at the Lao Friends Hospital for Children in Luang Prabang, Laos. This team exemplifies the goal of “training the trainer” by sharing their skills nationally and regionally.

Through the Wyss Scholarship for Future Leaders in Global Health, host country colleagues can apply for funding to attend conferences and short-term

training throughout the world. Selection is based on the applicants’ demonstrated skills and leadership, as well as on references from their supervisors and HVO volunteers with whom they have worked.

Two 2019 Wyss scholars received high praise from their colleagues at international conferences.

Dr. Dinesh Pradhan, then a resident in the pediatrics department of Bhutan’s Jigme Dorji Wangchuck National Referral Hospital, attended the German Society of Tropical Pediatrics and International Child Health conference. There he and HVO volunteer Christoph Funk, MUDr, presented their study, “Acute Encephalopathy in Children Admitted to PICU of the National Referral Hospital in Bhutan.” Their presentation was received with great interest and excitement. In a unanimous decision, the scientific panel honored the two physicians with the Helmut Wolf Award for Best Presentation.

Assuman Nuhu, a physical therapist faculty member at the University of Rwanda, traveled to Geneva to attend the World Confederation for Physical Therapy. He presented his research titled “Sports and Sports Injuries” and was honored with the Best Platform Presentation Award from the Africa Region.

As their peers have so clearly recognized, these talented health care professionals represent the future of global health care.

INVESTING IN HEALTH PROFESSIONALSAdvancing the Physiotherapy Profession in Malawi

In early 2018, a challenge was presented to Enock Chisati, PhD Fellow of the Consortium for Advanced Research Training in Africa

(CARTA) and Head of the Physiotherapy Programme at the University of Malawi. Would he provide training for diploma-prepared physiotherapists to help them upgrade their skills and meet the accepted international standards for a bachelor’s degree, as recognized by the World Confederation for Physical Therapy?

A chance meeting with Leslie Glickman, PT, PhD, an HVO member working in Malawi on a Fulbright assignment, provided an unexpected opportunity. Dr. Glickman’s experience with developing and implementing a transitional and post-professional program at the University of Maryland was the expertise needed.

Over the next two years, the two educators prepared a curricular proposal and navigated the complicated process of approvals from the university—all while working together online and contending with time-zone differences and connectivity glitches. The new program has now been merged with the existing bachelor’s program, and formal approval is pending. Dr. Glickman and Mr. Chisati say that the program meets the needs of working professionals, can be delivered on a part-time basis, and is available almost entirely via the internet.

International collaborations, particularly those related to education, require patience, persistence, dedication, and a commitment to producing legitimate outcomes. Challenges clearly create opportunities, and Mr. Chisati and Dr. Glickman are excited to have developed a new, innovative way for diploma-trained physiotherapists to advance their careers and strengthen the physical therapy profession in Malawi.

ADVANCING GLOBAL HEALTHThe Keystone to Improving Health Care

After a fulfilling career caring for newborns and teaching young physicians, Winston Koo, MBBS, decided it was time to explore other parts of the world and share his skills in resource-challenged countries. Through

an internet search, he found HVO and was drawn to its emphasis on education and training—what he calls “the keystone” to improving health care.

While others might opt for one assignment a year, Dr. Koo decided he would volunteer in as many sites as possible within a year. Then, he would focus on a particular site where his skills would be most helpful. He managed to volunteer at four sites over five months—Nepal, St. Lucia, Laos, and Bhutan—but then his plan to travel to Cambodia was sidelined by the COVID-19 pandemic.

Dr. Koo loves working with babies and observes that parents everywhere want the same thing—healthy, happy children. His HVO assignments provided him with several insights about global health care, which echo those of many other HVO volunteers.

Dr. Koo says he learned as much as he taught. For example, he saw diseases that he had previously only read about, such as neonatal tetanus. Although Dr. Koo saw some nutritional deficiencies at the sites where he volunteered, the babies were generally healthy. The deficiencies sometimes were related to cultural issues—for example, mothers chewing betel nut or following severely restricted dietary habits. In those situations, which might require long-term patient education and public health campaigns, he says it was best to temper his expectations about what could be accomplished in a short visit.

The premature infants Dr. Koo saw were larger and more mature than those he cared for in the US. As a result, they were more amenable to treatment. By providing a moderate amount of technical support that can be obtained in a resource-limited setting, he says, it is possible for these infants to survive and live a normal life.

Dr. Koo was surprised by a tendency to focus too much on treating possible infections. This practice, he says, could miss noninfectious causes of a baby’s symptoms. Furthermore, excessive antibiotic therapy risks development of antibiotic resistance. “Babies don’t lie to you,” he notes. “You pay attention to them, and their symptoms often will help you determine what needs to be done.” As long as the principles of neonatal care are understood and practiced, Dr. Koo states, it is not necessary to have all the equipment commonly found in the US. In fact, he says, much of the equipment he saw was of the generation he trained with years ago.

For anyone considering an HVO volunteer assignment, Dr. Koo advises to keep an open mind, learn from colleagues at the site, and be willing to share best practices. He looks forward to the world reopening so he can continue offering his skills and knowledge to help give more infants a strong chance for a healthy life.

TRANSFORMING LIVESA Legacy of Education

Dr. David Beaton Lovejoy Jr. was a dynamic and dedicated orthopaedic surgeon. In addition to serving many patients in his orthopaedics practices in large and small towns in New

England, he generously gave his time and talent to his local community. For many years, however, Dave was looking to make an impact farther away by working with HVO.

Growing up, Dave played football and hockey and enjoyed the wilderness, leading canoe trips in Wisconsin and Canada. He graduated with honors in history from Yale and received his medical degree from the University of Rochester School of Medicine. Dr. Lovejoy then completed both a surgery residency at Tufts Medical Center in Boston and a second residency in orthopaedics at the Harvard Orthopaedic Combined Residency Program. An avid reader and skilled writer, he wrote an award-winning article about the development of hospitals in 19th-century America.

Dr. Lovejoy was a founding partner of an orthopaedics practice in the Boston area, worked at a hospital as a fracture surgeon, served as a clinical instructor in orthopaedics at Harvard Medical School, established and volunteered at an amputation clinic, and even made house calls. A devoted father of three daughters and two sons, he was deeply involved with his community, including serving as the chairman of the Board of Selectmen in Bethlehem, New Hampshire.

Dr. Lovejoy and his youngest daughter served in a clinic in Senegal, fulfilling his longtime desire to share his skills and knowledge with colleagues abroad. Following his unexpected death in 2013, his family decided to celebrate his life and his dream by sharing his excitement about practicing and teaching medicine in resource-challenged countries through a special fund at Health Volunteers Overseas.

TRANSFORMING LIVES

The David B. Lovejoy Memorial Fund was established in 2013 to give orthopaedic residents hands-on experience at HVO overseas project sites. To date, 19 residents have benefited from the opportunity, including four in 2019. Placed in areas of the world with scarce resources, the HVO physicians share their knowledge of disease and trauma conditions with local medical counterparts, simultaneously improving their skills as they learn from local colleagues. The residents return home not only with a deeper understanding of life and orthopaedics in resource-challenged countries, but also with a more expansive medical skill set and techniques that will serve them well in their careers.

HVO is grateful to Dr. Lovejoy and his family for their commitment to advancing global health by educating the physicians of the future.
