The philosophy of the Driving to Make a Difference in Diabetes is that each person is unique and valuable. A mobile clinic that

provides high quality care to those with diabetes is a valuable asset to the community and will result in fewer readmissions to hospitals in addition to fewer Emergency Room visits. Providing

care to this group of undeserved patients will ensure stewardship of other valuable health resources the community has to offer. The Driving to Make a Difference mobile clinic aims

to fill an essential community health need.

1) Ensure diabetic services are available to diabetic patients in the community. To provide weekly group meetings at the clinic that focus on

disease education and healthy lifestyles to help strengthen the bond between staff and clients and to allow for clients to support one other within 6 months of starting.

To reduce complications with diabetes or people who may have had uncontrolled diabetes by educating the patients on diabetes and providing eye exams, foot exams, and diabetic wound care

within a year of starting.

2) Provide a mobile environment for patients to seek treatment for their diabetes outside of the acute care setting. To be mobile and travel to different areas in the community to

serve diabetics, people who may be unsure if they are diabetic, help the uninsured, and people who may not be able to access a diabetic clinic noting a reduction in diabetic hospitalizations (as primary diagnosis) and an increase in knowledge on diabetes in the

community within a year of starting. To reduce the number of hospitalizations (with primary diagnosis

of diabetes), in the area, by educating diabetic clients on “how to help manage their diabetes” within 6months of starting Driving to Make

a Difference in Diabetes.

3) Ensure current information and teaching are available to diabetic patients in the community. To educate the people in the area and diabetics people who may be

at risk (or pre-diabetic), what diabetes is, how it can affect their body, and signs and symptoms to look for by their third visit. To reduce complications with diabetes or people who may have had uncontrolled diabetes by educating the patients on diabetes and providing eye exams, foot exams, and diabetic wound care within a year of starting.

To reduce hyperglycemia in the patients, in the area, by educating on how to manage diabetes, increasing knowledge on medications, and increasing access to medications within 6 months of starting

The physician in the medical director of DMDD. The Chief Medical Officer (CMO) has the overall responsibility for providing leadership, direction, and administration of medical management. In addition this person works in the integration, coordination, and improvement of clinical care for the diabetic population. The director of the organization is responsible for reporting to ABC Hospital.
