Observe that there is calcium both inside AND outside the artery per IVUS. This differentiation is not evident with an angiogram. Endovascular therapies will only affect intraluminal calcium or plaque – YET many decisions to treat are based on angio only – and angio greatly overestimates the amount of intraluminal calcium.
