Gastric peritoneal carcinomatosis
Major problem of the disease evolution
More than 50% of potentially curable gastric cancer died of peritoneal
recurrence
60% of all causes of gastric cancer deaths is from peritoneal carcinomatosis
Gastric carcinomatosis Natural HistoryPalliative treatment

Conclusions sur le traitement curatif

Conclusions Comment améliorer la
sélection?
• Coelioscopie précoce+++++
Evite laparotomies inutiles (ouverture-fermeture)P

préthérapeutique du cancer gastrique
•Ambulatoire

Criteria of inclusion
Gastric carcinoma with moderate PC (PCI ≤ 12), synchronous and resectable
Modalities of treatment Total gastrectomy with D1.5-D2
lymphadenectomy R0-R1 resection of the PC Then randomization between two scheldules:

• Radiochimiothérapie Mac Donald (USA)• Chimiothérapie adjuvante (Japon)
Contexte

Take Home Messages
