The network providers listed in this Provider Directory have agreed to provide you with your health care, vision, and dental benefits. You may go to any of our network providers listed in this Provider Directory. All network providers may not be listed in this Directory because contract negotiations with hospitals, doctors and other care providers are still underway. A providers listing does not guarantee the provider is still in the network and may have been added or removed from the list after the Directory was printed. A providers listing does not guarantee the provider is at the same location as listed in the Directory. Providers may have been added or removed from the list after the Directory was printed. Information.in.the.HPSM.CareAdvantage.Provider.DirectoryThe CareAdvantage Provider Directory lists the Primary Care Physicians, clinics, hospitals, pharmacies, and other health care providers and facilities available to you through the HPSM CareAdvantage Plan. The Directory also has the doctors and other providers addresses, telephone numbers, languages spoken and the hospitals they work with. HPSM updates the directory on a regular basis and shows which doctors are not accepting new patients. You can write or call the CareAdvantage Unit to request a Provider Directory or ask for specific information about a doctor. You can also visit our website at www.hpsm.org for an up-to-date Provider Directory.

How.to.find.informationThe Table of Contents at the front of this Directory will show you where each provider specialty can be found in this booklet. The Index at the back of the Directory will show you where you can find a particular doctor, and also shows you where you can find provider specialties and pharmacies.

Always.Carry.Your.CareAdvantage.ID.CardPlease carry your CareAdvantage membership card with you at all times. You will need to show this card when you get covered services. You will also need it to get your prescriptions at the pharmacy. If your membership card is damaged, lost, or stolen, call the CareAdvantage Unit right away and we will send you a new card.

While you are a CareAdvantage Plan member and using plan services, you must use your plan membership card instead of your red, white, and blue Medicare card to get covered services. If you do not use the CareAdvantage card, your provider may not know you have coverage and you may have to pay for services. If this happens, you can ask us to reimburse you. To learn how to submit a claim, please refer to the claim process described later.

CareAdvantage..A.Health.Plan.for.People.with.Medicare.and.Medi-CalCareAdvantage, a Medicare Advantage plan, is offered by the Health Plan of San Mateo for San Mateo County residents. CareAdvantage includes your Medicare medical and prescription drug benefits. If you have Medi-Cal, you will also receive your Medi-Cal benefits through CareAdvantage. CareAdvantage is not a Medigap or supplemental Medicare insurance policy.

With CareAdvantage you will be getting most or all of your health services from the doctors, hospitals, and other health providers that are part of the CareAdvantage provider network. Since these doctors, hospitals, and other providers are the ones we have contracted with to provide your care, they are the ones you must use (except in special situations such as emergencies). Contact the CareAdvantage Unit for more details.

Choosing.Your.Primary.Care.Physician.(PCP)A Primary Care Physician (PCP) is the plan doctor you choose who will be the main person you contact about your health care needs. Adults generally choose an internist, family practitioner or general practitioner. Children usually have a pediatrician or family practitioner as their PCP. You will get your preventive care, regular check-ups and the basic care for illness that you may need from your PCP. Your PCP will also coordinate the rest of the covered services you get as a plan member.

Your.PCP.and.Youa.Winning.TeamThe main goal of our services at HPSM is help you to get and stay healthy. Working together with your PCP is a key way to achieve this goal. Once you choose a PCP, it is best to stay with the same doctor, so she or he can get to know you and your health care needs. If you change doctors often, your health care may not be as good as it could be. This is because it can take some time for a PCP to become familiar with your health history, medicines, and past illnesses. Once your PCP learns this information about you, and you have begun working with your PCP to improve your health, it can be a setback to move to a new physician who has to start again in getting to know you and your health care needs. Sometimes it is necessary to change PCPs. However, if at all possible, it is best to choose and then keep your PCP so you can both focus on a healthier you.

The PCP you choose will provide, authorize and coordinate your health care, except for emergency and out-of-area urgent care services. Your PCP will refer you to specialists when needed. If there is a particular CareAdvantage specialist or hospital that you want to use, you should check first to be sure your PCP makes referrals to that specialist or uses that hospital. The name and office telephone number of your PCP is printed on your membership card. If your PCP is not printed on your membership card, this means you have not yet selected your personal PCP. You then should call the CareAdvantage Unit to choose a PCP.

If.you.need.to.change.your.PCPYou can change your PCP if you feel it is necessary. A PCP change will be effective the first day of the following month as long as we receive your request by the 22nd day of the month. To change your PCP, call the CareAdvantage Unit at 1-866-880-0606 (toll free) or 650-616-2174. The CareAdvantage

call center hours are Monday through Sunday from 8:00 a.m. to 8:00 p.m. Members with hearing or speech impairments can use the California Relay Service at 1-800-735-2929 (TTY) or dial 7-1-1. When you call, be sure to tell the CareAdvantage Navigator if you are seeing specialists or getting other covered services that needed your PCPs approval (such as home health services and durable medical equipment). A CareAdvantage Navigator will help make sure that you can continue with the specialty care and other services you have been getting when you change to a new PCP.

Your PCP may request that you select a new PCP if you miss many appointments or do not follow your PCPs medical advice.

What.to.do.if.you.have.a.medical.emergency.or.urgent.need.for.careIn an emergency, you should get care immediately. You do not have to contact your PCP or get permission in an emergency. You can dial 911 for immediate help by phone, or go directly to the nearest emergency room, hospital, or urgent care center.

What.to.do.if.it.is.not.a.medical.emergencyIf you need to talk with your PCP or get medical care when the PCPs office is closed, and it is not a medical emergency, call your PCPs office. Your PCP or a doctor on call will always be available to tell you how to handle the problem at home, or if you should go to an urgent care center or a hospital emergency room. If you are hearing impaired you can call the California Relay Services at 1-800-735-2929 (TTY) or dial 7-1-1. For California Relay Services in Spanish call 1-800-835-3000.

Getting.Care.from.Specialists.-.When.You.Should.Get.a.PCP.ReferralWhen your PCP recommends that you receive care or treatment from a doctor who is an expert in a specific disease, condition, or part of the body, he or she will refer you to a specialist. Examples of specialists include cardiologists (experts in heart disease), or orthopedists (experts in bone, joint, or muscle conditions). If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from non-network providers. Because your PCP is there to take care of your overall health, he/she should be aware of all of your health concerns. That is why we strongly recommend that you talk to your PCP first before you go to a specialist.

There.are.some.services.that.you.will.always.be.able.to.get.without.a.written.referral:You can get the following services without a written referral or approval in advance from your PCP.

Routine womens health care, which includes breast exams, mammograms (x-rays of the breast), pap tests, and pelvic exams.

Flu shots and pneumonia vaccinations. Podiatry, chiropractic, vision, and hearing services. Emergency services, whether you get these services from plan providers or non-plan

providers (see CareAdvantage Evidence of Coverage for more information). Urgently needed care that you get from non-plan providers when you are temporarily outside

the plans service area. Also, urgently needed care that you get from non-plan providers when you are in the service area but, because of unusual or extraordinary

circumstances, the plan providers are temporarily unavailable or inaccessible. (See CareAdvantage Evidence of Coverage for more information about urgently needed care). The CareAdvantage service area is San Mateo County.

Renal dialysis (kidney) services that you get when you are temporarily outside the plans service area. If possible, please let us know before you leave the service area where you are going to be so we can help arrange for you to have maintenance dialysis while outside the service area. The CareAdvantage service area is San Mateo County.

x Introduction

Notice.of.Future.Policy.Changes.Requiring.Approval.of.ReferralsAt some time in the future, HPSM may change its policy on whether or not HPSM approval is needed for PCP referrals to see specialists or non-contracted providers. If we do, we will give you advance notice of the effective date of any change to the referral process. After the effective date of the change, you may be required to have HPSM approve a written referral from your PCP before you can see a specialist, or non-contracted provider. If you do not have an approved written referral before you obtain services, you may have to pay for these services yourself.

Hospital.careServices covered are limited to plan hospitals unless services are for emergency or urgent medical care. Your PCP may only have admitting privileges at certain hospitals. Be sure to check with your PCP to find out which hospital he/she has admitting privileges.

Your.prescriptions.must.be.from.plan.doctors.and.must.be.filled.at.a.plan.pharmacy.In nearly all cases, your prescriptions are covered only if they are written or ordered by a plan doctor or other plan provider. In addition, you must fill your prescriptions at plan pharmacies. There is an exception for medical emergencies and urgently needed care. If it is a medical emergency or urgently needed care, we cover prescriptions you get from doctors who are not plan providers and prescriptions that are filled at non-plan pharmacies.

Filling.prescriptions.at.a.plan.pharmacyTo see the pharmacies you can use, please check the CareAdvantage Provider Directory or call a CareAdvantage Navigator. We call the CareAdvantage pharmacies in the Directory our plan pharmacies because we have made arrangements with them to handle prescriptions for members of CareAdvantage. We have retail, home-infusion, and long-term care pharmacies in our network.

To fill your prescription at a plan pharmacy, you must show your CareAdvantage Member ID card. If you do not have your ID card with you when you fill your prescription, you may have to pay the full cost of the prescription (rather than paying just your co-payment). If this happens, you can ask us to reimburse you for your share of the cost by submitting a claim to us. To learn how to submit a claim, please refer to the claims process described later.

If you are a new CareAdvantage member and need to have an existing prescription refilled, remember that the prescription must be written by a plan provider or it will not be covered (even if you fill it at a plan pharmacy). You should consult with your current doctor to see if he or she wants you to continue on the medication, and you must get a new prescription from your current doctor.

Filling.prescriptions.at.a.non-network.pharmacyThe following are a few exceptions when we will pay for a prescription filled at a pharmacy outside of our network.

Getting.coverage.when.you.travel.or.are.away.from.the.plans.service.areaIf you take a prescription drug on a regular basis and you are going on a trip, be sure to check your supply of the drug before you leave. When possible, take along all the medication you will need.

If you are traveling within the United States and territories and become ill, or you lose or run out of your prescription drugs, we will cover prescriptions that are filled at an out-of-network pharmacy. In this situation, you will have to pay the full cost (rather than paying just your co-payment) when you fill your prescription. You can ask us to reimburse you for your share of the cost by submitting a claim. To learn how to submit a claim, please refer to the paper claims process described later.

You can also call a CareAdvantage Navigator to find out if there is a network pharmacy in the area where you are traveling. Look for a chain pharmacy that is in the CareAdvantage network. If there are no network pharmacies in the area, a CareAdvantage Navigator may be able to make arrangements for you to get your prescriptions from an out-of-network pharmacy.

When.Outside.the.United.StatesExcept in limited circumstances we cannot pay for items or services furnished outside the United States and its territories. The law permits payment to a foreign hospital for emergency patient services only if:

You were present in the United States at the time the emergency occurred that necessitated the inpatient hospital service, and

The hospital outside the United States that provided the emergency inpatient services was closer to where the emergency arose, or was substantially more accessible, than the nearest adequately equipped hospital within the United States.

Getting.a.prescription.because.of.a.medical.emergency.or.urgent.careWe will cover prescriptions that are filled at an out-of-network pharmacy if the prescriptions are related to care for a medical emergency or urgent care. In this situation, you will have to pay the full cost (rather than paying just your co-payment) when you fill your prescription. You can ask us to reimburse you for your share of the cost by submitting a claim. To learn how to submit a claim, please refer to the paper claims process described later.

Other.situations.when.you.can.get.your.prescription.covered.if.you.go.to.an.out-of.network.pharmacy.We will cover your prescription at an out-of-network pharmacy if at least one of the following applies:

If you are unable to obtain a covered drug in a timely manner within our service area because there is no network pharmacy within a reasonable driving distance that provides 24 hour service.

If you are trying to fill a prescription drug that is not regularly stocked at an accessible network retail;

If you are getting a vaccine that is medically necessary but not covered by Medicare Part B and some covered drugs that are administered in your doctors office.

Before you fill your prescription in these situations, call a CareAdvantage Navigator to see if there is a network pharmacy in your area where you can fill your prescription. If you do go to an out of network pharmacy for the reasons listed above, you will have to pay the full cost (rather than paying just your co-payment) when you fill your prescription. You can ask us to reimburse you the amount you paid by submitting a claim form. To learn how to submit a claim, please refer to the claims process described next.

What.should.you.do.if.you.have.bills.from.non-plan.providers.that.you.think.we.should.pay?

Submit.a.claim.to.HPSMWe will repay you for health care services that you get from non-plan providers who ask you to pay in certain situations. If you have to pay for services you get in the situations described below, please contact the CareAdvantage Unit by phone or in writing.

Care for a medical emergency, urgently needed care, or renal dialysis that you get when you are outside the plans service area

Care that has been approved in advance by HPSM Services that we denied but that were overturned in an appeal. A bill from an out-of-network pharmacy

It is best to ask a non-plan provider to bill HPSM first, but if you have already paid for the covered services we will repay you the amount you paid. If you receive a bill for the services, you can send the bill directly to us for payment. We will pay your doctor or pharmacy for our share of the bill and will let you know what, if anything, you must pay. You will not have to pay a non-plan provider any more than what you would have had to pay a plan provider for those services.
