We’ve been talking & listening to people …

• To help shape the implementation of our care model we’ve proactively been talking to:– People from all walks of life via on-street engagement– Mums and Dads at toddler groups– Young people at schools and colleges– Older isolated people through Age UK and housing

associations– Black and minority ethnic groups through Pathways CiC– People with mental health conditions through Cheshire

and Wirral Partnership

We’ve been talking about …

• Where to go to for advice and support• What additional help they feel they need• The way in which they use GP services• How they feel we could join up care• What hospital services they’ve used and where• Expectations around accessing hospital care• Personal experiences and ideas

So to date …

• 3 Stakeholder Panel events with participation from over 150 individuals

• Over 700 surveys completed• On street engagement in 15 locations• Web site visits 30,023 and 811 visitors• Twitter reach – 290 followers, 23,450 accounts and

74,677 impressions• Facebook – 59 fans, 10,763 views• YouTube – 1,597 views• Press clippings – 25, circ 331,876, value £58,600
