management is a system design to identify people with chronic illness and the use health intervention to improve

self-care and quality of life for people with chronic illness. Disease management program attempt to do this by

preventing or minimizing the effects of disease, or chronic condition through integrative carehaping theirunderstanding of medical should follow several criteria. The diseases should be associated with high level of visits

to physician or emergency department, hospitalizations and is very expensive to manage based on other utilization

criteria. There is a high percentage of preventable complication associated with disease. The disease is highly

prevalent within the target population. A disease management program can show an effect within a short time frame

such as one to three years. The chronic condition can be managed in the out patient setting using low technology and

non-surgical interventions.condition and 16. Catastrophic care focuses on catastrophic injuries and rare illnesses.

Catastrophic cases are defined as those high-severity cases that require specialized complex management, usually

involving multiple providers and multiple sites of care.17. Traditional candidate for disease management meetscriteria such as high prevalence or specific symptoms and disease outcomes that are modified within a short time

period. In contrast, catastrophic conditions are associated with a high rate of variability among current practicepatterns, treatment may be characterized by multiple referrals to multiple providers and outcomes may take a longer

are five common characteristics of why catastrophic illness can benefit from specialized catastrophic care

management programs. First is the Immediate referral to a team of care manager with specialized expertise in both

the individual condition and the catastrophic care management process that helps initiate and coordinate necessary

care and support services. Next is a Care plan that develops guidelines for appropriate care for different categories of

cases. The third characteristics is the medical management by professionals with expertise in catastrophic. Another

is the individualized management, which recognizes that every patient is different. The last characteristic is thepatient and program satisfaction. Patients and family are satisfied with the care and services they receive as well as
