This list contains essential product information needed for authorization of enteral nutrition products. The list displays "contracted" metabolic, standard and diabetic enteral nutrition products in yellow highlights. Each type of contracted product is identified with either an "M" for metabolic, "S" for standard or "D" for diabetic and a future category of "SI" for specialty infant (not highlighted ) in the 3rd column. The "non-contracted" enteral nutrition products which include all other types of enterals (NOT metabolic, standard and diabetic enterals) are shown without yellow highlights; these products CAN BE authorized as usual. All listed product numbers have been verified as active in CMS Net and are Medi-Cal benefits. This list is NOT inclusive of every available product, but it includes all contracted products and the vast majority of routinely prescribed non-contracted enteral nutrition products. Non-restricted products such as, parenteral nutrition products are not included here. Purpose: The following is a list of CCS "restricted" and Medi-Cal "contracted" enteral nutrition products that may be authorized by CCS. This list contains essential product information

needed for authorization of enteral nutrition products. The list displays "contracted" metabolic, standard and diabetic enteral nutrition products in yellow highlights. Each type of

contracted product is identified with either an "M" for metabolic, "S" for standard or "D" for diabetic and a future category of "SI" for specialty infant (not highlighted ) in the 3rd column.

The "non-contracted" enteral nutrition products which include all other types of enterals (NOT metabolic, standard and diabetic enterals) are shown without yellow highlights; these

products CAN BE authorized as usual. All listed product numbers have been verified as active in CMS Net and are Medi-Cal benefits. This list is NOT inclusive of every available

product, but it includes all contracted products and the vast majority of routinely prescribed non-contracted enteral nutrition products. Non-restricted products such as, parenteral nutrition
