Center Star ACT
What is the Center Star ACT program?Our Center Star Assertive Community Treatment (ACT) program offers 24-hour community-based treatment for individuals with a criminal justice background who have been diagnosed with a severe and persistent mental illness.
All clients of Center Star ACT are eligible to receive medication management and monitoring, therapy, drug and alcohol counseling, case management, vocational rehabilitation, peer counseling and support, and housing services.

How Can Center Star ACT Help? Our Center Star ACT program provides community-based treatment, rehabilitation, and support around-the-clock, 365 days a year. Our culturally sensitive, gender responsive services promote mental wellness and independent living, and are designed to meet the individual needs of each client we serve.
Who Do We Serve?Center Star ACT serves individuals who need more intensive services than traditional outpatient mental health services can provide and who meet the three following requirements:
• Theyarebetweentheagesof25to59
• Theyhavebeendiagnosedwitha serious mental illness
• Theyhavebeenincarceratedsome time during the last year
How Do We Get Referrals?Referrals are made through various resources, including hospitals, jails and outside agencies.
