Safety priorities for 201401/04/2014 11
Fatal accidents 2008-2013
zero fatal accidents in 2013
8 accidents in the 6 years 2008-2013
24 in the previous 6 years
27.4 in the average 6 year period 1978-2007
01/04/2014 12
How did 24 fatal from 2002-2007 become 8 fatal from 2008-2013?
4 fewer winch
3 fewer collision; Flarm? Fortuitous?
3 fewer hit mountain (abroad)
2 fewer medical (in tugs)
1 from each of technical (K7 wing), undershoot (photographer), suicide, TMG (winch cable)
01/04/2014 13
Potential fatal accidents in 2013
Potential fatal accidents
winch launch with disconnected elevator
winch cartwheel, fell right way up
final turn at 200ft, spin
field landing, final turn, spin
inadvertent spin at 900ft on trial lesson
tug upset, tug flew away from 40ft
TMG touch and go, picked up winch cable
hit tree then hill on South Downs
No actual fatal accidents in 2013 but 8 accidents could easily have been fatal:
01/04/2014 14
Fatal accident prognosis
fewer winch should continue
no collisions in 2013; fewer collisions maycontinue (Flarm? Probably)
there will be future fatalities unless we can stop having potential fatal accidents
01/04/2014 15
To avoid future fatal accidents
CFIs: please ensure your members apply known good practice to:
– rig correctly
– avoid inadvertent spins
– avoid tug upsets
– avoid collisions (Have you got Flarm?)
– avoid winch stall/spin
– combat winch cartwheel
01/04/2014 16
Serious injury accidents in 2013
field landing, spin in final turn
undershot field
airbrakes opened on launch, pilot unaware
01/04/2014 17
Substantial damage in 2013
17 more than in 2012
– twice the usual landing total
– many from towing behind a car, or wind

airbrakes opened on launch, field landing (P1 unaware, ‘slow learner’ aged 68, 185 launches to solo in 2006, BI rating only in 2013; should never have become a BI?)
aerotow, wing hit control vehicle (lined up behind and close to the vehicle)
trial lesson, EFATO, glider landed safely, tug pilot flew to a controlled crash

Trial lesson accidents/incidents 2013 minor or no damage
Puchacz, 40kt in rough thermal at 900ft, inadvertent spin (also 90°cross wind gusting to 30kt; P1 should not have taken off!)5/8 cloud at 1700ft, unable to return to airfield (should have stayed on the ground?)Tug pilot concerned about fuel, asked P1 for quick release, P1 pulled trim instead of release, tug upset (tug pilot should have refuelled!)
Heavy landing
P2 opened canopy in flight (the family pressed a reluctant P2 into flying; P1 should have said no)P2 broke canopy disembarkingHangar flight, airbrake hit bankDuring pre-flight preparation the front rudder adjustment handle detached

Safety Messages
we have been lucky to avoid any fatal accidents in 2013
we have the right safety messages but, except for avoidance of winch stall/spin, they are not always being heeded
we need to reinforce the following messages
01/04/2014 34
Reinforcement-main messagescollision Your life depends on your lookoutrigging Rig without interruption or distraction; one person must be
responsible Get a second person to check connectionsstall/spin Fly the glider is ALWAYS the first priority, whatever the
workload and distractionswinch Reliable kit; correct set-up (Stop the Drop); release
BEFORE the dropping wing hits the ground; continue to follow the BGA advice to avoid a stall and spin during rotation or after a launch failure
aerotow Avoid tug upsets
trial lessons
Ensure trial lessons are as risk-free as possible!! That was not the case in 2013
