Com plete the right form and send it to DVLA with your old driving licence and any supportingdocum ents.

Photocard driving licence

For a car or m otorbike licence, com plete form D1 ‘Application for a driving licence’, available from theDVLA form ordering service (https://www.dvla.gov.uk/dvla/onlineservices/order_forms.aspx) or m ost PostOff ices.

For a lorry or bus licence, com plete form D2 ‘Application for a lorry/bus licence’, available from theDVLA form ordering service. (https://www.dvla.gov.uk/dvla/onlineservices/order_forms.aspx)

All drivers and riders will also need to:

include original docum ents conf irm ing your new nam e (https://www.gov.uk/driving-licence-

application-identity-documents)

include your photocard driving licence and paper counterpart

You don’t need to send a new photo.

include your driving licence and a passport-type photograph

If you can’t send in your old driving licence

You m ust declare on the form if your old licence has been lost, stolen or destroyed.

Change your title

Use the processes above to change your title (eg Ms to Dr).

You don’t need to send evidence for the change unless it’s for an hereditary title.

When to expect your licence

DVLA aim s to deliver your new driving licence to you within 3 weeks of getting your application.

It’ll take longer if they have to check your health or personal details.

Allow at least 3 weeks for your licence to arrive before contacting DVLA.

If you’re disqualified from driving

You can still tell DVLA if you change your nam e while disqualif ied.

Write to DVLA, Swansea, SA99 1AB giving details of your old and new nam e, your driving licencenum ber (if known) and date of birth.

Your driver record will be updated and you’ll receive a D27 driving licence renewal application 56 daysbefore your disqualif ication ends.
