• Systematic and structured reviews of control systems and electrical power systems during design or modification projects should be part of any safety management process as such systems are often safety critical on offshore oil and gas platforms
• New or modification projects are usually designed onshore while pratical details on installation are often left for offshore personnel based on their hands-on experience
• If computer based safety systems are reviewed independently, potential hazards related to interfaces between various systems may not be detected. Interfaces between detailed component level design of computer based systems and overall production and safety systems should be assessed in order to identify and mitigate potential single points of failure
• A systematic method of reviewing the design and installation of computer based and electrical systems is advisable to reduce the risk

• Computer Hazard and Operability study
• Structured study of control and safety systems to assess and minimize the effect of failures of its subsystems impacting the plant or affecting the ability of an operator to take corrective action
• Based on HAZOP methodology, but specialized for control and safety systems, including appropriate guidewords and parameters (e.g. no signal, out of range signal, no power, no communication, I/O card failure, etc.)
• Covers the entire safety instrumented loops, from the field instrumentation to the relay’s, PLC’s (DCS/SCADA, PSD/ESD, F&G), IO cards, circuit breakers, actuators, local control panels, power supply, etc.
• Can be performed for new designs or modification projects, at different stages: FEED, detailed design, construction and commissioning

• Electrical Hazard and Operability study
• Structured study of electrical power systems to assess and minimize potential hazards presented by incapability or failure of electrical apparatus
• Based on HAZOP methodology, but specialized for electrical systems, including appropriate guidewords and parameters (e.g. power surges, 24VDC supply failure, UPS availability, etc.)
• Covers the power generation, transformation, transmission and distribution, load shedding philosophy, UPS, etc.
• Can be performed in new designs or modification projects, at different stages: FEED, detailed design, construction and commissioning

• Thourough C&E review, alarm management system, inhibit and reset philosophy, HMI interfaces
• During preparation encourage the client and involved parties to contribute with images as an additional documentation basis for the review, on top of the usual C&E’s, P&ID’s, connection diagrams, single line diagrams, equipment data sheets and schematics, wiring diagrams and layouts
• Workshop facilitation enabling dialogue between the involved parties rather than just ticking off known hazards
• Engaging end users in design decisions
• Additional focus on contingencies in case of failures
