TECHNICAL COMPONENT OF BID (FIRST ENVELOPE)

I. ELIGIBILITY REQUIREMENTS

Class "A" LEGAL DOCUMENTS

i. Platinum PhilGEPS Certificate of Registration and Membership in accordance with

Section 8.5.2 of the IRR;

ii. Statement of all ongoing government and private contracts, including contracts awarded but not yet started, if any, whether similar or not similar in nature and complexity to the contract to be bid, within the relevant period as provided in the Bidding Documents. The Statement shall include all information required in the Philippine Bidding Documents (Sec. 12.1.(ii));

iii.Statement of the bidder’s Single Largest Completed Contract (SLCC) similar to the

contract to be bid, in accordance with ITB Clause 5.4, within the relevant period as provided

in the Bidding Documents in the case of Goods except under conditions provided for in

Sections 23.4.1.3 and 23.4.2.4 of the IRR;

iv.Computation of Net Financial Contracting Capacity (NFCC) in accordance with ITB

Clause 5.5 supported by latest Audited Financial Statements submitted to BIR or a

committed Line of Credit from a Universal or Commercial Bank;

Class “B” Documents

Valid joint venture agreement (JVA), in case of joint venture

Technical Documents

i. Bid Security in accordance with ITB Clause 18;

ii. Comformity with technical specifications as enumerated and specified in Sections VI and

VII of the Bidding Documents;

b. Technical Specifications under Section VII

iii. Omnibus Sworn Statement in accordance with Section 25.3 of the IRR of RA9184 and

using the prescribed form in Section VIII of the Bidding Forms;

1. Duly signed Financial Bid Form which includes bid prices and the applicable Price Schedules, in accordance with ITB Clauses 15.1 and 15.4

2. Duly accomplished form - Goods Offered from within the Philippines in accordance with

ITB Clause 15.4
