Unlock your incentives To earn incentives, you need to be covered under the Cigna Medical Plan and complete a health assessment and biometric screening.

Build a strong foundation Achieve or work toward a healthy blood pressure and BMI. (Up to $266)

Choose your wellness path Earn incentives and meet your personal health goals with various Cigna activities and health coaching. (Up to $500)

To be eligible for any incentives, you and your covered spouse/partner must complete two activities:

1. Take the online health assessment.Complete the confidential questionnaire at myCigna > My Health Tab > Take Health Assessment to get a personalized assessment of your health each year.

2. Complete and submit your numbers. Provide validated biometric results for your blood pressure, cholesterol, blood sugar, and Body Mass Index (BMI) every other year.

Healthy weight and blood pressure (Up to $266)To earn this incentive, you need to have:

• A Body Mass Index (BMI) under 30

• Blood pressure of 139/89 or less

Can’t meet those numbers? You can also earn the incentive if you:

• Work with a health coach to improve your numbers. Call 1.888.992.4462 to get started.

Healthy habitsUp to $400, $100 each

Participate in a qualified exercise or weight management program and submit a Healthy Habits Form.

Centers of ExcellenceUp to $500, $100 each

Use a Center of Excellence for back, joint, cardiac, bariatric, or inpatient transplant surgery.

CIGNA ACTIVITIES Take advantage of health-boosting activities, programs, and events offered through Cigna.

Health seminarsUp to $250, $25 each

Watch specific seminars on Cigna University. Cigna’s Mindfulness seminars are available now through December. Visit the Health Campaigns section of Your Cigna Life for the complete list of eligible seminars. Watch your email for details.

Healthy Life Challenges Up to $450, $150 each

The next Healthy Life Challenge will be the Fitness Challenge. Track your physical activity to compete with employees around the world. The challenge will be held July 18 through August 29. Watch for registration information from July 11 through 22.

Healthy Life pilot programUp to $300, $100 each

Participate in pilot programs to test new products and services. Watch for information in Your Cigna Life or via email communication. The Healthy Weight, Healthy You weight management pilot is currently running in offices with on-site Healthy Life Care Centers, but space is limited. See your clinic staff for more information.

If you don’t live or work near a Cigna Healthy Life Care Center, there may be an on-site biometric screening event at your location. Invite your covered spouse/partner to participate and earn up to $500 in incentives.

Go to myCigna to register. Then visit My Health Tab > Incentives for the individual activity amount. The health assessment and biometric screening are required before your spouse/partner can start earning incentives.

Telephone coaching for dependent children If your covered dependent child could use a little extra support, consider health coaching by phone. You can earn up to $300 per child ($100 each call), for up to three children. These incentives will be tracked under your name (not your child’s). Call 1.888.992.4462 to get started.

THE NITTY GRITTY

We’ve covered the big picture. Here’s the fine print — a few key details to know.

To earn an incentive, you must be enrolled in the Cigna Medical Plan when incentives are earned and paid. The same applies to your covered spouse/partner and dependent children. Keep in mind that you can’t earn more than the maximum incentive amounts ($766 for you, $500 for your spouse/partner and $300 per child, up to three dependent children).

You must take the health assessment and submit your validated biometric results to earn any incentives. Your covered spouse/partner must also complete these two activities; however, this does not apply to dependent children.

Incentives are paid quarterly — in your July 14, October 6, and December 15 paychecks. See 2016 Incentive Payments and Deadlines at the bottom of the webpage for details.

We have options for you. If you don’t think you can meet a health standard or complete an activity to earn an incentive, you might be eligible to earn the same reward by different means. Contact the Healthy Life Personal Health Team at 1.888.992.4462, and a coach will work with you (and, if you wish, with your doctor) to find a wellness program with the same reward that is right for you in light of your health status.
