Malabsorption – Steatorhoe
Definition Erwachsene: mehr als 10g Fett im Stuhl pro Tag
Ursachen:
- Chronische Pankreatitis, exokrine Pankreasinsuffizienz (siehe oben)
- Sprue/Zoeliakie (siehe später)
- Pankreasabflusshindernis (Pankreaskopfkarzinom, Papillenprozess, Gallensteine)
- Bakterielle Dünndarmüberwucherung
- Kurzdarmsyndrom (ausgedehnte Dünndarmresektion)
- Medikamente zur Gewichtsreduktion (Orlistat = Xenical®)
Folgen:
1) Durchfall, Blähungen, Völlegefühl
2) Mangelnde Fettresorption Kalorienmangel (Gewichtsverlust)
3) Mangel der fettlöslichen Vitaminen (ADEK-Mangel – Nachtblindheit, Osteoporose, Gerinnungsstörungen)
4) Selten: Ca-Fettsäuren-Verseifung (intraluminaler Kalziummangel mehr freies Oxalat vermehrte Oxalatresorption Oxalatnierensteine/Oxalatnephropathie

Chronische Pankreatitis – Diagnose
1) Klinische PräsentationChronische Oberbauchschmerzen (epigastrisch, gürtelförmig, Rücken)
AnamneseExokrine Pankreasinsuffizienz (Steatorrhoe wenn Lipase-Menge <10%)
Labor: Pankreaselastase im Stuhl (seltener Bestimmung Stuhlfettmenge)Endokrine Pankreasinsuffizienz (pankreatopriver Diabetes mellitus – Typ 3c)
Labor: Nüchternglukose > 7.0 mmol/l im venösen Plasma + passendeAnamnese (P.S.: Insulin und C-Peptid tief im Unterschied zum Typ 2-Diabetes)
2) Labor (Amylase/Lipase) bedingt hilfreich (falsch normale Werte)
1) Bildgebung (Abdomen leer > Sonografie > CT > MRCP=MR-Cholangio-Pakreatografie/Endosonografie): Verkalkungen, sowie weitere spezifische Veränderungen (Pankreasgangabnormalitäten – Dilatationen, Strikturen, Pankreasatrophie, Zysten/Pseudozysten)
2) Histologie: Fibrose, Verkalkungen, Atrophie der Azinuszellen

Prognose
Bei chronischer Pankreatitis
zuerst Verlust der exokrinen Funktion
(“Verdauungsprobleme”, Schmerz)
später Verlust der endokrinen Funktion
(Diabetes mellitus, Glucagonmangel)
Die morphologischen Veränderungen sind – auch wenn
es gelingt, die Ursache zu beseitigen – progredient,
irreversibel und können mit einer Organinsuffizienz
einhergehen (revidierte Marseille-Klassifikation 1985)

Chronische Pankreatitis – Therapie
Schmerz:Kann sehr stark sein; Paracetamol Novalgin® Opiate (Nebenwirkungen: Sedation, Obstipation, Abhängigkeit), nicht studiert aber diskutiert: Enzym-Substitution, coeliakale Plexusblockade, chirurgische Pankreas-Resektion
Exokrine Pankreasinsuffizienz:Enzym-Substitution: z.B. Creon® (Lipase/Amylase/Protease); 25‘000 U pro Hauptmahlzeit, 10‘000 U pro Zwischenmahlzeit – keine spezifische Diät nötig
Endokrine Pankreasinsuffizienz (Diabetes mellitus Typ 3c):‚Per definitionem‘ Insulin-pflichtig (ergo Basis-Bolus-Schema), jedoch erhöhtes Hypoglykämie-Risiko bei ebenfalls fehlendem Glucagon
Abstinenz von Alkohol und Nikotin
Anmerkung: spezifische OP bei spezifischer Indikation (z.B. Pseudozysten)
Anmerkung: Frühform, so lange asymptomatisch, nicht zwingend Therapie-Bedarf
