FY-2018/19 III-D Health Promotion Proposal Instructions Each prospective grantee should review the service definitions and read them carefully prior to completing the proposal form. In addition to the service and unit definitions found later in this document, all proposals funded with Older Americans Act Title III-D funds must meet the highest level of criteria listed below: Highest Level Criteria Demonstrated through evaluation to be effective for improving the health and wellbeing or reducing disease, disability and/or injury among older adults; and Proven effective with older adult population, using Experimental or Quasi-Experimental Design;* and Research results published in a peer-review journal; and Fully translated in one or more community site(s); and Includes developed dissemination products that are available to the public. A list of the most common, highest tiered programs is included in this packet.

Each prospective grantee should review the service definitions and read them carefully prior to completing the proposal form.

In addition to the service and unit definitions found later in this document, all proposals funded with Older Americans Act Title III-D funds must meet the highest level of criteria listed below:

There are two tabs for the Excel budget sheets that must be completed. One is a

blue tab labeled “Revenue”, and the other a red tab labeled “Expense”.

Completed proposals must be saved and emailed as an attachment to [email protected]

Completed proposals are due to CICOA by March 29, 2017, no later than 3:00 p.m.
