The “My Number Policy” for Social Security and Taxes will start operation in January 2016.Prior to the policy being implemented, foreigners who have legally resided in Japan for more than three months and have an address in Japan will be sent a “My Number Notification Card”, showing the assigned individual 12 digit (13 digit to registered companies) number beginning in October 2015. INDIVIDUAL NUMBER CARD SAMPLE The “Notification Card” shown on page 2 is needed when applying for this card. The Benefits of Using the “My Number” for use in administrative duties concerning social insurance, local tax, and disaster response (http://www.cas.go.jp/jp/seisaku/bangoseido/pdf/en2.pdf) With this system:

J-Alert nationwide test broadcasting will take place on November 25th (Wed) around 11AM. Through the citywide public address system, you will hear the following Japanese announcements. ① The Notification Card will not serve as a piece of official identification. ② Do not lose it. It must be returned when you accept your Individual Number Card. ③ A fee may be incurred when you request the reissuance of the notification card. ④ An indication will be made on your Notification Card when you have procedures for moving in/out.

Please report to the Citizen’s section (Shimin-Ka) at the City Hall. The Individual Number Card is an official identification card with your photograph, holding an IC chip

bearing your address, name, date of birth, gender, and “my number” (individual number).

The IC chip does not contain information other than identity. Please contact the help desk in the event of a theft or misplacement of the card. The help desk is accessible 24 hours a day, 365 days a year. The unlawful use of the card is prevented by assigning a password to access information, which makes it hard to make illegal use. (Please apply for the card after you receive the notification packet.)

PROCEDURES FOR APPLICATION OF THE INDIVIDUAL NUMBER CARD: → → →

• January 2016 ~ The issuance of the Individual Number Card starts. • January 2017 ~ The “my number” portal website for each individual will start operation. This site

provides an access log for your reference, and shows history of use for your individual number that is being held by administrative agencies.

• July of 2017 ~ The public agencies and relative associations will start coordinating using this system. There have been reports of suspicious phone calls of ill intent to citizens asking for personal information related to the “My Number” system. The City Hall will never make calls regarding the “My Number” system. * It should be noted that strict identification mechanisms will be employed to prevent imposters from using other's Individual Numbers, and measures to protect personal information will be in place for information management and sharing among organizations that possess Individual Numbers. Your employer will also utilize the individual “my number” individual number for insurance, tax, and other administrative purposes starting in January 2016. Collecting this sort of information for other purposes will not be possible as stipulated in the law. General Inquiries: Shakai Fukushi Ka Shomu Kakari, Ext 107 For Utilization of this Service: Kourei Fukushi Kaigo Ka Chiiki Houkatsu Shien Center, 197 Shougai Fukushi Ka, Shougaisha Shien Kakari Ext 185, Kenkou Ka Kenkou Suishin Kakari, Ext 624 The Legal Guardian is a person who is determined to be suitable for acting as a representative to act on behalf of the client, who have mental conditions due to dementia symptoms, intellectual impairment or mental disability that imposes limitations upon their capability to make important decisions (such as matters in daily life routine, or dealing with signing a contract) for themselves, to protect his/her legal rights and benefits. They are also able protect the disabled individual who is being represented by nullifying any contract that was signed under misleading circumstances in the past. It might be necessary to make a decision to appoint a legal guardian if:

You are not capable of signing a contract or cannot be responsible to manage your own money. You are afraid that you might be intimidated into a sales contract. You want to be ready for when you are incapacitated.

There are two ways for you the incapacitated adult to receive protection under the Legal Guardian of Adult System (Seinen Kouken Seido 成年後見制度), or also known as Adult Guardianship. Either a legal guardian or a power of attorney gets appointed by the court.

A Legal Guardian is appointed by the court. (offering guardianship to incapacitated adult) There are three types of Guardianships- Full Guardianship, Partial Guardianship, Person Offering Support- depending on level of incapacity.

Procedures are made at court which exercises jurisdiction over municipality in which the ward of court is registered at. Hamura City falls under jurisdiction of Tachikawa Branch of the Tokyo Family Court.

Documents proving the condition of the ward of court provided by physicians will be needed to be provided as evidence concerning the individual's medical condition and ability to perform certain tasks. An official petition to a court must be submitted to appoint the individual who will serve the as legal guardian for the alleged person with disabilities. The court will make the determination of whether or not an individual lacks legal capacity and is in need of a guardian to make decisions on their behalf. Please inquire at Family Court, Tachikawa Branch, Legal Guardian Section042-845-0324・0325 for details. ~ Who Is Appointed? ~ When the disabled person in no longer able to make logical decisions, this person may be a relative, a person with legal or social welfare background, who is fit to act on behalf of and see to it that matters are beneficial to the disabled for important decisions, etc. A Power of Attorney is appointed by notary public's office The disabled individual himself prepares for the occasion when they are no long capable to make sound decisions. A contract is signed by the disabled individual and the power of attorney before the individual loses their abilities. There are several steps involved, and when the individual can no longer make their own logical decisions, and when the occasion comes, the court will formally appoint this power of attorney to act legally on behalf of the individual.
