Class II Restoration on Distal #6 with Missing Cusp

Dr Rosenfeld is a graduate of Cornell University and Northwestern University Dental School. He served a year of General Practice residency and a second year as chief-resident at Long Island Jewish Medical Center. Dr. Rosenfeld has completed additional training in aesthetic dentistry from numerous sources, including the Nash Institute, where he has also served as a mentor. Clinical Brief:It is likely that there had been a diastema prior to loss of old composite, but I decided to try to close it with a new restoration.

Initial placement of sectional band, wedge and 3DXR ring. Technically this is an anterior tooth restoration, and some might not think to use a matrix system (generally thought to be for posterior teeth). Since there is no lingual cusp on #6, this can be a challenge for most separating rings, which would have a tendency to pop off. Notice the signifi cant distance to be bridged in order to create contact with the adjacent tooth, and the fact that the 3DXR ring does NOT pop off.

Burnishing band to “stretch” it, to ensure that good contact will be created, a VERY important step. (Remember, composite is not stiff enough to deform the band, like amalgam, and thus a good contact will not be achieved without this step).

Application of adhesive (Clearfi l SE, Kuraray).

Polymerization (Valo cordless, Ultradent). Strong contact immediately after removal of band and ring, prior to any fi nishing.
