Introduction

The purpose of the VA Approved Enterprise Standard (VAAES) Acute Inpatient NSG

Admission Screening is to create and implement a standardized nursing admission screening

note within all inpatient acute settings (med-surg, telemetry, intermediate care, intensive care*,

and acute rehabilitation) with a corresponding Standard Operating Procedure to guide nursing

practice.

This release contains the reminder dialog VA-AES ACUTE INPATIENT NSG ADMISSION

SCREEN. National health factors were developed to promote uniformity of data, and facilitation

of reports to meet directives, policies, mandates, and regulatory metrics that support the

characteristics of a high reliability organization. A complete list of health factors and the

corresponding text within the VA-AES ACUTE INPATIENT NSG ADMISSION SCREEN can

be found at the following link:

The content is based on the Acute Care Delivery (ACD) Essential Data Set (EDS) and a

prototype developed and piloted at North Florida South Georgia Medical Center. The content

has been vetted across RAPID, IPEC, Behavioral Health, Dietary, Social Work, Chaplain,

Ethics, Infection Control/Prevention, Skin/Wound, Safe Patient Handling, National Falls

Workgroup (NCPS), DOD, and multiple consulting stakeholders.

The screening template must be implemented in the production environment by all inpatient

acute (med-surg, telemetry, intermediate care, intensive care, and acute rehabilitation settings) no

later than Monday, August 10th..

* Special Instruction Intensive Care Units:

The VAAES Acute Inpatient Nsg Admission Screen note is mandatory for all ICUs regardless if

currently using CIS product.

• The template may not be changed or revised at the local level except for local elements

identified in post-install instructions.

• Requests to add elements to the note must go through the approval process outlined in the

SOP.

Please Note: The corresponding national nursing admission and shift physical assessment will

be released in a future update and is not included with this release. Until which time, the current

COVID – 19 ACUTE INPATIENT NSG SHIFT ASSESSMENT section should continue to be

used.

Pre-Install

1. Sites may want to print out an inquiry to the VA-COVID-19 NSG ADMISSION AND

SHIFT ASSESSMENT dialog or check for quick orders in the VA-COVID-19 NSG

ADMISSION AND SHIFT ASSESSMENT dialog and have those available when

installing this dialog. Below are the names of the elements you can check for quick

orders:

2. Sites may want to print out an inquiry to the VA-COVID-19 NSG ADMISSION

SCREENING AND SHIFT ASSESSMENT dialog or check local groups and elements

for local content and have the sequence items available for post install. Local content

may have been added to the reminder group VAL-COVID NSG LOCAL CONTENT

GROUP. 3. This template includes a travel symptom guide that is locally editable. This is a TIU template

field named VA-TRAVEL SYMPTOM GUIDE. Please review and save. Please Note: You

may add additional information (such as telephone extension) but do not change any of the

existing content.

Install Details

This update is being distributed as a web host file. The address for the host file is:

The file will be installed using Reminder Exchange, programmer access is not required.

Installation:

This update can be loaded with users on the system. Installation will take less than 30 minutes.

Install Example


To Load the Web Host File. Navigate to Reminder exchange in Vista

At the Select Action: prompt, enter LWH for Load Web Host File. At the Input the url for the

.prd file: prompt, type the following web address:


You should see a message at the top of your screen that the file loaded successfully

Search and locate an entry titled UPDATE_2_0_147 VA-AES ACUTE INPATIENT NSG

ADMISSION SCREEN in reminder exchange.

At the Select Action prompt, enter IFE for Install Exchange File Entry

Enter the number that corresponds with your entry UPDATE_2_0_147 VA-AES ACUTE

INPATIENT NSG ADMISSION SCREEN (in this example it is entry 186 it will vary by site).

The date of the exchange file should be 07/14/2020.



At the Select Action prompt, type IA for Install all Components and hit enter.

Select Action: Next Screen// IA Install all Components

You will see several prompts, for all new entries you will choose I to Install

For any existing items, you will choose O to Overwrite.

You will be promped to install the reminder dialog component – You will choose to install

all even though some components may have an X to indicate that they exist.

At the Select Action prompt, type IA to install the dialog – VA-AES ACUTE INPATIENT

NSG ADMISSION SCREEN

Select Action: Next Screen// IA Install All

Install reminder dialog and all components with no further changes: Y// Yes


After completing this dialog install, you will type Q.

You will then be prompted to install the health summary components, choose I to install all new

components.

For any existing items, you will choose S to Skip.

You will then be returned to this screen. At the Select Action prompt, type Q.

Install complete.

1. Enable the dialog(s) to be attached to a progress note title.

2. Create the following local note title: VAAES ACUTE INPATIENT NSG ADMISSION

SCREEN. Map it to the VHA Enterprise Standard Title of NURSING ADMISSION

EVALUATION NOTE.

3. Place the dialog in the Document Title section of Shared Templates and associate the new

reminder dialog with the new progress note title.

To do this, navigate to the Template Editor, Shared Templates, then Document Titles.

Select the appropriate folder (if applicable), and choose New Template (top right-hand

corner).

Type in the following information:

Name: VAAES ACUTE INPATIENT NSG ADMISSION SCREEN

Template Type: Reminder Dialog

Reminder Dialog: VA-AES ACUTE INPATIENT NSG ADMISSION SCREEN

appropriate.

a. Map your local social work consult quick order (or update text to reflect local

process) to OI- NSG CONSULT SOC WORK element.

b. Map your local Chaplain Consult order (or update text to reflect local process) to the

reminder element OI-NSG CONSULT CHAPLAIN.


c. Review VAL-NSG CONSULT TEXT PSYCHOSOCIAL CHAPLAN. Contains

instructional text for placing Chaplain Consult. Update text or DISABLE as

appropriate to reflect your local process.

d. Map your local Dietitian Consult order (or update text to reflect local process) to the

reminder element OI-NSG CONSULT DIETITIAN.

e. Review VAL-GP NSG DIETITIAN CONSULT OPTION - update instruction for

positive malnutrition screen, if entering a consult is not your local process.

f. VAL-NSG ADM ALLERGY CONS TO PHARM – Map your Pharmacy Consult

order if this is your local process, otherwise DISABLE this element.

g. VAL-NSG ADM MEDS DONE CONS TO PHARM – Map your Pharmacy

Consult order if this is your local process, otherwise DISABLE this element.

h. VAL-NSG ADM RISK ETOH INTERESTED PROVIDER NOTIF – can be

disabled if does not reflect your local process.

i. VAL-NSG ADM RISK ETOH INTERESTED SW MH CONS – Map the

appropriate consult order. May update text to reflect local process or disable if

appropriate. Please Note: Do not disable both VAL-NSG ADM RISK ETOH

additional information (such as telephone extension) but do not change any of the existing

content such as the precaution types including the precautionary names.


6. Populate local reminder dialog group VAL-NSG LOCAL CONTENT GROUP if needed.

NOTE: The content within VA-AES ACUTE INPATIENT NSG ADMISSION

SCREEN dialog must not be changed or revised at the local level in a way that would

alter and/or eliminate the essential content. While it is highly discouraged, facilities

can add additional screening elements to the dialog. VA medical facilities must

ensure that added content does not negatively affect system performance. All added

content must go through the following approval process:

a. Approval of the proposed added content by the facility’s nursing leadership (i.e.

Nurse Executive Board).

b. Submit nursing leadership approval memo with Associate Director of Patient Care Services

signature, including the proposed change and reason for change to

[email protected].

c. Changes should only be implemented after approval by ONS/ONI via confirmation from

[email protected]

7. NOTE: This step should only be completed by sites that have the Basic TIU Parameter

Blank Character String set to “***”. If your site uses a different character string, skip this

step. Please coordinate any changes with your local QVS (Quality Safety Value) Staff

responsible for performance measurement. If your site has the Basic TIU Parameter Blank

Character String set to “***”, you will not be able to utilize the allergy and medication

health summary (HS) objects that are located in the Allergy/Adverse Reactions Review

section and the Medication Review section of this template. You will have to insert local

objects.

a. Edit the element VAL-NSG ADM SCREEN ALLERGY DATA OBJECT and

replace the HS object VA-ALLERY REV with a locally created object that displays

documented allergies. The local HS object MUST INCLUDE Local and Remote

allergies AND must include the name of the allergy/adverse reaction.

If you are unable to provide local HS object that contain the keys elements, you

will need to complete the following steps:

• Insert text stating what is missing from the list. (Example: CAUTION does

not include remote allergies)

• Notify VHA Med Info Standards - [email protected]

b. Edit the element VAL-NSG ADM MEDICATION LIST and replace the HS object

VA-MED REC with a locally created object that displays medication lists. The

local HS object(s) MUST INCLUDE:

• Medication name and prescription sig (Example: Lovastatin 40 mg two pills

in the am)

• Active and pending inpatient medications

• Active outpatient and clinic medications

• Active remote medications

• Non-VA local medications

• 90 to 180 days of expired medications

• 90 to 180 days of discontinued medications

If you are unable to provide local object(s) that contain the keys elements, you will

need to complete the following steps:

• Insert text stating what is missing from the list. Example: CAUTION does not

include 90 days expired medications

• Notify VHA Med Info Standards - [email protected]
