Corrections is an Ideal Setting for Treatment ofSubstance Use Disorders

q Substance use disorders, including opioid usedisorders, are markedly overrepresented in ourincarcerated populations.

q Release from jail and prison is associated with adramatic increase in death from opioid overdoseamong those with untreated opioid use disorder.One study showed 12.7 increased risk of deathduring the 2 weeks post release (1.Binswanger et al2007)

q Providing treatment access during incarceration isoften the first time detainees in treatment.

q Unified system covering jails and prisons; 3,000 total detainees

q In response to overdose of a son of prominent RI businessowner, Governor mandated comprehensive opioidtreatment initiative and allocated $2 million; half went to theRIDOC

q Provides all forms of MAT to pre-sentenced and sentenceddetainees

q All detainees screened for SUD and withdrawal at intake;positives further screened with TCU5.

q TCU5 positives are urine tested within 24 hours of booking;results go in medical record, not to custody

maintained

uDetainees with SUDs are offered methadone, oralnaltrexone, and buprenorphine inductions along with SUDtreatment services and SUD treatment services under anabstinence model.

u Each inmate’s unique treatment objectives are met.

uDetainees are not cohorted in housing units based onSUD treatment, as this is felt to deter detainees fromaccepting treatment.

uMAT inductions currently can take up to 5 days to start,RIDOC is working to reduce that period.

RIDOC OutcomesuCurrently one-third of female detainees are on

MAT

u 76% of detainees released on MAT continue theircare post release and receive at least one doseof MAT; average 3 Vivitrol injections post release;buprenorphine has less continuity of use post-release than methadone

RIDOC has seen a 61% drop in opioidoverdose deaths after release within a

year of program launch, contributing to a12% overall drop in overdose deaths across

the state.

anyone released from Rikers can access methadone thenext day

q Buprenorphine not widely available yet in NYC SUDtreatment network

q Rikers is a licensed NTP provider of methadone andbuprenorphine

q “Presumptive eligibility” to be maintained on methadone,unless charges will send detainee to prison. DOC does notprovide methadone, so detainees with 45 days or less beforeprison are stared on methadone taper

Rikers Islandu Methadone provided in pill form dissolved in water,

u Suboxone provided in film form, in regular medication line

u Offers SUD treatment in Therapeutic Community too; maybe on MAT in that program

u Average 2017 185 detainees per day on methadone; 30per day on buprenorphine

u Rikers provides naloxone kits to visitors on the Island whoreceive training while waiting and pick up kits at exit.

u New York has seen twice the rate of adherence inoutpatient treatment when methadone is continued duringa jail stay compared to withdrawing detainees frommethadone

u Must build understanding across all health care andcustody staff about addiction as a disease, treatmentobjectives, and treatment tools

u Training, training, training of everyone

u Partner with custody and nursing to design strategies toreduce drug diversion

u Build a treatment system that can be sustained in thecommunity THIS SAVES LIVES

u People have different treatment objectives – honor them,including abstinence models

u Need excellent data system and evaluation plan

q On average 18,000 daily populationacross seven jails under thesupervision of the Los AngelesCounty Sheriff’s Department.

q Approximately 68% have adiagnosable SUD and 72% areco-occurring mental health andSUD.

q Of these, approximately 18% havean opiate addiction.

In January 2017, The Board of Supervisors approvedfunding to create jail-based substance use disorder (SUD)treatment services to treat 500 inmates on any given daylocated in four jail facilities for high-risk populations.

disorders.q Inmates assessed using the American Society of

Addiction Medicine (ASAM) who scored a level 2.1 orabove.
