You Will Use the MHM Data Registry Continuously Throughout the MH Model

Any point in the Model

Submit a beneficiary’s demographic and clinical data to the MH Model Data Registry for CMS validation and enrollment if you believe he or she is eligible.

Submit updated clinical data for each enrolled beneficiary annually to fulfill MH Model requirements.

Obtain MHM Data Registry Access For You and Your Team

At a minimum, all clinical and non-clinical staff who will be entering beneficiary data into the MHM Data Registry are required to set up an account

Gaining Access to the MHM Data Registry Requires Completion of Three Steps

Register for an EIDM Account

Visit the CMS secure portal and register as a new user to create an Enterprise Identity Management (EIDM) account.

Request Innovation Center Access

Use your new EIDM credentials to log into the portal and complete identify proofing and multi-factor authentication. Request access to the Innovation Center (IC).

Request Access to the MHM Data Registry

Once you are granted IC access, log into the IC and request access to the MH Model Data Registry, selecting the appropriate user role based on what function you will be serving in the Model.

MHM Data Registry FAQs

Question Answer

How do I enter beneficiary data into the MHM Data Registry?

There are three ways to enter beneficiary data:

1. Manual data entry: Enter demographic and clinical data for one beneficiaryat a time during or after a patient visit.

2. Bulk upload: Use excel templates provided in the MHM Data Registry toupload demographic and/or clinical data for multiple beneficiaries at once.

3. EHR upload: Upload a CCDA zip file exported from your EHR directly intothe MHM Data Registry.

Can I pull data into the MHM Data Registry from my EHR? What about from the MHM Data Registry back into my EHR?

You can export patient data from your EHR in a standard CCDA zip file and upload the zip file directly into the MHM Data Registry Data Page without extracting the files. The Data Page provides templates and EHR code guides to assist you with this process.

There is no functionality for exporting data from the MHM Data Registry intoyour EHR.

Can I access the MHM Data Registry on my tablet?

Yes! The MHM Data Registry can be accessed from an internet browser on anydevice.

FAQ Answer

When is my organizationexpected to begin entering data into the MHM Data Registry?

You will be able to register for access to the MHM Data Registry in December 2016. Starting in January 2017, you will be able to begin uploading and entering data into the MHM Data Registry. Any data you enter in December 2016, prior to the January 2017 official MH Model start date, will be cleansed from the system.

How do I get access to the MHM Data Registry?

Gaining access requires completion of a multi-step process. You will receive a written guide on the access request process via email in the coming weeks. All practice POCs need to secure access first because they are designated as the approvers of all access requests for other members of their organization. You can begin requesting access to the MHM Data Registry in December 2016.

How can I learn more about MHM Data Registry features?

Take advantage of the following upcoming trainings: the Understanding Beneficiary Enrollment & Alignment video and the Using the MHM Data Registry in Practice Web-Based Training. Learn more about how to access these activities by reading the weekly training bulletins distributed via email from the Model Team.
