Process Safety Courses:The Process Safety Management Competence Programme Board (PSMCPB) - comprising of the Health and Safety Executive (HSE), trade associations, industry forums and employer representation, provides guidance and expert advice on the development of national training standards for Process Safety Management which underpin our cutting-edge courses. Our courses are recognised by the HSE within the ‘Understanding COMAH: The Performance and Recognition Framework’. Process Safety Leadership

Process Safety Management Foundations

The Process Safety Leadership for Senior Executives course is designed for senior managers and board members of major hazard facilities who are ultimately responsible for developing a strong process safety culture. It provides a clear insight into how to promote the culture through the effective engagement of the workforce. Delegates are given the tools to develop an understanding of the business case for effective process safety.

The Process Safety Management Foundations course provides managers with a detailed understanding of their role in delivering effective process safety management. The course covers the identification, evaluation and prevention of accidents and incidents, as a direct result of failures in equipment, processes and procedures.

All delegates will be registered for an online assessment to test their understanding of how to apply the learning from the course.

Process Safety Management for Operations focuses on what really matters to you and your operations staff. Participants will focus on site specific hazards and control measuresas they apply to them. There are two delivery options available:

Process Safety Awareness 1

This course is designed for employees who are not directly involved in Process Safety but require a basic knowledge of process safety to enable them to work safely on site and to help them to understand the importance of a good process safety culture. It is particularly relevant to new apprentices and has been developed to meet the requirements in the new Apprenticeship Training Standards.

Process Safety Management Refresher

This course is designed for companies who want to refresh and substantiate their knowledge and understanding of Process Safety Management issues. They are flexible courses based on the industry standards, that are designed so you can choose the areas of process safety that are important to you and your organisation. It allows you to revisit key learning and principles of Process Safety Management helping you to maintain the right level of understanding of the subject within your organisation.

Effective process safety management needs to start at the Leadership level in an organisation, but also to be recognisably provided at other management and operational levels. One comprehensive way companies can achieve and demonstrate this is by adopting the Process Safety Management Competence Programme Board (PSMCPB) standards-based approach. The PSMCPB is a partnership approach started in 2011 and involving industry (both Industry Associations and some COMAH companies), HSE representing the CA, Trade Unions, and the Skills bodies Cogent Skills. Standards have been developed for 3 key job roles – Process Safety Leadership for Senior Executives (PSL); Process Safety Management Foundations (PSMF); and Process Safety for Operations and Operators (PSMO). Cogent Skills, on behalf of the PSMCPB, arranges flexible accredited training, which meets the industry-led Standards developed in each of these areas.

The Process Safety Training Standard

In 2010, writes Neil Smith, Cogent Skills and the Chemical Industries Association (CIA) identified the need from customers and members for an integrated programme of process safety management (PSM) training. They shared a vision to create a framework of PSM training standards tailored to each level in the organisation, sponsored by the key trade bodies, and supported by the Health and Safety Executive (HSE).

After further consultation, the Process Safety Management Competence Programme Board was established including Cogent Skills, CIA, HSE, UKPIA, Unite the Union, IChemE, with representation from senior industrialists across the sector with experience, expertise and interests in PSM.

Our research showed that the bulk of existing training was aimed at process safety specialists, with minimal support available at the senior executive level, or for that matter for process operators and supervisors.

Consequently, we committed to the development of a suite of PSM Training Standards, with an initial focus on the competence at board and site director level in organisations that must manage the risks and potential for catastrophic incidents.

Process Safety Leadership for Senior Executives was launched in 2011, followed by Process Safety Management Foundations in 2012, for operational managers and supervisors, and Process Safety management for Operations in 2013 for front line operators and maintenance personnel.

The purpose of these Training Standards is to provide a clear knowledge and understanding of the principles of process safety management across an organisation, and help organisations to develop and maintain a strong process safety culture.

The commitment shown by the founder members of the PSM Competence Programme Board to continue to drive forward this agenda is outstanding.

The Board continue to meet monthly with the aim of constantly build-ing awareness and engaging new businesses and industry sectors; stakeholders support the independent quality assurance process for all course providers; and Board members have shared their vision through speaking at numerous industry events including three Process Safety Management Sum-mits, in 2012, 2015 and 2018.

The initial goals have long been exceeded, and the programme has now successfully engaged 182 Upper and Lower-Tier COMAH sites and 115 non-COMAH sites, with >10,000 individuals trained – with around 1500 of these in Leadership roles.

It is however clear, that there is still a long way to go in engaging and convincing industry leaders of the importance of their role in demonstrating visible Process Safety Leadership, hence the focus of the Health & Safety Executive on Leadership as a strategic topic from 2019 onwards.

This programme provides front line managers and supervisors with the tools and knowledge to evaluate, develop and maintain the ongoing competence of employees, against agreed competence frameworks. Competence Management System Review The Competence Management System Review is the starting point for identification and quantification of the levels of workforce competence within an organisation. WHO has the biggest impact on process safety in your organisation?

Is it the operators and shift managers who ensure manufacture continues smoothly on a daily basis, the engineers who continually strive for improvements in process integrity, efficiency and effectiveness, or the senior leaders who set the culture and direction of the organisation?

In reality, all of these people and more have a role to play in managing process safety, and each has to recognise just how important their influence is on the continued safe operation of a major hazard organisation. Of course, it can be difficult to help others – especially those senior leaders with a non-technical background – to recognise the effects that their decisions can have on process safety.

From my experience of meeting with non-technical senior leaders, it’s crucial not to use the sort of niche, technical terms that you might freely exchange with your peers. If your audience doesn’t understand the jargon that you’re using, then frankly the discussion will be useless to them.

Effective communication is about the audience not the author. Believe me, this isn’t the time to be getting out your Swiss Cheese model. You need to tailor your message in non-technical language.

I hope this article finds its way into the hands of senior business leaders, and if you are looking at ways to get their attention, I would urge you to pop it into your bosses’ inboxes.

If this article has found its way to you and you’re a non-technical reader then it’s important to clarify that by ‘process safety’ I mean the way in which major hazard risks are managed and controlled, rather than risks to occupational or personal safety. I’m not talking about systems to prevent slips, trips or falls but rather much larger incidents that not only harm health but can cause multiple fatalities, as well as huge environmental, financial, and reputational damage.

We mustn’t forget that major hazard businesses have the potential for catastrophic impact. The exploration and production of oil and gas, the manufacture and storage of chemicals and petrochemicals, and the generation of energy involve complex processes with in-built intrinsic hazards that need careful management.

The measures your business needs to contain such hazards in a controlled way are equally as complex. This requires a systematic approach to managing risk, basedon having several layers of protection to guard against each process risk, and robust emergency arrangements in case the worst does happen.

Senior leaders need to recognise how important their influence is on the safe running of hazardous processes or the execution of a major project, whether the activity is located on their doorstep, or on the other side of the world. It might not be immediately obvious for example, what effect reducing the number of maintenance hours, or extending the frequency of inspections of critical process plant or delaying shutdowns or turnarounds can have on the safety of a plant, but the future consequences can be extreme.

The need for this recognition from the Board becomes especially pertinent if you reflect on the investigations into a number of high profile accidents such as the explosions and fires at BP’s Texas City oil refinery and the Buncefield fuel storage depot in the UK, which identified various technical failings, but also how shortcomings in leadership by senior executives contributed to the incidents.

Addressing these shortcomings is not easy, as while it is easy enough to focus on any number of training courses available for engineers, it can be much harder getting the attention of senior leaders of major hazard organisations;despite the fact that understanding the risk from a major accident is just as important as understanding every other type of business risk, if not more so.

The good news is that industry and the regulator have developed recognised training standards in process safety leadership and process safety management for everyone from front-line operators to the chief executive of a major hazard organisation.

The prime offering coming from this partnership is a one-day programme, Process Safety Leadership for Senior Executives. The benefits of this training are clear:

When we first developed these standards in 2012, I truly believed this to be a ground-breaking piece of work. It filled that gap in training for senior leaders and has been proven to be appropriate for all executive boards, no matter what size the company or where in the world they are located.

Those of us with less direct contact with such events must also connect with these emotions to strengthen our drive to learn and implement the lessons. Accounts such as Stephen McGinty’s ‘Fire in the Night: The Piper Alpa Disaster’ can help with this.

Piper Alpha has been a primary source for many vital lessons for both high hazard and occu-pational health and safety. These include the need to ensure, for example,

> effective permit-to-work and shift handover procedures > that hazardous events and how they may escalate are fully understood> emergency systems and procedures are maintained in a state of readiness, are protected from the hazardous events where practicable, and will be put into action without delay> thorough high quality auditing that is, by definition, independent

Underpinning these practical lessons, Piper Alpha led to a major change in the regulatory environment for off shore installations, the Offshore Installations (Safety Case) Regulations 1992 (and 2005). This was an offshore adaption of the regulatory framework propose in 1976 in the First Report of the Advisory Committee on Major Hazards (ACMH) set up after the Flixborough explosion. The work of the ACMH was heavily drawn on for the first Seveso Directive, implemented as the CIMAH Regulations in the UK, and more recently the COMAH Regulations.

The ACMH took inspiration from the Nuclear Installations licensing regime introduced in 1965. Following the King’s Cross fire in 1987 and crash at Clapham Junction in 1988, the Railways (Safety Case) Regulations 1994 (and 2000) were introduced. The MOD introduced safety case principles for military systems during the 1990s. And now, following the shocking Grenfell tower fire, the Independent Review of Building Regulations and Fire Safety: Hackitt review has proposed safety case principles are applied to buildings fire safety for large complex projects.

These regulations require the organisations accountable for risks capable of causing catastrophic widespread harm to demonstrate to a regulatory authority that they have:

> a comprehensive understanding of their hazards and accident scenarios,> identified and implemented appropriate prevention, control and mitigation measures, and> systems in place to ensure these measures remain present and reliable

This demonstration, when sufficient, gives such organisations their license to operate. Whilst not perfect, this ‘prove it’ approach to high hazard regulation has without doubt resulted in improved risk control.

We use a similar approach for occupational health and safety competence where the con-sequences of failure can be serious. We use knowledge and skills tests for driving on the roads and for vehicles such as fork lift trucks and cranes. We license asbestos contractors, certify radiation protection advisers and register gas engineers.

So why do we so often accept superficial demonstration of competence for management and leadership roles where the individual is accountable for serious health and safety hazards? Has it ever been acceptable to take it on trust that good decisions are always made when people are appointed to positions with such serious safety accounta-bilities and responsibilities? Why do we think that training that concludes with short term memory tests is proof of safety management and leadership competence?

Just one organisation I have encountered requires new managers to robustly demonstrate the success of their efforts to understand the hazards and control measures in their area of responsibility. They do this at the end of a 6 month probationary period with a panel of experienced managers and special-ist advisors. They get support, but retaining their job depends on success with this process.

More organisations need to put in processes like this. Perhaps our failure to invest in de-veloping and demonstrating competence in safety management and leadership, from front line managers to chief executives, explains why slow progress is made with improving safety performance and culture.
