    Oral care devices are extremely critical for achieving optimum oralhealth. Toothbrushes are considered vital for daily oral hygiene mainte-nance and helps in reducing plaque, caries and other dental problems.An ideal toothbrush may be defined as the one which removes plaqueeffectively.1 However, there is a lack of awareness regarding theselection of toothbrushes and proper brushing method.

    1. How to useMechanical cleaning refers to removal of dental plaque and bacteria

    from hard and soft tissues, which may be achieved by correct toothbrushing, inter-dental cleaning and tongue cleaning.

    Selection of correct need based toothbrush It is very importantto select the right kind of toothbrush. Super-soft toothbrush shouldbe used by patients with dentinal hypersensitivity; inter-dentalcleaning devices by patients with caries or periodontal diseases;orthodontic brushes for patients undergoing orthodontic treatmentand a soft tissue cleaner (like the implement on the back of theColgate 360 toothbrush head) for cleaning the tongue and cheeks.

    Brushing technique The most popular methodinvolves placing the brush at 45 to the axis of thetooth and pressing lightly against the tooth and gummargin (Fig 1). This is followed by about 10 smallvibratory back-and-forth movements over the width

    of approximately half of the tooth. Then the brush isgently taken towards the occlusal or incisal surface.These movements are repeated with all teeth and allsurfaces of teeth present in the oral cavity.

    Inter-dental cleaning devices Inter-dentalcleaning devices may be classified as: inter-dentalbrush, dental floss, dental stick and water-pik.Inter-dental brush (Fig 2) is efficiently able to cleanlarge inter-dental spaces while dental floss (Fig3) ismost effective for narrow interdental spaces. Water-pik is very effective in removing food-debrisespecially after it is loosened with the floss orinterdental brush.

    Why do we have to brush the teeth clean?It is clear that oral cleanliness is the final goal of tooth brushing. Eachindividual should be able to feel and appreciate the well beingassociated with oral cleanliness, so that otherwise would not beacceptable.

    The motivation for good oral hygiene practice has for long been the

    prevention of oral diseases. In recent years, it has become evenmore convincing, thanks to a better understanding of the pathogen-esis of plaque linked oral diseases and their impact on the generalhealth. Mechanical oral cleaning has been confirmed as the mostefficient approach to plaque removal.

    The key factors to achieve good tooth brushing performance areundoubtedly, proper instructions and a strong personal commit-ment to oral health.1

    How to brush the teeth clean?

    Although people are familiar with tooth brushing as a mechanicalcleaning routine, they are not aware that there has been scientificevidence to support each recommendation for good oralhygiene2, 3, 4 such as:

    We should also insist that a good oral care regimen be customizedas per each patients needs and applied in an individualized wayto

    What do we have at hand to brush the teeth clean?Nowadays, a variety of tooth brush heads and handles are availableto address the difference in conformity of oral structures. Innovativetooth brush design along with appropriate brushing technique canhelp us to better address the needs to prevent and control specificoral conditions.

    In case of special needs and impaired manual skills, the use ofelectric brush has been proven to improve teeth cleaning.

    Finally, whenever needed, the use of toothpastes with specificactive ingredients should be recommended to provide additionalbenefit for better protection against caries, gum problems anddentin hypersensitivity.2, 3, 4

    In conclusion, brushing the teeth clean is within our hands. Effortsshould be made to turn tooth brushing from a mechanical routinethe least, regular visit to the dentist will help to professionallyaddress our oral health needs in order to maintain good oralfunctions and a healthy smile throughout our life.

    We are taught tooth brushing techniques sincekindergarten, we are overloaded withrecommendations to promote oral health, a great

    variety of oral hygiene utilities are available to us,and still our teeth are not as clean as they shouldbe! Why does the problem persist? May be, weshould get back to the very basic issues of oralmechanical cleaning to look for some answers.

    Over the last decade, therehas been a significant

    thrust in media (print &

    electronic) on the aware-

    ness campaign towards

    oral healthcare & its

    related benefits. Reams

    of literature, tons of

    newsprint & gigabytes of

    cyberspace have been

    utilized to drive home all theimportant messages of Oral

    health & general health links.

    This piece is yet another attempt

    with a slight difference and there in

    lies the significance.

    Teeth and gums are designed and structured to assist the variousfunctions of mastication and the allied benefits of speech andaesthetics.1,2 They also offer the added dimension of a windowon systemic (general) health. Early signs of disease and variations

    in general health patterns may be reflected in the manner inwhich the gums and teeth display the condition of intactness orbreakdown.5 Diseases of the teeth and gums often begin at thehighly inaccessible area between the teeth called as the INTER-DENTAL AREA (area between two teeth).3 While the plaquewhich builds up in this area can be removed by brushing andflossing, it needs a highly motivated patient and some effort toreach these areas.4

    Hence, in view of the above stated facts, maintaining a goodmeticulous oral hygiene regime is essential. The importance ofbrushing twice a day is known to all. As an adjunct to this, dailyroutine dental floss, interdental brushes, toothpicks, water

    irrigation devices etc. are of greatbenefit. Inaccessible interdental areas,beyond the reach of our toothbrush,can be effectively cleaned by the abovestated mechanical interdental aids.2,6

    The use of a dental floss must be empha-sized to maintain a good oral environment

    for prevention of dental caries (tooth decay)& a healthy periodontium (gums).1,4 It is also

    easily available and easy to use. In cases ofmild bone destruction and gum recession,interdental brushes are of a great help. These

    brushes help cleanse the interdental areas therebypreventing further destruction of teeth and gums.4

    Water irrigation devices like water picks are also usedextensively ensuring a tooth shower and give a fresh and

    clean feeling to the patient.

    In addition to the above mentioned interdental cleaningmethods, anti-plaque products like mouthwashes (mouth rinses)and dentifrices can also be your mouths great friend. They helpto keep bad breath (halitosis) & disease causing microorganismsat bay.4,6

    The ideal time to start brushing a child's teeth iswhen the teeth first begin to appear in the mouth.To establish good brushing habits, parents shouldset a good example for their children. It is alsonecessary to introduce children to a toothbrush atan early age.

    The benefits gained from starting tooth brushing at an early age areclear. When to start brushing is as important as the habit itself. Usinga brush without toothpaste is sufficient for children who have startedbrushing their teeth at an early age. Yet, toothpaste use should alsobe encouraged after making sure that the child will not swallow it.

    In addition to caries caused by feeding bottles, one of the mostcommon causes of dental caries in younger children is the habit ofeating foods that are rich in sugar and its derivatives. Feeding bottlecaries mainly occur when children sleep with feeding bottles in theirmouth. A reduction in sugar consumption is not sufficient to preventsuch caries. Children should be taught to adopt good brushinghabits, and they should learn not to eat or drink anything after theybrush their teeth.

    Protecting and maintening milk teeth is of great importance to thehealth of permanent teeth that erupt later. Milk teeth that have beenprematurely lost pave the way for serious future problems, includingdefects in the order of permanent teeth eruption and placement aswell as tooth irregularities also known asorthodontic anomalies. And so, parents can notsimply take the attitude of they will be lostsooner or later.

    If some primary molar teeth remain in the mouthlonger than others, dental shifts will occur later onthat lead to tooth irregularities. Therefore, school-age children should be encouraged to brush theirteeth with toothpaste regularly. It is also important

    to choose a toothbrush that is appropriately sizedfor children. Brushing techniques recommendedfor adults should be encouraged as children growolder.

    Regular visits to the dentist are important inchildhood, particularly in two ways: First, parts ofteeth that are susceptible to decay can be coatedwith protective substances and dental cariesprevented by the application of fluoride. Second,meeting a dentist at an early age will also helpprevent dentophobia from occurring in childrenand will make it easier to see a dentist in thefuture.
