Heraldry of tHe College

The upper portion of the badge of the College consists of a golden lion in a passantstance superimposed on the chief which is red in colour.

The lion represents our nation of Singapore. Red is the colour of blood which is theprecious liquid on which life depends and which is our mission to save.

The lower portion of the badge consists of a green cross superimposed on a whiteshield. The green cross symbolizes the humanitarian mission of our College. White isthe colour of purity which is our aim.

On the green cross is superimposed an open book and a caduceus. The open bookrepresents knowledge and continued learning and the caduceus is the recognizedsymbol for the art of healing.

The past year has been a busy one for the College, where we have seen many good developments for family medicine! As we continue to work to advance the cause of family medicine in our fair isle, the future for our specialty looks especially bright. I am pleased to report that the 26th Council has done very well and its work is well reported in the pages below.

Membership The membership of the College as of 31 March 2018 increased from 1998 to 2202. This represents an over 10% increase in membership numbers and bodes well for our fraternity. This rate of increase makes us very hopeful that our numbers will continue to grow well with each year. A larger membership would reflect improved representation of our family medicine and primary care fraternity, giving us a stronger mandate to be the voice of family medicine in Singapore. Our Council continues to engage medical students to increase the visibility of family medicine. Together with the Joint Committee on Family Medicine Singapore (JCFMS), our co-organised career seminars have attracted many, much to the tremendous effort by the medical student Special Interest Group in Family Medicine. Many of our young colleagues have shown interest in family medicine as a career option, as evidenced by the large numbers applying for the Graduate Diploma in Family Medicine (GDFM) course, as well as many applicants for the Master of Medicine (Family Medicine) [MMed(FM)], in both the residency and College courses.

Assets The College’s financial situation continues on a strong footing, all thanks to the prudent fiscal stewardship of our Council. College’s financial reserves remain healthy, providing stability and security for the operations of our College. Although we continue to generate a healthy surplus, we need to be constantly mindful of the need for fiscal prudence, so as to ensure our College continues to be financially secure, and able to be a viable champion of family medicine into the future.

Courses conducted The GDFM, the MMed(FM) (College Programme) and the Fellowship of the College of Family Physicians Singapore (By Assessment)(FCFPS) are the signature programmes run by our College and they continue to do well. The curriculum and delivery of these programmes are constantly reviewed and improved over the years. We have just launched our enhanced curriculum for the GDFM, to cater the needs of our ageing population. We have 247 applicants for the GDFM this year and we expect these numbers to increase over the years.

PartnershipsOur College continues to have excellent relationships with the Singapore Medical Association and the Academy of Medicine Singapore. The three professional bodies meet regularly, and keep each other abreast of our activities. On a number of occasions that warranted a profession-wide approach, we had taken joint action, issuing joint statements on a number of issues. The College continues to have a good working relationship with the Ministry of Health and the Singapore Medical Council. We are consulted regularly on policy matters, with many of our suggestions being well-received, resulting in improvements to proposed regulations.

The 6th Asia Pacific Primary Care Research Conference, in conjunction with the Family Medicine Symposium, was held at the Academia from 21-23 Sept 2017. This was co-organized by the College, SingHealth Polyclinics and Singhealth Duke-NUS Family Medicine Academic Clinical Programme. The conference was a resounding success, with over 380 registrants from 9 countries, including Malaysia, Australia, India, Sri Lanka, the United Kingdom, Fiji, China and Hong Kong. The FM symposium also attracted many participants, with much interest in the Geriatrics and Acupuncture breakout sessions.

Major Developments The 26th Council continued well the work of the College in various initiatives. In particular, we have worked closely with the Ministry of Health to further enhance the Graduate Diploma of Family Medicine (GDFM).

The enhancement of the GDFM is an on-going development between College and the Primary and Community Care (PCC) Division of the Ministry of Health. The revamp incorporates enhancements in areas of need such as chronic disease management, geriatrics and mental health. An additional Certificate of Competency for competency in community hospital practice is available to those with a special interest in community hospital work. This certificate is recognised by the community hospitals, and will be issued by the College.

We are now midway through the term of the 26th Council. It is through Unity that we have been able to achieve so much in such a short time. A huge word of thanks to our dedicated and self-sacrificing Council Members, who have worked tirelessly for the profession and fraternity! We are also very grateful to our academic faculty who serve in our College training programmes. It is because of their commitment and dedication that we are able to pass on the skills and knowledge to the next generation of family physicians. Thank you very much!

We have also to thank our excellent Secretariat, who work long and hard to make the operations of College run smoothly. Thank you, Jennifer, Anica, Patricia, Guorong, Chester, Joel, Qiyin and Peggy!

It has been a privilege to serve the College and the fraternity, and we seek your continued support and mandate to continue the good work of the College.

Sreenivasan Orator 2017The Sreenivasan Orator for 2017 was Professor James Best, Dean of Lee Kong Chian School of Medicine, Singapore.

Koh Eng Kheng Gold MedalThe Koh Eng Kheng Gold Medal was established under the kind donation of the late Dr Koh Eng Kheng’s family. The Medal would be awarded to the Best Fellowship Candidate of each academic year.

The presentation of the mock cheque was held at the Family Medicine Convocation Ceremony on 18 November 2017. Adj Asst Prof Tan Tze Lee is presented with the mock cheque from Dr Kevin Koh.

The recipient of the Koh Eng Kheng Gold Medal for 2017 was Dr Loo Yuxian.

Family Practice Skills Course (FPSC)

CONTINUING PROFESSIONAL DEVELOPMENT/ MEDICAL EDUCATION PROGRAMMES AND COURSES

The College promotes Continuing Professional Development (CPD) and Continuing Medical Education (CME) through the Family Medicine Modular Course, Family Practice Skills Course, MMed (Family Medicine) Programme B, Family Medicine Membership and Fellowship Programme.

The Institute of Family Medicine (IFM), an organ within the College, plays a significant role in developing the contents and syllabi of these programmes and courses.

During the year the IFM reviewed, updated and edited the notes as well as their e-learning components for Modules 4, 5, 6 and 7 of the Family Medicine Modular Course to make them more relevant and user-friendly to the course participants.

The Family Medicine Modular Course consists of 8 modules of knowledge acquisition and is a requirement for doctors who are preparing for the Graduate Diploma in Family Medicine (GDFM) Programme and the Master of Medicine (Family Medicine) College Programme. Each year, four modules are covered.

For the year April 2017 - March 2018, there were 236 trainees registered in the GDFM course, 20 trainees in the MMed(FM) College Route and 23 trainees in the Fellowship [FCFP(S)] programme.

During this financial year, the College conducted four Family Practice Skills Courses.

The components of a skills course may include one or more of the following:1. Distance Learning with verification (6 CME points upon attaining minimum pass grade of 60% in online MCQ Assessment) 2. Seminars (2 CME points per seminar) 3. Workshops (1 CME point per workshop) 4. Small group activities including clinical attachments, tutorials, ward-rounds (for some courses)5. Assessments which can be written, oral, demonstration and/or clinical (for some courses)

A typical FPSC template is a course with six units of distance learning with 30 MCQs test, conducted in two seminars, and also workshops conducted over two afternoon sessions typically, one on Saturday afternoon and one on Sunday afternoon.

E-LearningThe E-Learning programme launched in 2003 has been incorporated into the GDFM training programme. An E-Learning component for each module was introduced, consisting of therapeutic notes, case scenarios, assessment by MCQs and a case review to be put up by a tutorial group.

Accreditation of core CME pointsThe College is represented by Adj Asst Prof Tan Tze Lee in the Singapore Medical Council (SMC) CME Co-ordinating Committee. The College works closely with the SMC to ensure that the quality of the CME events is acceptable. It continues to advise SMC on the accreditation of core Family Medicine CME events through its panel of Family Medicine (FM) CME advisors appointed by the College.

In the year under review, the College Mirror has seen a revamp in its masthead and content layout. There has also been some restructuring of the College Mirror editorial board into 4 teams, with each team covering the same quarter of the year. The quarterly publication continued in its mission of putting together timely news to update the family doctor fraternity. More details of the restructuring is given in the publications committee report.

Annual General Meeting 2017The College AGM was held on 29 July 2017 at College of Medicine Building (COMB). The minutes have been circulated.

Commencement of Academic Year 2017The Commencement Ceremony was held on 29 July 2017 at College of Medicine Building (COMB).

There were 236 trainees in the GDFM course, 20 trainees in the MMed(FM) College Programme and 23 trainees in the FM Fellowship [FCFP(S)] Programme.

Convocation 2017The Family Medicine Convocation Ceremony was held on 18 November 2017 at NUSS Kent Ridge Guild House and the Guest-of-Honour was Mr Ng How Yue, Second Permanent Secretary, Ministry of Health.

15 doctors were conferred the MCFP(S). Of which, 20 were awarded the MCFP(S) by Election on 18 November 2017, 12 were awarded the MCFP(S) by Election on 4 January 2018 and 1 was awarded the MCFP(S) by Assessment on 18 November 2017. 78 GDFM graduates and 45 MMed(FM) graduates were recognised.

GRADUATE DIPLOMA OF FAMILY MEDICINE (GDFM) The Graduate Diploma in Family Medicine (GDFM) is a vocational training certification for primary care doctors. The aim of this 2-year part-time trainee programme is to train primary care doctors to practise Family Medicine at an enhanced level to meet the needs of the child, the adolescent, the adult and the elderly. The course emphasises on basic clinical, diagnostic and management skills essential to general practice.

The programme is planned to accommodate the busy doctor’s schedule as almost all courses that require in-person attendance are conducted outside regular office hours. The course consists of the following: l 8 modules of Family Medicine Modular Course (FMMC) with each comprising of 4 workshops, 1 tutorial, online case studies and multiple choice assessments l 3 Practice Management Courses and 1 elective Family Practice Skills Course

The examination is conducted by Division of Graduate Medical Studies (DGMS), National University of Singapore (NUS).The standard set is a pass in all three segments: l Written paper I - Applied Knowledge Test: 100 MCQ questions (2 hours) l Written Paper II - Key Features Problems (KFP): 10 questions (1 hour) l Skills Assessment in the Objective Structured Clinical Examination (OSCE) - 10 stations of 11 minutes each, comprising 3 minutes for reading and 8 minutes for assessment (2 hours)

The GDFM(NUS) is a title or qualification approved by the Singapore Medical Council for display by Registered Medical Practitioners.

The 16th GDFM Examination was held on 23 June 2017 for the written papers and 1 & 8 July 2017 for the OSCE. A total of 78 of the 130 candidates passed, giving a passing rate of 60%for the GDFM Examination 2017.

MASTER OF MEDICINE (FAMILY MEDICINE) [MMed(FM)]

There are currently two programmes to prepare doctors for the MMed(FM) examinations.

Family Medicine Residency Programme The Family Medicine Residency Programme aims to equip the resident with educational and clinical experiences in accordance with ACGME-I standards, which focuses on structured training and close supervision. The resident will undergo clinical rotations in the hospitals and polyclinics, call-back sessions and teaching sessions that combine foundation teaching, continuity clinics, skills workshops/attachments and community medicine.

MMed(FM) College Programme The programmeis a structured training programme designed for mature practising family physicians in Singapore. The programme is jointly organised by the College of Family Physicians Singapore and the DGMS, NUS.This 1-year part-time course provides a comprehensive and structured training programme for doctors working in primary care who have completed the 8 modules of the Family Medicine Modular Course (FMMC) to prepare them to sit for the MMed(FM) examinations.

THE AWARD OF MCFP(S) BY ELECTION TO MMED(FM) HOLDERS WHO FULFILL CRITERIA On 18 November 2017, 20 applicants were awarded the Collegiate Membership of the College of Family Physicians Singapore [MCFP(S)].

On 4 January 2018, 12 applicants were awarded the Collegiate Membership of the College of Family Physicians Singapore [MCFP(S)].

Family Practice Skills Course (FPSC)

The Family Practice Skills Courses (FPSC) continue to serve the continuing professional development needs of the General Practitioner/ Family Physician. Each skills course includes one or more of the following components in which the family physician can choose to participate in: l Distance Learning with verification (6 CME points upon attaining minimum pass grade of 60% in online MCQ Assessment) l Seminars (2 CME points per seminar) l Workshops (1 CME point per workshop) l Small group activities including clinical attachments, tutorials, ward-rounds (for some courses)l Assessments which can be written, oral, demonstration and/or clinical (for some courses)

The IFM is encouraged by the big increase in the number of doctors enrolled in the GDFM modular courses year on year. The Family Practice Skills Courses also saw an increase in some of the courses.

The enrolment numbers in the 2017 intake was 236 compared to 169 in the 2016 intake. The 4 Family Practice Skills Courses had attendances ranging from 122 to 225 in the seminars and 110 to 220 in the workshop; the number of doctors who did the CME reading and passed the MCQs ranged from 186 to 516. Newer topics attracted bigger attendances e.g., the extended consulta-tion, and updates in rheumatology.

The year of 2017 has been a fruitful one for College Mirror. Report for the period under review

1. We are pleased to report that there were no staff resignations for the last financial year.

2. The medical education programmes and training activities undertaken by College remain strong and a core function. There have been enhancements made to both the GDFM and MMed programmes to better need the needs of the trainees for the future. This would mean increased work for the secretariat.

3. Thus the committee will be looking into increasing the manpower of the secretariat to strengthen the team to meet these challenges and to carry out its mission better and more effectively in the future.

In the financial year from 1 April 2017 to 31 March 2018, the Group derived its income mainly from subscription fees, rental income and fees from its courses and seminars. The 3 items together accounted for 88.8% of its total income.

Fees from courses and seminars accounted for 68.6% of the total Group income, rental income accounted for 7.2% of the total income and subscriptions accounted for 12.9% of the total income.

In the financial year from 1 April 2017 to 31 March 2018, the Group’s consolidated total income was $2,386,347; the consolidated total expenditure for the same period was $ 1,879,671. The consolidated net surplus for the financial year from 1 April 2017 to 31 March 2018 was $497,542.

I am pleased to report that the Group continues to be on a firm financial standing for the financial year 1 April 2017 to 31 March 2018.
