Attached is a copy of the Suspend Admission Form, the Teach Out Plan, and documentation of an Open Forum. The last students enrolled were in academic year 2016-2017 and the students were granted the BHS degree in the fall of 2017. There are no active students enrolled. The faculty in the Division of Physical Therapy were asked to vote to suspend and close the PHTH-BHS in Physical Therapy. 19 Faculty were queried. 19 voted in favor of suspending and closing the program, with 0 opposed, and 0 abstentions. The College held an open forum on Tuesday April 9th to document and hear any concerns or questions about the suspension. Two faculty attended and no concerns or questions were proposed that would suggest not supporting the closure and suspension of the BHS in Physical Therapy. PHTH-BHS Physical Therapy

    The faculty of the college in which a degree program resides may opt to suspend admissions for up to one year without going through the University Senate, pursuant to the established college faculty rules document. A college faculty may opt to have an issue concerning temporary suspension of admissions to an academic program forwarded to the Senate Council for Senate consideration. The decision to suspend admissions to a Senate-approved academic program for longer than a single year is a significant reduction, which must be approved by the Senate. After five years of suspension, the Council on Postsecondary Education (CPE) considers the program to be closed and no students will be admitted. After the CPE designates a program as “closed” any attempt by the department to resurrect that program will be treated as a new program proposal. Once approved at the college level, your college will send the proposal to suspend admissions to the appropriate Senate academic council (Health Care Colleges Council, Graduate Council, Undergraduate Council) for review and approval. Once approved at the academic council level, the academic council will send your proposal to the Senate Council office for additional review via a committee and then to the Senate for approval. Once approved by the Senate, the Senate Council office will notify appropriate entities, including the Registrar’s office. The contact person listed on the form will be informed when the proposal has been sent to committee and other times as appropriate.

    Suspending and closing the program because the College of Health Sciences no longer wishes to offer the program. Previously students transferring into UK could enroll and earn a Bachelors under the Scholars Program while enrolled in the masters or doctoral in physical therapy program. It was determined that the initial mechanism that allowed this process was inappropriate. It has since become apparent that there are other pathways for students to earn a BS using transfer credits therefore, the College of Health Sciences wishes to suspend and remove the PHTH-BHS. The last students enrolled was in academic year 2016-2017 and they were granted the degree in the fall of 2017. There are no active students enrolled.

    The faculty of the Health Sciences Education and Research Division in the Department of Clinical Sciences

    and those in the Medical Laboratory Sciences program held a vote to vet the proposed change to the requirement

    for the Certificate in Nutrition for Human Performance from “Student must earn a B or higher in each required

    certificate course to receive the certificate,” to “Student must earn a C or higher in each required certificate

    course to receive the certificate.” Of these eleven faculty members, 9 votes were received to approve and no

    votes received to vote against or abstain, thus passing unanimously in the Division and Medical Laboratory

    Sciences programs.
