This is a lzrter of attestation required by Title 10 CPR 35.5 1 in ordm to have Silvana Oliveira added to our NKC license as an authorized medical physicist for HDR use.

Silvvna graduated from McGilI University in June, 2003 with a Masters o f Science in Medical Physics.

Iier High-Dose-Rate (HDR) Afterloading training included both the HDR A€brloader unit itself and the Vtrisource treatment planning sybtem. This training was given by the Vendor (Varh Medical Systems) on-site Ocbber 20-22, 2004. The training included hands-on operation, safery procedures, clinical use, and proper use of the treatment planning system.

Silvana has also been involved with rhe performance of daily S ~ O K checks, full calihations, decay corrections, and the clinical treatment of over twenty patient trearments under my supenvision since muting, her full-tune employment here on September 15,2003. In addition, Cynthia Goodman-Mumma has supervised her training and parriciyatioa in sealed source I& test$, s o m e inventory, and rddiarion surveys around the HOR remote afterloding Unit and treatment room.

In summary, Silvana has compl&d Master’s Degree training in medical physics, and an additional two years of full- time work exparience under the supervision of an individual who meets fhe requiruments for an authorized medical physicist for KDR.

Her training and work experience were conductud in clinical radiation facilities thar provi& high-energy exrernal b a n Lherapy and bmhytherapy services.

The experience and mining outlined above satisfies 10 CFR 35.51 (b) (1) and (c).

Finally, in my opinion, she has reached a levo1 of competency sufficient to function dependently as an authorized medica1 physicist for any HDR activitius including rratment procedurcs.

His training and work experience were conducmd in clinical radiation facilities that provide high-energy enhmal beom therapy and brachytherapy services.

In summary, Alex has completed two years of full-time training in medical physics, and an additional one ind a half years of full-time work experience under the supervision of an individual who moeB the requirements for em authorized medical physicist for HDR.

The experience and training outlined above satisfies 10 CFR 35.51 (b) (1) and (c).

Finally, in my opinion, Alex has reached a level of competency sufficient to function independently as an authorized medical physicist for any HDR activities including treatment procedures.
