From giving you incentives to make smart lifestyle choices, to providing you with individualized solutions and information for health issues that concern you, AmeriHealth New Jersey Commit2Wellness programs are designed to help you and your family stay well, prevent illness, and benefit from healthy lifestyle choices.

Commit2Wellness Rewards | ProgramsThrough AmeriHealth New Jersey Commit2Wellness Rewards, you can earn Wellness Dollars for completing a variety of healthy activities, such as seeing your primary care physician (PCP) or gynecologist (GYN) or by exercising regularly. As you take steps toward a healthier you, you’ll work toward receiving a reward. Wellness Dollars are awarded for completing milestones in each program. Once you earn Wellness Dollars, you can redeem them for a wide selection of gift cards from hundreds of retailers.

To be eligible to redeem any of your Wellness Dollar accruals, you must complete an online Personal Health Profile (PHP) on amerihealthexpress.com and one other Commit2Wellness Rewards Activity. Once you complete the PHP and an additional Commit2Wellness Rewards Activity, you can start redeeming your Wellness Dollars.

Fitness Program | Earn 100 Wellness Dollars for staying active. Among other things, regular physical activity can help you reduce cholesterol and high blood pressure, manage your weight, maintain bone strength and muscle tone, and even boost your mood. Complete three 30-minute sessions each week every month and we’ll reward you 100 Wellness Dollars annually.

Healthy Weight, Healthy You | Earn 100 Wellness Dollars when you complete an approved weight loss program. No one ever said weight loss would be easy, but support from others can make the challenges feel more manageable. Enroll in a weight loss program, and you can get encouragement to stick with it for the long haul. Complete your program and we’ll reward you 100 Wellness Dollars annually for program fees.

Tobacco Cessation | Earn 100 Wellness Dollars when you complete a program to help you quit using tobacco products. Quitting isn’t easy, and many people try more than once before they succeed. To help you quit for good, we’ll reward you 100 Wellness Dollars for completing a tobacco cessation program.

Parenting Class | Earn 50 Wellness Dollars for attending a parenting class (childbirth preparation, prenatal exercise, education of expectant siblings).

Please contact customer services or your benefi ts administrator to determine if this benefi t applies to your coverage. If nutritional counseling services are provided in addition to other covered services, an offi ce visit copay may apply. Deductible and coinsurance may apply to services obtained out of network.

Nutrition Counseling | Earn 50 Wellness Dollars for attending two nutritional counseling visits with a registered dietitian or physician. Registered dietitians can counsel you on using food and nutrition to help prevent and control disease, gain or lose weight, improve fi tness or live healthier. As part of your AmeriHealth New Jersey benefi ts, you can schedule up to six nutritional counseling visits per year* at no additional cost when you visit an in-network dietitian or physician. No referrals needed!

Self-reporting Instructions | To participate in the Commit2Wellness Rewards program, healthy activities must be self-reported and a Personal Health Profi le (PHP) must fi rst be completed on amerihealthexpress.com. You’ll need to enter your score on the Commit2Wellness Rewards homepage along with one other activity to start accumulating points. To get started, follow these easy steps!

1. Log on to amerihealthexpress.com. Select the Health & Wellness Programs tab then select Commit2Wellness Rewards.

2. On the Welcome Page select Start Earning your Wellness Points Today to be directed to the Commit2Wellness Rewards site.

3. First time users follow the registration prompts or simply log in if you already have an account.

4. On the home page select the activity you would like to report. For example: To report a Fitness Activity, select Fitness and Exercise, select Report, then enter in the days in which you engaged in at least 30 minutes of exercise activity, then Submit. Points earned may take one business day to appear in the system.

5. To redeem your points, select Rewards and follow the instructions.

Stress Management | Learn techniques to restore balance – because severe or prolonged stress can really threaten your health. It’s been linked to six of the leading causes of death, including heart disease and cancer. Through AmeriHealth New Jersey Commit2Wellness, you can discover ways to let go of stress or transform it into positive energy. Complete an approved Stress Management or Meditation Activity and we’ll reward you 50 Wellness Dollars.

Early Detection | The best way to live and stay healthy is to prevent health problems before they occur. Yet no matter how well you take care of yourself, the effects of heredity, the environment, and other factors may not prevent illness. That’s where early detection comes in. By scheduling regular health screenings, you’re more likely to identify and treat a potential problem during its early stages. And, early detection and early treatment mean you have the best chances for a complete recovery. With our ConnectionsSM Health Management Programs* and Wellness Guidelines, we will help you make decisions about and schedule important screenings in the following areas:

on teenage health issues. • Baby Footsteps® helps you prepare for your new baby’s arrival. • Adoptive parent support provides information about health resources for

adoptive parents and children. • Health Coaches* are specially-trained health professionals, including nurses, dieticians,

and respiratory specialists, who are available 24 hours a day to address your health

concerns. Spanish-speaking Health Coaches are also available.
