we're going to continue the process that we began 11
believe it or not almost two years ago in June of 2015 12
of honoring and recognizing and showing our 13
appreciation to employees who have reached the 14
milestone of ten years of service in the Gaming 15
Control Board. These employees for their loyalty and 16
their commitment to their employment with us have 17
certainly contributed to the success of our agency. 18
So, Joe's going to help me. We have ten 19
honorees today. The first five please stand up and 20
come forward a couple feet. 21

So, our first honoree today is Todd 23
Snyder. Todd was hired on March 19th, 2007 as an 24
Applications Developer within the Office of 25

Our next honoree is Karen Miller. Karen 4
was also hired on March 19th, 2007 as a Clerical 5
Support Specialist for the Bureau of Investigations 6
and Enforcement (BIE). Today Karen serves as an 7
Investigative Analyst for the BIE. Congratulations, 8
Karen, and thank you for you service. 9
Next we have Jenn Schneider. Jenn was 10
hired on March 26th, 2007 as a Clerical Support 11
Specialist for the BIE. Today Jenn holds a position 12
of Licensing Technician within the Special Services 13
Unit for the Bureau of Licensing. Congratulations, 14
Jenn, and thank you for your service. 15
Our next honoree is Kenneth Zielonis. 16
Ken was also hired on March 26, 2017 --- I often 17
wonder if you guys remember coming in on the same day. 18

Ken began his career as a Hearing 21
Officer for the Office of Hearings and Appeals (OHA) 22
and he continues to fulfill the responsibilities of 23
Hearing Officer for our agency today. Ken, 24

The first matter before the Board is the 21
OEC's request to revoke Brandy Micheli-Cheatham's 22
Gaming Employee 2 Permit as well as to place her on 23
the Board's Involuntary Exclusion List. 24
The uncontested facts in the matter were 25

that on July 15th, 2016, Ms. Micheli-Cheatham, while 1
employed as a Table Game Supervisor at Sands and while 2
working as a Boxman at a Craps Table, took six $500 3
chips and placed them inside her clothing. After her 4
conduct was identified, she was arrested and charged 5
with theft by unlawful taking as well as terminated 6
from her employment with Sands. 7
In August of 2016, the OEC filed the 8
Complaint to revoke her G2 Permit and to request that 9
she be placed on the Involuntary Exclusion List. Ms. 10
Micheli-Cheatham initially requested a hearing but 11
subsequently signed a waiver of that hearing and 12
therefore the matter is before the Board based upon 13
the documentary record. 14
As indicated, Ms. Micheli-Cheatham is 15
present and has been sworn to address the Board. 16

Before we hear from you, what is the 18
status of the criminal Complaint? 19

benefit of the ARD Program. That program is six 1
months and I believe it is scheduled to come to a 2
conclusion in late July. So, I believe that would 3
mean they're still technically pending. 4

Yes, first I'd like to say thank you for 15
even having me here. I've been in the casino business 16
for 20 years. This is the first time anything has 17
ever happened and they have seen fit to give me the 18
ARD which I really appreciate which is up in July 19
23rd. And I definitely have learned my lesson from 20
this. 21
I had gotten a divorce in May, which has 22
prompted me to do this, and since then I've lost my 23
job, I lost my house, my car, my animals and my kids 24
are being taken away this summer and I'd just like to 25

get consideration of possibly being able to go back 1
into the gaming industry again so I can try and get my 2
life back on track. 3

applied fairly and this kind of conduct, which you 19
exhibited, is extremely serious --- 20

credibility of casinos here in the State of 25

Yes, the individual was working as the 14
box person at the Craps Table. She was a G2 Employee 15
which is a Upper Level Gaming Employee. It had been 16
detected that she had placed six $500 gaming chips 17
inside her clothing and secreted them in the clothing. 18
It was seen --- caught on surveillance and verified 19
after which, you know, presumably that the report to 20
the State Police resulted in the charges. 21

which Deputy Chief Counsel Steve Cook is here to 5

Good morning. The only other things on 8
the OCC's agenda is our three Petitions for 9
Withdrawals or Surrenders of Credentials filed by 10
Advance Gaming Associates, LLC, Linda S. Cook, no 11
relation, and Herbert Lugmayr. The OCC has no 12
objections to any of these Withdrawals or Surrenders. 13
As a result, if the Board chooses to grant the 14
request, they'd be doing so without prejudice to each 15
requester. 16
I would note however that Advance Gaming 17
Associates' Petition requests a pro rata refund of the 18
$45,000 three year Supplier License Fee paid in April 19
of 2016. The facts triggering this request are that 20
Advance Gaming merged into and became part of American 21
Gaming and Electronics, Incorporated just seven months 22
after having renewed it's Supplier License and paid 23
that fee. 24

subsequently post-merger submitted an application to 1
the Board to become a Supplier itself. That company's 2
application is presently under review with the BIE and 3
it is likely that matter will come before the Board 4
for licensure in one of the next few meetings. If 5
American Gaming and Electronics is ultimately approved 6
for a Supplier License, that new company will be 7
required to pay a $25,000 licensing fee. 8
So, because of this situation, Advance 9
Gaming is seeking a pro rata or basically a two-thirds 10
refund of the $45,000 fee that it paid. 11

Jersey based company that manufactures, among other 1
items, gaming chips, tablets --- tables, playing 2
cards, dice and gaming table layouts. It also serves 3
as a supplier for its associated company GPI Mexicana 4
whose license we will take up next. 5
The BIE has completed its investigation 6
of the company and the Bureau of Licensing has 7
provided you with a background investigation and 8
suitability report. I have provided you with the 9
draft Order and ask that the Board consider the Order 10
to renew the Table Game Manufacturer License. 11

On February 23rd, 2017, two blue cards 20
went missing during table game play and were later 21
discovered in an automatic shuffler. A surveillance 22
review of the incident determined that between 2:31 23
p.m. and 7:24 p.m., on five separate occasions, the 24
automatic shuffler malfunctioned indicating that the 25

blue deck was missing cards. In each occasion, the 1
dealer continued table game play with the compromised 2
blue deck. 3
The surveillance review determined that 4
approximately two or five hands were played using this 5
compromised blue deck and the compromised blue deck of 6
cards was in play for approximately four hours and 53 7
minutes. The surveillance review further determined 8
that eight different players were impacted by the 9
compromised play and as a result a total of $215 was 10
returned to the players. 11
Under the terms agreed upon, Lady Luck 12
would pay a civil penalty of $10,000 as well as a 13
$2,500 investigative fee. 14
At this time, OEC requests that the 15
Board approve this Consent Agreement. 16

allowed an unlicensed employee to serve on its Board 7
of Directors and function in the position of Chairman 8
of the Board of Directors for approximately 96 days. 9
The terms of the Consent Agreement would require IGT 10
Global Solutions Corporation to pay a civil penalty in 11
the amount of $12,500 for allowing an unlicensed 12
employee to serve as a Director and Chairman over the 13
Board of Directors for approximately 96 days and IGT 14
Global Solutions Corporation is also required to remit 15
$2,500 to the Board for investigative costs incurred 16
in relation to this Consent Agreement. 17

Mr. Peles was charged by the 14
Pennsylvania State Police with one count of 15
harassment, subject other to physical contact and one 16
count of disorderly conduct engaging in fighting. 17
Ultimately Mr. Peles pled guilty to the one count 18
harassment subject other to physical contact. 19
At this time, OEC requests that the 20

On January 17th, 2017 Mr. Anderson was 7
part of a group of three individuals who caused a 8
disturbance at the Rivers Casino by refusing the leave 9
after he and his group were asked to do so. For his 10
part, Mr. Anderson hid on the gaming floor while 11
security personnel attempted to escort him out and 12
placed his hand on the chest of security personnel 13
after being told not to do so. 14
Mr. Anderson was arrested by the 15
Pennsylvania State Police and charged with three 16
summary offenses, defiant trespass, disorderly conduct 17
and harassment. 18
As a result, OEC request that the Board 19
issue an Order placing Nicholas Anderson on the 20
Involuntary Exclusion List. 21

one count of theft by deception, one count of access 23
to vice counterfeit, one count of access to vice 24
issued to another, one count of possessing access to 25

In April 2016, Mr. McGarvey pled guilty 2
one count of --- to one count of theft by deception. 3
As a result, Mr. McGarvey was sentenced to five years 4

pay Harrah's $3,735 in restitution. 7

As stipulated by the parties on the --- 1
on April 28th, 2016, Mr. Glaser left his 11 year old 2
son alone in the vehicle in the Sands parking garage 3
while he entered the phone --- the casino to retrieve 4
a phone which was in the possession of a friend of 5
his. Mr. Glaser was on the gaming floor for a total 6
of 32 minutes, during which he played at a slot 7
machine for 23 minutes. 8
The stipulation further states that 9
while on the gaming floor retrieving the phone, Mr. 10
Glaser checked his players card and found that he had 11
free play available. He decided to use the free play 12
believing that he would only be a few minutes until it 13
ran out and that his son would be fine during the 14
time. 15
Mr. Glaser agreed that it was poor 16
decision on his part to leave the child in the car, 17
however the --- he was not criminally charged but was 18
evicted from the Sands Casino. With that explanation, 19
Mr. Glaser is here to address the Board. 20

left his cell phone he had and I said of course if 6
there's any issues call me. I feel I'd just be right 7
in and out. I found out I had free play when I was 8
there. I put my card in and Murphy's Law actually won 9
and as I'm playing I took the money out and then I 10
heard my name over the loudspeaker. Well, I 11
immediately tried to call my son figuring something 12
had happened. And his phone went dead, he was reading 13
a book in the car. 14
I normally don't leave my son in a car 15
anywhere. I figured at the Sands Casino being in the 16
parking garage, it was safe because I know the 17
security's excellent there and I figured I'm just 18
running in to retrieve a phone. It was a horrible 19
decision of course to leave my son any time. I wasn't 20
aware that I was breaking any kind of law. I'm sure 21
you guys are aware that I believe any children under 22
the age of seven can't be left. 23
Since then I've talked to the Sands 24
Casino and they said they've I believe made it the age 25

14. But either way I can understand where they're 1
coming from and they had told me just to write a 2
letter and they'd let me back in. Because this 3
happened I believe over a year ago now. 4
And I just figured I wasn't going to go 5
in there for --- before writing the letter. I figured 6
I did wrong, I was going to wait, you know, six 7
months, a year before writing the letter to security. 8
And like they said, the Bethlehem State Police --- or 9
Bethlehem Police came to the scene and said, you know, 10
it was a horrible decision, there was nothing I did 11
criminally illegal but, you know, --- and the officer 12
told me to apologize to my son which I did. He ended 13
up getting the money that I won on the game. 14
Again, I just hope that the Board will 15
see that, you know, I was looking over the exclusion 16
list and it says --- and I looked at online who these 17
people are and I just feel that, you know, I don't 18
belong on that list. And it says exclusion of 19
individuals who are career or professional offenders 20
and I just don’t feel that I'm, you know, one of those 21
persons. 22
I do understand that I did wrong. 23
Security --- they had said they had a lot of problems 24
