How It Works (Technical Description) ��32

Why pressure mapping? It is often assumed that nothing should be in the bed between the patient and a pressure reliev-ing mattress, but a pressure relieving mattress it not enough. Patients must be turned or reposi-tioned several times a day for a number of reasons including personal hygiene, dressing, moving and handling and pressure relief. Turning or repositioning can cause shear on the user and the risk of strain or injury for the caregiver.

The combination of a turning/slide system and a pressure relieving mattress will often make good sense to protect the patient from pressure sores and the carer from injury. In this study we have placed the Immedia “in bed systems” on top of a standard foam mattress and an alternat-ing air mattress to see whether it will be within acceptable pressure levels.

Pressure mapping is a “snapshot” of pressure on a person’s body lying on a defined mattress in a given time period under given conditions. If one parameter is changed such as person, posi-tioning, mattress, temperature or clothes, a new and different snapshot will appear. All measurements have been taken with FSA “Body Track”independently by Raskov1 & Co together with Ole L. Thy, Physiotherapist, Etac Education, and Anette R. Raun, International Product Manager Transfer.

ObservationsIn general the air mattress has a lower average pressure on all measurements, BUT during the circulation the maximum can be higher on certain pressure points. Supine positioning gives the highest average and maximum pressure measurements, but on a smaller surface area.

Whether an in-bed system is used or not, the pressure has the same tendency to be higher on known and defined risk points; shoulder, elbows, hip-area and heels.

4WayGlide:Whichever 4WayGlide mattress (cotton/polyester or polyurethane) and base sheet is used, the results show nearly the same picture as they would if the mattress is being used on its own. There is even evidence of a small improvement when the 4WayGlide is used in conjunction with a pressure mattress for supine positioning.

TwinSheet4Glide:Whichever option of BaseSheet is used with the TwinSheet, on both foam and air mattresses results show slightly higher average and maximum measurements as on the mattress without an in-bed systems. It is worth noting that the 2-part top of the TwinSheet4Glide Mattress is not visible on the measurements.

SatinSheet:Whichever version and combination of SatinSheets are used with the foam and air mattress, results show very little difference when using the mattress without an in-bed system. There is a small improvement on the average and maximum measurements using the SatinSheet, especially on the foam mattress.


A mattress and in-bed system can be combined without any significant change to the benefits of the mattress. Furthermore, in-bed systems improve the working environment for the carer and minimise shear on the user. When combining an in-bed system with a pressure relieving mattress, a risk assessment must be carried out. The conclusion was clear:

Results show that there is an insignificant change of pressure recorded on the test patient when combining Immedia’s various “in-bed” systems with foam or air mattresses”

Remember: the best way to prevent pressure ulcers is pressure distribution and minimising shear and friction.

What is pressure mapping? Interface pressure mapping involves using sensors to quantify the pressure between two contact-ing objects, such as a person and their support surface. Pressure mapping has many wide spread applications, but in assistive technology it is commonly used by clinicians to determine the suit-ability of a wheelchair cushion and by researchers investigating support surfaces, risk factors for ulceration, and ulcer prevention protocols.

Cost of pressure ulcers2 Pressure ulcers are a painful, debilitating and potentially serious outcome of a failure of routine medical and nursing care.In the UK the total cost of pressure ulcer care is around £1.4bn – 2.1bn per yearMore than 90% of the cost of treatment is the cost of nursing time.Without concerted effort this cost is likely to increase in the future as the population ages and the incidence of pressure damage increases.The focus should be on prevention— prevention of initial tissue damage, prevention of pro-gression of an ulcer to a more severe grade and prevention of infection. Pressure UlcerInternational NPUAP-EPUAP Pressure Ulcer Definition

A pressure ulcer is localized injury to the skin and/or underlying tissue usually over a bony promi-nence, as a result of pressure, or pressure in combination with shear. A number of contributing or compounded factors are also associated with pressure ulcers; the significance of these factors is yet to be made clear

What causes a pressure ulcer?

Numerous factors can cause a pressure ulcer, one of 3 will always be present:

• Friction • Shear• Pressure

Adding one or more factors from the list below will in all probability increase the risk further:

• Age• Mobility • Weight• Moisture• Incontinence• Infections• Skin conditions• Temperature• Medicine• Nutrition• Sensory perception• Mental capacity• Material• Clothes• Medical history• And ……….


Immedia “in-bed systems”IN-BED SYSTEM FOR USERS WITH LIMITED MOBILITY

Immedia in-bed systems are all characterised by a 2 part system consisting of a sheet placed directly on the bed mattress and a top layer either as a drawsheet or a mattress. The main advantage with in-bed systems, is that they can remain in bed at all times. No application or removal of the slide sheets is necessary. User-friendly moving and handling solutions are critical to ensure safe working environments.

Suitable also for users with dementia, or for those who feel unsafe, have brain damage, pain, pressure ulcers or are heavier.

4WayGlide The Immedia 4WayGlide System is an in-bed system for severely immobile patients.

Our 4WayGlide System always consists of two parts: a nylon sheet attached to the mattress and a Glide Mattress. The combination of the underside of the Glide Mattress and the nylon sheet provides an extremely low friction surface for movement in all directions, which facilitates positioning and turning, whether manual or with support of a hoist.

TwinSheet4Glide

THE IN-BED SOLUTION FOR INCONTINENT USERSImmedia TwinSheet4Glide is a 2-part glide mattress suitable for turning and positioning incontinent users with severely reduced mobility.

Disconnecting the lower part, when used with a hoist, provides better access for care of pressure ulcers, hygiene, dressing and even to change the bedding.

SatinSheet

A COMPLETE IN-BED SYSTEM FOR TURNING AND POSITIONING

Immedia SatinSheet System reduces friction between the user and the bed. By reducing friction, two major challenges are addressed.• The workload on the caregivers is reduced which means less work related injuries• The shear forces on the patient are reduced which lowers the risk of pressure sores.

Mattress Type Foam100 mm HR40The cover is stretchable. The mattress is used for prevention and treatment of NPUAP Pressure Ulcer categories 1 and 2.

AirDynamic pressure relieving mattress, consists of 17 individual air cells, each 120 mm high.The pressure in the mattress adjusts to the user’s weight and the pump is running in a cycle with a time interval of 10 minutes, which alternately increases and decreases the pressure. The cover is stretchable. The mattress is used for prevention and treatment of category 1,2 and 3 pressure sores.

TestMeasurements The results are described in three main areas:• mm Hg = Maximum pressure in relation to the range of the study. A low figure is indicative of minimal pressure.• Average mmHg = The average of the number of cells that are used. Lower numbers are indicative of low pressure value. • Coefficient of Variation: standard deviation/mean - the lower the % the better! This shows how evenly is the pressure distributed over the surface.• Sensing area = cm2 area of the cells that are in use, the higher the number the smaller the average pressure.

The results are highlighted in the measurements which cover the legs, pelvis, spine and shoulder but not the head. All measurements have been done by Raskov & Co

Method of testing

• Supine and lateral positions • 6 minutes per position on the foam mattress• 10 minutes per position on the air mattress to cover a whole cycle• For this presentation the FSA pressure distribution scale is set on 50 mmHg - this is a generally accepted base line. It is believed that pressure above this level is not acceptable for patients• Surface area 850 x 1980 mm• Average pressure• Peak pressure

General Guidelines

How it Works (Technical Description)4

The FSA pressure mapping system is based on Piezo Resistive Technology. This means that the resistance changes with applied pressure.

FSA has a proprietary Piezo resistive semi conductive polymer sandwiched between two layers of highly conductive rip stop nylon fabric. The floating sandwich allows conformability to the compound curved surfaces of the seating environment as the slippery layers move freely and minimise hammocking.

The changes in resistance which result from the different pressures on the semiconductor are interpreted by the Interface module and relayed to the computer where they are displayed as an array of colors and digital pressure values. Corrections are made along the way for hysteresis (direction of loading) and creep (changes with time) and individual sensor variations.

FSA measurement elements:FSA-pressure measuring carpet consists of a set of product protected pressure sensitive sensors that are minimally affected by temperature and humidity. Researchers have verified that the measuring carpet possesses the following characteristics: reliable, repeatable, durable and accu-rate.The technical components found in the ultra-thin measuring blankets, incorporated in the sub-stance is 0.36 mm thick. Each sensor unit is very thin (1“x 1”) and consists mainly of three layers of fabric.

Two measurement options:1. Continuous scanning that provides a real time picture2. Scanning and recording that provides a real time image and all images are recorded

There are eight display options including combinations of isobars, iso numbers, press center, fencing (3D), statistics and opposite isobars or iso numbers.The screen displays a range of statistics, including max. pressure, number of active sensors, aver-age pressure, pressure center, variation coefficient and standard deviation.FSA-pressure measurement system currently used in:• rehabilitation clinics and other health care facilities• research • manufacturers and suppliers of medical equipment• equestrian sports
