CONFERENCE OBJECTIVESAt the conclusion of the conference, the participant will be able to:

• Integrate the knowledge of past foundations in Native Hawaiian healthinto their practice.

• Implement culturally appropriate models of health care service anddelivery in improving the health and wellness of Native Hawaiians.

• Integrate into the efforts of the existing programs in workforcedevelopment for culturally competent providers.

TARGET AUDIENCEThis program should be of interest to physicians, physicians-in-training, social scientists, nurses and health care providers who serve Native and Pacific Peoples.

CONTINUING MEDICAL EDUCATIONThe Hawai‘i Consortium for Continuing Medical Education is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

The Hawai‘i Consortium for Continuing Medical Education designates this live activity for a maximum of 6.5 AMA PRA Category 1 Credits TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

A limited number of hotel rooms are available for conference participants at the Waikiki Beach Marriott Resort & Spa at the special group rate. To receive conference room rate, please call in your reservations directly to the Waikiki Beach Marriott at 808-921-5444 and inform the reservation agent that you are with the UH Native Hawaiian Health conference, Group codes: 33WDIG (Kama'aina - HI ID required) and 33WDIH (non-residents).

Cut off date for special group rate is August 10, 2015. Hotel room reservations are based on availability of rooms.

Attendees will be registered on a first-come, first-served basis. Registration fees cover course materials, continental breakfast, breaks, and lunch.

DEADLINEDeadline for early registration is August 28, 2015. Please complete the online registration form and submit. If paying by credit card, complete the Billing Information form. If paying by check or purchase order, please submit online registration and contact the UH Hilo Confrence Center directly for payment instructions - Email: [email protected] or Phone: (808) 974-7555. Deadline for all registrations is September 4, 2015.

REGISTRATION REFUND POLICYRequests for refunds must be received by the University of Hawai‘i Hilo Conference Center in writing by August 28, 2015 via email at: [email protected]. No refunds will be made thereafter. All cancellations are subject to a $25 administrative fee. Refunds will be processed within four (4) weeks after the completion of the conference.

CONFERENCE PARKINGConference parking will be available at the Waikiki Beach Marriott Resort & Spa. Daily validated parking rates per vehicle are $8.00 for self-parking and $12.00 for valet.
