Friends, This past year is characterized by organizational improvements and connections to better address a community need for accessible healthcare. Each day we find ourselves inspired by our patients and focused on delivering on our promise to be the health center where you belong. In partnership with others who share our passion and dedication to health and wellness, we opened two additional health center sites. One as part of the refugee coalition initiative at Reynolds Middle School where we serve refugees and the general community, and the other embedded within Philhaven’s outpatient center on North Prince Street to more effectively address both the physical and behavioral health needs of our patients. We’ve responded to patients’ needs for increased women’s health services, a pediatric specialty, and more care coordination and social work services. Our dental department is enjoying a renovated and expanded waiting room area, thanks to a generous donor, and increased appointment slots using best practices for patient care. To accommodate growth, our staff has increased from 154 at the beginning of the year to 178 employees. The Board of Directors completed a self-assessment, resulting in a renewed commitment to the organization’s mission and our responsibility to assure delivery of high-quality and compassionate care for all. The Affordable Care Act included a mandate and resources to assure that no one is living without the healthcare they need. We take seriously our opportunity to play a critical role in achieving that ideal and purposefully look forward to continued growth to meet the healthcare needs of our neighbors.

Zai Da Be and daughter Rose StannaWhen Rose was two months old, she developed a high fever. Her mother,
Zai, had taken her to the emergency room where it was discovered that Rose had heart issues. A refugee from Thailand, Lutheran Refugee Services referred Zai to SouthEast for Rose’s follow-up care. Zai and Rose were also referred to Georgia Clack, SouthEast’s Patient Care Coordinator.
Georgia, knowing that Rose needed to go to the Children’s Hospital of Philadelphia for further testing, could sense Zai’s anxiety with the
language barrier. Zai herself suffered from anxiety and depression. Georgia decided it was best to travel with Zai and Rose to the hospital,
and helped with buying the train tickets, calling a taxi, and the hospital visit to put Zai at ease during a trip that may have otherwise not happened without
Georgia’s assistance.
With the help of her care team, Rose now takes medication regularly, sees a SouthEast provider for regular checkups, and continues to have her heart monitored as she grows older.

Baby Rose was able to receive follow-up care and important testing, despite language barriers. Muriel was able to manage her health and wellness by regularly seeing her provider. You gave care, healing and hope to many who otherwise would have had no place to turn. You’re the reason we can keep offering care to everyone, and we’re so grateful for your caring support.
Dana BairMemory issues. Transportation issues. Lack of health insurance. Difficulty
connecting to local resources. These reasons and more were why Dana was missing his medical appointments and unable to manage his deteriorating health. During a time that he was rescheduling yet another appointment, he began talking to Patient Care Coordinator Georgia Clack. He immediately scheduled an appointment with her as a “cry for help.”
Georgia asked Dana what his main barriers were to receiving the care that he needed. She then helped him with making various appointments,
connecting to local resources, finding a provider within his new health insurance plan, and most importantly, making it to his appointments. Georgia
simply gave him the support that he desperately needed.
Dana continues to see Georgia and to keep his medical appointments. “Georgia is the best support I’ve had,” Dana smiled. “She’s like family to me.”

Muriel BardAn Addictions Counselor from Normandy, France, Muriel understands the
importance of taking care of one’s health. She also understands first-hand the obstacles that can appear while trying to manage chronic health issues.
Throughout her life, Muriel suffered from alcoholism, depression, and other medical concerns. While she was in-between jobs, she lost her health insurance and was doing her best to self-manage her health.
She was anxious to find a primary care provider that could dive into her medical and behavioral history and regularly care for her.
Muriel began coming to SouthEast six years ago because of our sliding fee discount program. Dr. Weitzel became her primary care provider, and has helped
Muriel every step of the way in managing her health and wellness.
All five of Muriel’s children come to SouthEast, and she often refers coworkers and clients to the friendly, culturally competent services available in various languages, something she can personally appreciate.
