Consumer Confidence Report IMPORTANT INFORMATION (This report must be printed in Landscape Orientation to prevent cutting off of text) The following pages comprise the Annual Consumer Confidence Report (CCR) for your water system. To download the CCR into your word processing program follow these steps (Remember you must have the document set up in Landscape Orientation): Choose Select All from the edit dropdown MENU, ( i t w i l l highlight a l l the information). Choose Edit from the MENU, select Copy from the edit dropdown MENU. Open your word processing program. Choose Edit from the MENU, select Paste from the edit dropdown MENU and the information will transfer. Choose Edit from the MENU. In order to meet all of the requirements of the CCR, you must include the following additional information i f i t pertains to your water system. The report must include the telephone number of the owner, operator, or designee of the community water system as a source of additional information concerning the report. In communities with a large proportion of non-English speaking residents, as determined by the Primacy Agency, the report must contain information in the appropriate language(s) regarding the importance of the report or contain a telephone number or address where such residents may contact the system to obtain a translated copy of the report and/or assistance in the appropriate language. The report must include information about opportunities for public participation in decisions that may affect the quality of the water (e.g., time and place of regularly scheduled board meetings). • If your water system purchases water from another source, you are required to include the current CCR year's Regulated Contaminants Detected table from your source water supply. • If your water system had any violations during the current CCR Calendar year, you are required to include an explanation of the corrective action taken by the water system. • If your water system is going to use the CCR to deliver a Public Notification, you must include the full public notice and return a copy of the CCR and Public Notice with the Public Notice Certification Form. • The i n f o r m a t i o n about l i k e l y sources of contamination provided i n the CCR i s g e n e r i c . S p e c i f i c i n f o r m a t i o n r e g a r d i n g contaminants may be a v a i l a b l e i n s a n i t a r y s urveys and source water assessments and should be used when a v a i l a b l e to the operator. • I f a community water system d i s t r i b u t e s water to i t s customers from m u l t i p l e h y d r a u l i c a l l y independent d i s t r i b u t i o n systems fed by d i f f e r e n t raw water sources, the t a b l e should c o n t a i n a s e p a r a t e column f o r each s e r v i c e a r e a , and the r e p o r t should i d e n t i f y each s e p a r a t e d i s t r i b u t i o n system. A l t e r n a t i v e l y , systems may produce s e p a r a t e r e p o r t s t a i l o r e d to i n c l u d e data f o r each s e r v i c e a r e a .

• D e t e c t i o n s of unregulated contaminants f o r which monitoring i s r e q u i r e d are not i n c l u d e d i n the CCR and must be added. When added, the i n f o r m a t i o n must i n c l u d e the average and range a t which the contaminant was d e t e c t e d . • I f a water system has performed any monitoring f o r Cryptosporidium, i n c l u d i n g monitoring performed to s a t i s f y the requirements of the Information C o l l e c t i o n Rule [ICR] (§141.143), which i n d i c a t e s t h a t Cryptosporidium may be p r e s e n t i n the source water or the f i n i s h e d water, the r e p o r t must i n c l u d e : (a) a summary of the r e s u l t s of the monitoring; and (b) an e x p l a n a t i o n of the s i g n i f i c a n c e of the r e s u l t s .

• I f a water system has performed any monitoring f o r radon which i n d i c a t e s t h a t radon may be p r e s e n t i n the f i n i s h e d water, the r e p o r t must i n c l u d e : (a) The r e s u l t s of the monitoring; and (b) An e x p l a n a t i o n of the s i g n i f i c a n c e of the r e s u l t s .

• I f a water system has performed a d d i t i o n a l monitoring which i n d i c a t e s the presence of other contaminants i n the f i n i s h e d water, EPA s t r o n g l y encourages systems to r e p o r t any r e s u l t s which may i n d i c a t e a h e a l t h concern. To determine i f r e s u l t s may i n d i c a t e a h e a l t h concern, EPA recommends t h a t systems f i n d out i f EPA has proposed an NPDWR or i s s u e d a h e a l t h a d v i s o r y f o r t h a t contaminant by c a l l i n g the Safe D r i n k i n g Water H o t l i n e (800-426-4791). EPA c o n s i d e r s d e t e c t s above a proposed MCL or h e a l t h a d v i s o r y l e v e l to i n d i c a t e p o s s i b l e h e a l t h concerns. For such contaminants, EPA recommends t h a t the r e p o r t i n c l u d e : (a) the r e s u l t s of the monitoring; and (b) an e x p l a n a t i o n of the s i g n i f i c a n c e of the r e s u l t s n o t i n g the e x i s t e n c e of a h e a l t h a d v i s o r y or a proposed r e g u l a t i o n .

• I f you are a ground water system t h a t r e c e i v e s n o t i c e from the s t a t e of a s i g n i f i c a n t d e f i c i e n c y , you must inform your customers i n your CCR r e p o r t of any s i g n i f i c a n t d e f i c i e n c i e s t h a t are not c o r r e c t e d by December 31 of the y e a r covered by i t . The CCR must i n c l u d e the f o l l o w i n g i n f o r m a t i o n :

- The nature of the s i g n i f i c a n t d e f i c i e n c y and the date i t was i d e n t i f i e d by the s t a t e . - I f the s i g n i f i c a n t d e f i c i e n c y was not c o r r e c t e d by the end of the c a l e n d a r year, i n c l u d e i n f o r m a t i o n r e g a r d i n g the State-approved p l a n and schedule f o r c o r r e c t i o n , i n c l u d i n g i n t e r i m measures, p r o g r e s s to date, and any i n t e r i m measures completed. - I f the s i g n i f i c a n t d e f i c i e n c y was c o r r e c t e d by the end of the c a l e n d a r year, i n c l u d e i n f o r m a t i o n r e g a r d i n g how the d e f i c i e n c y was c o r r e c t e d and the date i t was c o r r e c t e d .

The sources of d r i n k i n g water (both tap water and b o t t l e d water) i n c l u d e r i v e r s , lakes, streams, ponds, r e s e r v o i r s , s p r i n g s , and w e l l s . As water t r a v e l s over the surface of the land or through the ground, i t d i s s o l v e s n a t u r a l l y - o c c u r r i n g minerals and, i n some cases, r a d i o a c t i v e m a t e r i a l , and can pickup substances r e s u l t i n g from the presence of animals or from human a c t i v i t y .

Contaminants t h a t may be present i n source water i n c l u d e :

M i c r o b i a l contaminants, such as v i r u s e s and aa c t e r i a , which may come from sewage treatment p l a n t s , s e p t i c systems, a g r i c u l t u r a l l i v e s t o c k operations, and w i l d l i f e .

I n o r g a n i c contaminants, such as s a l t s and net a l s , which can be n a t u r a l l y - o c c u r r i n g or r e s u l t from urban storm water r u n o f f , i n d u s t r i a l or domestic wastewater discharges, o i l and gas production, mining, or farming.

P e s t i c i d e s and h e r b i c i d e s , which may come from a v a r i e t y of sources such as a g r i c u l t u r e , urban storm water r u n o f f , and r e s i d e n t i a l uses.

Organic chemical contaminants, i n c l u d i n g s y n t h e t i c and v o l a t i l e organic chemicals, which are by-products of i n d u s t r i a l processes and petroleum production, and can also come from gas s t a t i o n s , urban storm water r u n o f f , and s e p t i c systems.

Radioactive contaminants, which can be n a t u r a l l y - o c c u r r i n g or be the r e s u l t of o i l and gas production and mining a c t i v i t i e s .

D r i n k i n g water, i n c l u d i n g b o t t l e d water, may reasonably be expected t o c o n t a i n at l e a s t small amounts of some contaminants. The presence of lontaminants does not n e c e s s a r i l y i n d i c a t e t h a t water poses a h e a l t h r i s k . More i n f o r m a t i o n about contaminants and p o t e n t i a l h e a l t h e f f e c t s can be obtained by c a l l i n g the EPAs Safe D r i n k i n g Water H o t l i n e at (800) 426-4791.

I n order t o ensure t h a t tap water i s safe t o d r i n k , EPA p r e s c r i b e s r e g u l a t i o n s which l i m i t the amount of c e r t a i n contaminants i n water p r o v i d e d by p u b l i c water systems. FDA r e g u l a t i o n s e s t a b l i s h l i m i t s f o r contaminants i n b o t t l e d water which nust provide the same p r o t e c t i o n f o r p u b l i c h e a l t h .

Some people may be more v u l n e r a b l e t o contaminants i n d r i n k i n g water than the general p o p u l a t i o n . Immuno-compromised persons such as persons w i t h cancer undergoing chemotherapy, persons who have undergone organ t r a n s p l a n t s , people w i t h HIV/AIDS or other immune system d i s o r d e r s , some e l d e r l y and i n f a n t s can be p a r t i c u l a r l y at r i s k from i n f e c t i o n s . These people should seek advice about d r i n k i n g water from t h e i r h e a l t h care p r o v i d e r s . EPA/CDC g u i d e l i n e s on a p p r o p r i a t e means t o lessen the r i s k of i n f e c t i o n by Cryptosporidium and o t h e r n i c r o b i a l contaminants are a v a i l a b l e from the Safe D r i n k i n g Water H o t l i n e (800-426-4791).

I f present, e l e v a t e d l e v e l s of lead can cause serious h e a l t h problems, e s p e c i a l l y f o r pregnant women and young c h i l d r e n . Lead i n d r i n k i n g water i s p r i m a r i l y from m a t e r i a l s and components associated w i t h s e r v i c e l i n e s and home plumbing. We cannot c o n t r o l the v a r i e t y of m a t e r i a l s used i n plumbing components. When your water has been s i t t i n g f o r several hours, you can minimize the p o t e n t i a l f o r lead exposure by f l u s h i n g your tap f o r 3 0 seconds t o 2 minutes before using water f o r d r i n k i n g or cooking. I f you are concerned about lead i n your water, you may wish t o have your water t e s t e d . Source of Water: ALEXIS To determine A l e x i s ' s s u s c e p t i b i l i t y t o groundwater contamination, a Well S i t e Survey, p u b l i s h e d i n 1990 by the I l l i n o i s EPA, and a Source Water Assessment Program, were reviewed. Based on the i n f o r m a t i o n contained i n these documents, seventeen p o t e n t i a l sources of groundwater contamination are present t h a t c ould pose a hazard t o groundwater pumped by the A l e x i s community water supply w e l l s . These i n c l u d e Harrison's Service, A l e x i s Motor Company, A l e x i s F i r e Equipment p a i n t i n g and below ground f u e l storage, V i l l a g e of A l e x i s V i l l a g e H a l l , unknown antique storage, Simpson's Repair Service, A l e x i s Lumber Company, A l e x i s E l e v a t o r , stockyard feed s t o r e , below ground f u e l storage at stockyard feed s t o r e , two unknown g r a i n storages. Remington T i r e s , Texaco, M&M A g r i c u l t u r a l Service, and a car wash. Based on i n f o r m a t i o n provided by A l e x i s ' s water supply o f f i c i a l s , the f o l l o w i n g f a c i l i t i e s , also i n d i c a t e d as p o t e n t i a l sources i n the s i t e data t a b l e , are no longer i n existence: A l e x i s F i r e Equipment, M&M A g r i c u l t u r a l Service, Simpson's Repair Service, Harrison's Service, A l e x i s Motor Company, Stockyard Feed Store, an unknown antique storage, an unknown g r a i n storage, and the C i t y of A l e x i s C i t y Hall.Based upon t h i s i n f o r m a t i o n , the I l l i n o i s EPA has determined t h a t A l e x i s Wells #1 and #2 are not s u s c e p t i b l e t o IOC, VOC, or SOC contamination. This d e t e r m i n a t i o n i s based on a number of c r i t e r i a i n c l u d i n g : m o n i t o r i n g conducted at the w e l l s ; m o n i t o r i n g conducted at the e n t r y p o i n t t o the d i s t r i b u t i o n system; and the a v a i l a b l e hydrogeologic data f o r the w e l l s . I n a n t i c i p a t i o n of the U.S. EPA's proposed Ground Water Rule, the I l l i n o i s EPA has determined t h a t A l e x i s ' s community water supply w e l l s are not v u l n e r a b l e t o v i r a l contamination. This d e t e r m i n a t i o n i s based upon the e v a l u a t i o n of the f o l l o w i n g c r i t e r i a d u r i n g the V u l n e r a b i l i t y Waiver Process: the community's w e l l s are p r o p e r l y c o n s t r u c t e d w i t h sound i n t e g r i t y and proper s i t e c o n d i t i o n s ; t h e r e i s a hydrogeologic b a r r i e r t h a t r e s t r i c t s pathogen movement; a l l p o t e n t i a l r o u t e s and s a n i t a r y d e f e c t s have been m i t i g a t e d such t h a t the source water i s adequately p r o t e c t e d ; m o n i t o r i n g data d i d not i n d i c a t e a h i s t o r y of disease outbreak; and the s a n i t a r y survey of the water supply d i d not i n d i c a t e a v i r a l contamination t h r e a t . However, having s t a t e d t h i s , the U.S. EPA i s proposing t o r e q u i r e States t o i d e n t i f y systems i n k a r s t , g r a v e l and f r a c t u r e d rock a q u i f e r systems as s e n s i t i v e . Water systems u t i l i z i n g these a q u i f e r types would be r e q u i r e d t o perform r o u t i n e source water m o n i t o r i n g . Because the community's w e l l s are c o n s t r u c t e d i n a c o n f i n e d a q u i f e r , which should p r o v i d e an adequate degree of p r o t e c t i o n t o prevent the movement of pathogens i n t o the w e l l s , w e l l h y d r a u l i c s were not considered t o be a s i g n i f i c a n t f a c t o r i n the v u l n e r a b i l i t y d e t e r m i n a t i o n .
