CHS is confident that this video conferencing technology will greatly enhance your ability to interview their SMI patients for supportive housing beds, prior to their transition from NYC DOC custody to the community. The steps to make a Telehealth call are simple: 1. CHS’ Social Work and Re-Entry staff emails the supportive housing provider a video conferencing link. 2. Once you click on the link, you will be guided to download a free software app, called “Cisco Jabber Guest.” This is a one-time download available for desktops, laptops, and mobile devices. 3. • All units are affordable to households earning under 50% of AMI

• All units are open to seniors aged 62+

Admission Process

WSFSSH staff, clinical coordinators, social workers, and other administrative staff will conduct

outreach to OMH field office, OMH Single Point of Access, OMH State Psychiatric Facilities,

Health Homes, NYC Department of Homeless Services, WSFSSH DOH licensed adult homes

and other adult homes, to identify high Medicaid users who may be ready to transition to a less

restrictive level of care. The admissions process will consist of a review of the initial application,

including a psycho-social and a psychiatric evaluation. If applicant appears to meet eligibility

criteria, an on-site interview will be scheduled. If applicant is still deemed eligible and is

interested in moving to Tres Puentes, a second interview will be set to complete paperwork,

obtain any additional information needed and/or answer additional applicant questions.

Services

WSFSSH staff will use a holistic, person-centered, strength-based assessment process to

engage each resident and develop a comprehensive approach to help each resident successfully

integrate into the Tres Puentes community. The steps involved in the process are engagement,

assessment, development of a Service Plan, provision of services directly by Tres Puentes staff

as well as from other community providers, periodic evaluation and adjustments to the initial

Service Plan. Briefly:

Engagement: Engagement will start at during the application process and continue through the

move-in process. Staff will provide orientation to the building and neighborhood and encourage

participation in program activities. Social work staff will spend time with resident and begin

assessment process.

TRES PUENTES: SOCIAL SERVICES

Assessment: The assessment looks at all areas of need, functioning and interest including

physical health, mental health, substance use (including tobacco dependence), trauma

experiences, socialization skills, habilitation skills, financial management skills, housing history,

family experience/support, education level, employment, volunteer activities and leisure

activities or interests particular to that resident. This will enable staff to have a holistic

understanding of the resident.

Development/Implementation/Ongoing Evaluation of a Service Plan: Based on the

assessment, resident and social worker will develop a Service Plan that assists the resident to

identify and meet goals that are important to the resident. The Plan will be person-centered,

contain specific services resident will access, clear steps for resident to implement and specific

outcomes/goals that will be accomplished. The social worker will provide case management

services that could include financial management and assist with referrals and connections to

services in the broader community, including psychiatric and medical services. The resident and

social worker will meet regularly to review Service Plan and adjust as needed.

Tres Puentes will also have an activity program that will provide opportunities for residents to

socialize, engage in recreational activities and pursue special skills or interests. The activity

program is seen as an additional support for residents, providing not only specific events and

classes, but through celebrations and similar events, the activity program will contribute to the

development of community among Tres Puentes residents. It has been the experience of

WSFSSH that helping residents develop a sense of belonging to the community is an important

component of efforts to improve quality of life and achieve housing permanency.

Outcomes

Though each resident will have personal measures of success, the overall goals of the Tres

Puentes program will focus on housing permanency, psychiatric, medical and financial stability.

New Activities of Daily Living Section in 2010e Application Effective July 27th, 2017, the Application Activities of Daily Living (ADL) section of the Supportive Housing Application (2010e) has been modified to gather important information on the level of support the applicant may need in supportive housing as a result of any medical or behavioral health conditions. The referring worker must select the type of support needed (Direct Assistance, Some Support, or Mostly Independent) for each of the ADL areas. The ADL areas are as follows:

• Personal Hygiene • Traveling/Mobility • Shopping & Meal Preparation • Manage Finances • Apartment/Room Upkeep • Social Skills/Supports • Manage Medical & Behavioral Health • Other (referring worker can document additional area)

Once approved for supportive housing, the information will be considered to determine the types of housing and services most appropriate for successful placement of the applicant.

Supportive Housing Approval Duration Increased The New York City Supportive Housing Task Force made several recommendations in an effort to improve and expedite access to Supportive Housing in NYC. Although not a recommendation in the final report, the process led to stakeholder representatives considering the benefits of increasing the 2010e approval duration from the current six-month period to one year. HRA in consultation with relevant NYC and NYS government agencies and supportive housing providers/membership organizations agreed that one year allows for efficiencies in the application process while ensuring the accuracy of the determination for appropriate placement opportunities for clients and supportive housing providers. Therefore, all HRA 2010e applications submitted after May 7, 2018 to the Placement, Assessment and Client Tracking (PACT) Unit will have an approval period of one year. Please note this change is prospective on the date of implementation and determinations made prior to this date will not change. HRA recommends if there is a significant change in the client’s service needs in the community and/or eligibility, e.g. client after a period of time homeless now meets chronic homeless criteria when they did not in prior approval, to submit a new application for review.

NYC Standardized Vulnerability Assessment

Effective January 23, 2018, all HRA 2010e applications that are approved for supportive housing will be reviewed utilizing the NYC Standardized Vulnerability Assessment (SVA). The NYC SVA will be used to readily assess and prioritize individuals and heads of households that are most vulnerable for referral to supportive housing. An individual and/or head of household’s vulnerability is indicated using the following categories: High, Medium and Low Vulnerability. Applications determined eligible for any category of supportive housing will include an SVA summary outlining the factors informing the assessment. This standardized assessment was developed in collaboration with relevant NYC and NYS government agencies, supportive housing providers, consumers and supportive housing membership organizations. The NYC SVA is in accordance with the U.S. Department of Housing and Urban Development’s (HUD) issuance of 24 CFR 578.7(a)(8) and subsequent notices requiring a centralized and/or coordinated assessment system. How does the NYC SVA impact an applicant’s referral to supportive housing?

Referrals to supportive housing programs receiving HUD funding as a part of the NYC Coalition on the Continuum of Care (CCoC) will be conducted in accordance to the CCoC’s written standards. To review the CCoC’s written please visit their website at www.nychomeless.com. Additionally, referrals to NYC 15/15 supportive housing programs will be prioritized utilizing the SVA.

Referring agencies completing supportive housing applications are strongly encouraged to complete the Understanding the NYC Standardized Vulnerability Assessment training prior to initiating a new application.
